Standardized Testing Registration Form

Please complete the information below and email back to modernhomeschooler@cox.net or mail to above address.

In order for the test results to be sent back to you directly, and not to me first, you will need to contact BJUP on your own and order the test. You will be asked to set up an account and pay with a credit card over the phone. You willthen be emailed a Homeschool verification form which you need to print, sign and either fax, mail or email (as anattachment) back to BJUP as soon as possible. The test will be sent to me as the test administrator, and I will let youknow when I have received it.If you don’t have access to a fax or a scanner to submit the form, or need assistance in ordering, please let me know right away. We can work out an alternative arrangement. Your tests will not be mailed until they have this form, so delay in sending it back will delay our receiving of the tests.

BJUP will ask for a testing date that is about 4-6 weeks away. In my experience, they will send the tests as soon asthe Homeschool Verfication Form has been processed in their office, and I receive the test much sooner than the 4-6weeks. (During the summer, I have gotten them in a week.)

It is imperative that you are able to complete the paper work by the date specified below in order for us to get thematerials on time for the given test date. If the form is not returned promptly and the shipment will definitely not arriveon time, it can be sent express delivery, which will be a $30 charge to your account.

Student information: The test cost for each student is $44 no matter who orders them. The administrator fee will bebased on the total number of students according to the chart below, and you will be sent an invoice for that amountwhen the registration has closed. (If HCHES orders the tests, that amount will be added to your invoice.)
