The t h r e e remaining members s h a l l be t h e c i t y h e a l t h o f f i c e r , t h e d i r e c t o r of community development and t h e b u i l d i n g i n s p e c t o r . To r e g i s t e r wi th t h e c i t y as a plumbing c o n t r a c t o r , a p p l i c a - t i o n s h a l l be made i n w r i t i n g on forms furnished f o r t h e purpose and f i l e d wi th t h e ch ie f b u i l d i n g i n s p e c t o r . The a p p l i c a t i o n s h a l l show t h e plumbing c o n t r a c t o r ' s name, l o c a l address and t e l e - phone number and such o t h e r infoxmation as may reasonably b e r e q u i r e d t o proper ly i d e n t i f y t h e plumbing c o n t r a c t o r .
