OneLegacy entered a new era with the July 1st launch of OneLegacy Tissue Services, which dramatically increases OneLegacy’s capabilities in all forms of donation for trans- plantation and medical research. “This is the result of more than a year’s preparation, designed to ensure a smooth transi- tion and opportunity for expansion of services to the community,” said Tom Mone, CEO of OneLegacy. At the outset, the department will provide tissue recovery services to hospitals, medical examiners/coroners and funeral homes in Riverside, San Bernardino and Kern counties. On April 28th, Secretary Thompson announced the HospitalBest Practices initiative as the sixth component of his program toincrease organ donation. The program seeks to spread best prac-tices to the nation’s largest hospitals to achieve organ donationrates of 75 percent or higher in these hospitals from a current aver-age of less than 50 percent.

Targeted by this initiative are 300 hospitals with the highestorgan donor potential; 19 of them are within the OneLegacy serv-ice area (see sidebar).

To support the initiative, the Department of Transplantationhas extended an opportunity for OPO-hospital teams to beinvolved in a structured action program in partnership with HHSand the Institute for Healthcare Improvement (IHI). The programinvolves designing, testing, implementing and diffusing lessonslearned—all with a view of seeking 75 percent conversion rates inthe primary target hospitals.

With the program limited to three participating target hospitalsper OPO service area, OneLegacy has extended invitations to LACounty-USC Medical Center, Kern Medical Center andProvidence Holy Cross Medical Center.

OneLegacy Spearheads Coalition on Donation Rose Parade Float

The Coalition on Donation, a not-for-profit alliance of local affiliates andnational organizations dedicated to inspir-ing all people to donate life through organand tissue donation, has been invited bythe Pasadena Tournament of Roses toenter a float in the 2004 Rose Parade. Theprized invitation is the result of an 18-month organizing effort spearheaded by

Coalition member OneLegacy, which isjoined by 37 partners from across thenation, including the Coalition onDonation, 22 organ/tissue recovery organ-izations, nine industry partners, and threeSouthern California transplant centers.

Carrying the theme A Symphony ofLife, the float will feature a dozen donorfamily members and transplant recipientswalking together across a beautiful gar-den bridge. A trellis running the length ofthe bridge will support a variety of inter-twining florals, while two rowboats underthe bridge will carry recipients and fami-lies of their organ donors.

"The Coalition on Donation’s RoseParade float dramatically illustrates theinspiring, life-transforming connectionbetween organ and tissue donors, their

families and transplantrecipients," said DavidFleming, ExecutiveDirector of the Coalitionon Donation. "We hope ourparade float will help peo-ple see organ and tissuedonation in a positive lightand inspire people to dis-cuss donation with theirfamilies."

This spring, nearly 1.5 million Southern California moviegoerswere exposed to organ and tissue donation messages in a reprise of acinema advertising campaign in six Krikorian Theatres multiplexesthroughout the region.

The public awareness campaign, a partnership of OneLegacy,Cassie’s Friends and the San Bernardino County Medical Society, ranfrom mid-April through mid-July on 72 screens in Buena Park, ChinoHills, Downey, Rancho Mirage, Redlands and San Clemente.Krikorian Theatres generously donated one month of on-screen timein support of the campaign.

The eye-catching slides, three of which appeared intermittentlyprior to each screening, honored four OneLegacy donors and theirfamilies: Johnny Loredo, Dr. Daw Khin Myint, Bonnie Newman (seePage 1) and Ge’Juan Sallee. On each slide, a photo is accompanied bya quote from their family, the "Donate Life" logo, and the probingquestions: "Are you an organ & tissue donor? Have you told yourfamily?"

"When people go to movies they are usually with family or closefriends," said Bryan Stewart, communications director at OneLegacy."We saw this as a perfect place to initiate a conversation about donation.After all, sharing one’s decision is just as important as making one."

The program partners hope to make this an annual campaign timedto coincide with the release of the summer blockbusters.

Nationwide, hospitals are updatingtheir agreements to comply with the pri-vacy regulations contained in the HealthInsurance Portability and AccountabilityAct (HIPAA). The core functions of anorgan procurement organization (OPO)are subject to two regulatory exemptionsin the final HIPAA privacy regulations.

First, a health care provider may useor disclose information if and as requiredby law. This exemption allows OPOs andhospitals to comply with the MedicareConditions of Participation, which specif-ically authorize referrals and recordaudits.

The second and most broad exemptionis found at §164.512(h) which allowsinformation to be released to organ pro-curement organizations or other entitiesinvolved in the procurement, banking ortransplantation of cadaveric organs, eyesor tissue for the purposes of facilitatingorgan, eye or tissue donation and trans-plantation. This allows the release ofinformation by and to, donor hospitals,transplant hospitals, UNOS, tissue banksand laboratories.

Pursuant to these two exemptions,hospitals do not need to obtain patientconsent for OPOs to do their core jobs:the coordination of donation and trans-plant, and the review of records.Furthermore, HHS responses to com-ments on the regulations clarify that

OPOs are not ‘business partners’ of hos-pitals. The response states, in pertinentpart: "...organ procurement organizationsand tissue banks are generally not busi-ness associates of hospitals."

Thus, OPOs do not need to enter into"business partner agreements" with hospi-tals, unless they are acting as somethingother than OPOs. OneLegacy’s AffiliationAgreement does not contemplate that we

act in any capacity other than as an OPO.Finally, in the preamble to the final rule,HHS states that OPOs are not "health careproviders" when they are engaged in theprocurement or banking of organs, bloodor tissues. Thus, with regard to hospitalaffiliations, OPOs are neither coveredentities nor business partners, and arespecifically permitted to perform theircore functions, with stringent confiden-tiality, but outside the ambit of HIPAA.

OneLegacy will be updating each hos-pital’s Administrative and Organ Donormanuals with the HIPAA regulationsdealing with the exemptions for organprocurement organizations. The adminis-trative manuals are supplied to the CEOof each hospital and the organ donor man-uals are offered to each critical care unitand emergency room.

HIPAA Regulations Include Two Exemptions for OPOs

The ER/ICU nurse plays an essentialrole in the organ and tissue donationprocess. There are a number of proactivesteps the nurse can take to ensure the fam-ily’s right to consider the opportunity todonate. They include:

Patient Care

• Maintain vital signs, including bloodpressure, heart rate and normal tem-perature

• Maintain pulmonary care as if thepatient were expected to recover (pO2> 100), even if a DNR or brain death

• Obtain requested/necessary organ-specific labs

• Ensure adequate urine output and fluidresuscitation

• Notify OneLegacy of any changes inthe patient’s status

Family Care

• Do not mention organ donation to thepatient’s family. (Premature notifica-tion has been shown to reduce familytrust in the hospital and caregivers.)

• Identify the family’s native/nurturinglanguage

• Notify OneLegacy of any visits byfamily members

For the first time, organ donors and their families throughoutOneLegacy’s seven-county region were honored at four donor recog-nition ceremonies held across the Southland in March.

The theme of the ceremonies, Fields of Gold, was inspired by therecording of Sting’s song of the same name by the late Eva Cassidy.

“By all accounts, families found the events to be an uplifting andmeaningful tribute to their loved ones,” said Jeff Fleming, Manager ofFamily Services for OneLegacy.

Altogether, more than one thousand donor family members hon-ored 170 loved ones at the four ceremonies.

Cedars-Sinai MedicalCenter celebrated NationalDonate Life Month with theirMulti-Organ Donor/RecipientFun Run/Walk and ReunionPicnic. The April 26th eventwas attended by 120 transplantpatients, friends and familyalong with staff from Cedars-Sinai, OneLegacy and the AmericanLiver Foundation.

Vibrant transplant attendees and speakers gave powerfulreminders of what National Donate Life Month is all about. The African American HealthInitiative (AAHI) is a community-basedresource focused solely on improvinghealth among the African American com-munity of San Bernardino County. Theinitiative is a partnership of the SanBernardino Medical Society, the SanBernardino County Department of PublicHealth, Inland Wellness InformationNetwork (IWIN), and over 30 organiza-tions throughout the county.

The mission of AAHI is aimedat “reducing and eliminating healthrisks and improving overall healthquality among the AfricanAmerican population in SanBernardino County by buildingcoordinated health systems.”Towards achieving its overall mis-sion, AAHI has set several man-dates for change in the health risksof the African Americans, includ-ing increasing by 3% the number ofAfrican Americans who pledge tobe organ and tissue donors, as wellas increase the incidence of diag-nosed and controlled hypertensionto minimize stroke, renal failure,

and/or congestive heart failure among theAfrican American population.

AAHI’s Organ and Tissue Donationcommittee, which is co-chaired byOneLegacy employees Tamika Hams andMichelle Harris, focuses on booths athealth fairs and other community eventsto educate the public so they can make aneducated choice about donation.

African American Health InitiativeMay saw the launch of the Hispanic

division of the OneLegacy Ambassadorsvolunteer program. Like their English-speaking counterparts, the Embajadoresde OneLegacy willhave a wide variety ofroles, programs andcommitment levelsfrom which to choose.

"The Embajadoresare a key element ofour Hispanic outreach program," saidDahiana De Francisco, HispanicCommunications Coordinator forOneLegacy. "As we develop opportunitiesto educate the primarily Spanish-speakingcommunity, the Embajadores will beessential to our efforts."

Some of the most innovative outreachefforts in the Hispanic community areSpanish-language presentations to adulteducation centers, trade schools and par-ent workshops. "Addressing people intheir communities allows us to fullyanswer the questions and concerns raisedin the community," says De Francisco.

The inaugural Embajadores orienta-tion, which featured an overview of thewaiting list by OneLegacy organ place-ment coordinator Teresita Nuila, RN, wel-comed seven Spanish-speaking volunteers:

Mone. "OneLegacy intends to maintain the high standards of tis-sue recovery service that have been offered by IETB." In order tocontinue this level of service, OneLegacy has extended offers toall key IETB personnel to join OneLegacy; most are expected toaccept this invitation.

Overseeing the new OneLegacy Tissue Services Department isAlan Cochran, Director of Tissue Services for OneLegacy, whomost recently was the National Director of Donor Services andOPO Relations for American Red Cross Tissue Services. Alongwith his incoming colleagues from IETB, he will focus the depart-ment’s efforts on bringing personalized attention to donor familiesas well as hospitals, coroners and funeral homes.

"Our goal is to increase tissue recovery from the current level,which hovers around 60% of the national average," said Cochran."We are confident that with the combination of IETB’s trackrecord and OneLegacy’s close relationships with area hospitalsand medical examiners/coroners, we will increase tissue donationdramatically."

The field of tissue donation and transplantation has undergonedramatic development in the past five years, marked by the devel-opment of "engineered grafts" that replace metal screws, wedgesand joints for spinal and joint surgical procedures.

OneLegacy Tissue Services will recover, process and distributeocular tissue including corneas and sclera to ophthalmologists. Inaddition. recovered musculoskeletal, cardiovascular and skin tis-sue will be provided to the most progressive tissue processors fordistribution, thus ensuring that orthopedists, neurosurgeons, burnsurgeons, dentists and other physicians who use donated tissue totreat patients in Southern California receive the finest and safestgrafts available.

OneLegacy Tissue Services will base its operations inRedlands, home of OneLegacy’s Inland Empire regional office

Bonnie’s life hastouched so many peopleand now she’s able to givelife to others. I found outthis morning that herorgans went to three otherpeople. We also did tissuedonation for corneas. Shehas such beautifulsparkling eyes, it’s wonderful to think thatshe’ll be giving sight to someone else thatcan look through the beauty of her eyesand how she saw life. It’s good to knowher spirit and energy and love for life getsto live on in other people and bring hap-piness to their families. I know that nowshe’s left the natural life, she’s dancingand laughing and singing in her heavenlybody and is free from the pain of life.

I want to share an experience we hadat the hospital. Bonnie was pronouncedtechnically brain dead on October 15th,and the hospital staff kept her on a venti-lator so this donor program could work atits highest level. I was the one that had theprivilege of holding Bonnie’s hand as werolled her hospital bed to the operatingroom to say goodbye to her. I couldn’tleave the hospital without standing in theparking lot waiting for the helicopter totake off and take Bonnie’s gift of life tosomeone else. We gathered in the parking

lot, there must have been 15 or 20 of us,and Tracy deGraaf from OneLegacy, whohad been in the operating room and keep-ing us posted on everything, came out andstood with us. As the helicopter blades

started to revolve on top of thehospital building, we could seethe red lights reflecting in theblades. It made sort of a redneon star, kind of an angelicthing. The blades started rotat-ing faster and faster and final-ly lifted off the hospital roofand flew off to the west forUCLA to give life to some-body else. We stood there andshouted out and waved good-

bye to Bonnie: "We love you! We missyou!" What a beautiful tribute to be ableto say goodbye to her.

In life, Bonnie didn’t give birth to herown child, but in her death she gave life toothers. There’s nothing greater that we cando, to allow our lives to live on in thehearts of others and in the bodies of others.

I know that this has changed my per-sonal thoughts about being an organdonor. I have no fear of it now. I’m goingto fill out my form and make sure myfamily knows so that they don’t have tomake these difficult decisions withoutknowing my wishes.

Bonnie contributed so much to us inher life, and she’s going to continueteaching us lessons in how we honor herspirit and her memory.
