Introduction

• Elderly people generally have lower levels of physical fitness, which can impair their functional performance .

• In South Korea, falls account for 13% and 21% of the injuries among the elderly aged 65 years or above and 70 years or above, respectively.

• Elderly people who have had a fall are fearful of having one again, resulting in decreased physical activity. This leads to a further decrease in function, accelerating the deterioration of their physical fitness.

• There are several fall prevention exercise programs in various formats, muscle strength, flexibility, and balance, elastic- band exercise, and Tai chi quan.

• This study was conducted to compare the effectiveness of resistance and balance exercise in reducing the risk of fall for females 65-years-old or older with prior fall experiences within a one-year period.

Study design and participants

• A total of 152 women aged over 65 years were recruited from three senior welfare centers.

• The inclusion criteria were over 65-years-old, able to ambulate at least 10 m independently , having at least one fall experience within the past year, inability to stand on one leg for 5 seconds or more.

Randomization and study group

• After the baseline measurement, 95 study participants were randomly assigned to one of three groups: the resistance exercise, balance exercise, and control groups.

Outcome measurements

• The static balance was assessed using the one-legged stand (OLS) test.

• The dynamic balance was assessed using the Figure-of-Eight Running (FER) test.

• Functional mobility was tested through the Timed Up and Go (TUG) test.

• The proprioception was assessed based on the joint position sense (JPS).

• The maximum strengths of the quadriceps were divided by the weight of the participants (%MVC).

• The participants’ fear of falling was evaluated using Tinetti’s Fall Efficacy Scale Korean version (FES-K)

Conclusion

• Resistance and balance exercise had a significant influence on proprioception and balance ability, which in turn are believed to exert a positive influence on the falls efficacy.

• Moreover, the dynamic-balance (FER, TUG) ability in activities of daily living improved more significantly in the resistance exercise group than in the balance exercise group.

• In contrast, the improvement in proprioception was significantly higher in the balance exercise group than in the resistance exercise group.

• Based on these results, it is believed that a better fall prevention exercise program can be formulated if different exercise patterns based on exercise specificity will be adequately applied to elderly people who have had falls and who are undergoing fall preventio exercises.
