2010 - 2012 Do Polymorphisms in TGF-1 gene Predispose Patients for the Development of Nephrogenic Systemic Fibrosis?

MGH Pathology Translational Research Grant, Austin L. Vickery Award Principal Investigator ($1,658.35) The goal of this study was to examine TGF-1 gene polymorphisms at codons 10 (T/C)

and 25 (G/C) for association with nephrogenic systemic fibrosis in patients with renal impairment and previous exposure to gadolinium.

2011 - 2012 Wnt and Hedgehog Signaling Pathway Activation in Cutaneous Fibrosing Disorders: a

Tissue Microarray Study Global Fibrosis Foundation, First Junior Investigator Award Principal Investigator ($10,000) MGH Pathology Mini-grant Principal Investigator ($2,000) The goal of this study was to screen for Wnt and Hedgehog signaling pathway protein

overexpression in keloid, hypertrophic scar, scleroderma and nephrogenic systemic fibrosis to offer a rationale for the use of Wnt and/or Hedgehog pathway inhibitors in fibrosing disorders of the skin. These results may potentially be generalized to fibrosing disorders involving other organ systems.

2011 - 2013 Characterization of T helper Cell Transcription Factors and Cytokine Signatures in

Acute Erythroderma and Chronic Dermatitis Eleanor and Miles Shore 50th Anniversary Fellowship Program in Medicine Principal Investigator ($25,000) The aim is to confirm the Th1- and Th2-skewed immune phenotype of psoriasis and

atopic dermatitis, respectively, and determine whether clinically acute exacerbations reflect loss of immune polarity towards Th17 expression predominance. The findings may form the basis for future clinical trials and therapeutic strategies aimed at shifting the Th1/Th2 balance during acute disease exacerbations to improve patient morbidity and mortality profiles.

2011 - 2016 The Vitamin D Receptor: Ligand-Dependent and Independent Actions National Institutes of Health R01 DK46974 Co-investigator (PI: Marie Demay, MD) My collaborative role in this study is to explore aberrant cutaneous wound healing in

Vitamin D receptor deficient mice using histopathology and immunohistochemical studies.

2013 - 2015 The effect of OASIS® Ultra on Critical Sized Wound Healing Healthpoint Biotherapeutics Co-investigator (PI: Daniel Yeh, MD) The aim of this study is to evaluate the speed and quality with which the dermal

substitute, OASIS® Ultra (tri-layer porcine small intestinal submucosa extracellular matrix), increases wound healing in the critical sized defect. My role is to histologically assess the amount of cellular recruitment, collagen deposition and neovascularization

present in serial wound-edge patient skin biopsies. The composition of wounds with the dermal substitute and control wounds will be analyzed for healing time, healed wound quality (e.g. re-establishment of a dermis for further skin graft application), and hospital stay as compared with standard of therapy.

2014 - 2016 mTOR, VEGF, and c-kit Signaling Pathway Molecules in Kaposi Sarcoma MGH Pathology Translational Research Grant, Austin L. Vickery, Jr. Award Principal Investigator ($5,000.00) Current therapeutic options for advanced Kaposi sarcoma include radiation,

chemotherapy, and interferon-alpha, which are limited by low efficacy and high patient toxicity. Activation of mTOR, VEGF and c-kit signaling pathway molecules have been implicated in Kaposi sarcoma pathogenesis and may suggest a novel therapeutic role for inhibitors, including Rapamycin and Gleevec. Via this international research collaboration with the Aga Khan Hospital (Kisumu, Kenya), we analyze protein expression in 250 Kaposi sarcoma patients using a tissue microarray approach.

2012 - 2017 Role of Vitamin D analogues, Stanniocalcin and Statins in the Development of Calcific

Uremic Arteriolopathy MGH, Department of Medicine, Renal Unit Funds Co-investigator (PI: Ravi Thadhani, MD) My collaborative role in this study is to evaluate cutaneous expression of stanniocalcin

1 (STC1) in patients with and without calciphylaxis; aid in the evaluation of vitamin D analogue administration and development of calciphylaxis in hemodialysis patients, and use of statins, agents which have anti-calcification, anti-thrombotic, and anti-inflammatory properties, in calciphylaxis (calcific uremic arteriolopathy) patients.

2014 - 2017 Phase I Trial of LDE225 for Steroid-Refractory cGVHD After Allogeneic HSCT Novartis Corporation (#LDE225XUS16T) Co-investigator (PI: Yi-Bin Chen, MD) Collaboration in correlative studies of a Phase I clinical trial using a Hedgehog pathway

inhibitor (LDE225, smoothened antagonist) in the treatment of refractory chronic graft-versus-host disease (cGVHD) in patients following allogeneic hematopoietic stem cell transplantation (HSCT). My role is to investigate pre- and post-treatment patient skin biopsies for the degree of pathologic fibrosis seen in cGVHD as assessed by myofibroblast mesenchymal cell proliferation and assayed for protein expression of Hedgehog signaling pathway molecules using immunohistochemistry. The safety and efficacy of LDE225 will be assessed for therapeutic response in cGVHD and shortened duration of corticosteroid therapy.

2015 - 2017 Does TNF- inhibitor therapy modulate Keratin 17 and ameliorate psoriasis? MGH Pathology Translational Research Grant, Austin L. Vickery, Jr. Award Principal Investigator ($5,000.00) TNF- binding to keratinocyte cell surface receptor, mediated by effect of keratin 17 on

TNF- receptor associated death domain (TRADD), activates transcription of downstream vascular cell adhesion molecule-1 (VCAM-1) which is recruited to endothelial cells by intercellular adhesion molecule-1, (ICAM-1) in response to proinflammatory cytokines. Keratin 17 expression in activated keratinocytes has been shown to induce Th1 and Th17 cytokine production which have been reported to play an etiopathogenic role in psoriasis. We examine the Th cell immune phenotype,

epidermal keratin 17 expression, and endothelial cell ICAM and VCAM expression in

patients with TNF- treated psoriasis and untreated psoriasis as compared to patients

with TNF- induced psoriasiform dermatitis, and atopic dermatitis. These investigations will expand our understanding of the role of keratin 17 in psoriasis pathophysiology and

TNF- antagonist mechanism of action, and may implicate keratin 17 as a marker for monitoring clinical treatment response.

2016 - 2018 Polarized immune infiltrates in localized scleroderma and eosinophilic fasciitis – a

potential marker for histopathological differentiation? MGH Pathology Mini-Grant Program for Clinical Research Principal Investigator ($2,000.00) Localized scleroderma and eosinophilic fasciitis are rare fibrosing disorders that can

pose a diagnostic challenge. Both are postulated to be autoimmune, with T-cell mediated immunity playing a major pathogenic role. Animal and serological studies have shown polarized Th1 and Th2 immune responses in localized scleroderma and eosinophilic fasciitis. Limited data is available on the role of the more recently identified helper T-cell subtypes, Th17 and Th22, in fibrosis. We investigate whether T-cell immune polarization enables histopathologic distinction between localized scleroderma and eosinophilic fasciitis in patient skin biopsy specimens. Our findings may inform anti-fibrotic therapeutic strategy.

Unfunded Current Projects

2010 - present Telepathology consultations in Dermatopathology, Massachusetts General Hospital Principal Investigator The object of this work is to provide diagnostic support to pathologists in resource-poor

settings wherein access to expert subspecialty second-opinion consultations are limited by cost and lack of subspecialty training.

2012 - present Cambridge Hospital Alliance Digital Pathology Consultation Pilot Study, Massachusetts

General Hospital Participant The goal is to assess the diagnostic accuracy and feasibility of whole slide imaging for

interinstitutional consultations in dermatopathology as compared with clinical glass slide diagnosis.

2015 - present Immune-Related Trafficking and Signaling in Human Skin Associated with Low-Power,

Infrared Laser Treatment Co-Investigator, (PI: Mark Poznansky MD, PhD) The aim is to demonstrate that a non-painful and non-damaging dose of continuous

wave near infrared laser results in alteration of cytokine and chemokine signaling and a significant increase in the number of dendritic cells as assessed by histological and immunohistochemical analysis of human laser-treated and control skin punch biopsies.

2016 - present Wnt pathway expression in keloid and hypertrophic scarring: further evidence of a

wound healing continuum? Principal Investigator To determine whether fundamental differences exist in Wnt signaling pathway proteins

in abnormal healing resulting in keloids and hypertrophic scar formation.

First Quantitative Approach to the Histopathologic Diagnosis of NSF

First-authored a publication reporting the first quantitative approach to the histopathologic diagnosis of Nephrogenic Systemic Fibrosis (NSF) using a simple, low-cost method based on enumerating dermal cellularity. A diagnostic cut-off range was established and no association between dermal cellularity and patient age, gender, duration of skin lesions, level of plasma creatinine, and symptom-preceding or cumulative gadolinium containing contrast agent dose was found suggesting that quantitative assessment of dermal cellularity is a reliable diagnostic aid in NSF cases.

Adoption of a Restrictive Gadolinium Policy at MGH Helps Prevent NSF

Co-authored a frequently cited publication describing the reduced incidence of NSF following the adoption of a restrictive gadolinium policy at MGH, a major public health achievement. After implementing strict guidelines on the

use of gadolinium containing contrast agents for magnetic resonance imaging studies in patients with impaired renal function, we reported no new cases of NSF in MGH patients suggesting that this strategy effectively prevents the occurrence of this potentially lethal, rapidly progressive systemic fibrosing disorder.

Application of Static- Image Telepathology in East Africa

Senior-authored a study establishing the use of a static-image telepathology system for dermatopathology consultations in East Africa. This MGH-based study revealed that telepathology is a feasible means of rendering diagnoses on dermatopathology cases and is a cost-effective technology for obtaining much needed second opinions in resource poor settings.

Histopathology Quality Improvement Initiative at MGH

Improved the histopathologic evaluation of onychomycosis by a simple, rapid, cost-effective strategy utilizing a 20% sodium hydroxide (NaOH) pretreatment technique for processing the nail plate, a frequently problematic histologic specimen. This technique led to improved quality of tissue sections reducing the need for repeat sections, thereby reducing costs and preventing delays in turnaround time, and has been adopted nationally.

Histologic scoring parameters proposed to characterize wound healing

Identified a modified method for the histological assessment of cutaneous wound healing utilizing parameters of acute inflammation and tissue repair, as standardized criteria for the quantitative assessment of cutaneous wound healing remain undefined. Employing this practical, semi-quantitative scoring system enables rapid evaluation of suboptimal wound repair or advanced wound healing in serial skin biopsies without necessitating costly immunohistochemical stains.

Immunologic underpinnings of erythroderma revealed

Clinically, acute exacerbations of psoriasis and atopic dermatitis can result in erythroderma, a severe dermatitis characterized by diffuse erythema and scaling that can be life-threatening. Although psoriasis and atopic dermatitis show clinical and histopathological differences during chronic disease, they are difficult to distinguish during erythrodermic exacerbations. This senior-authored investigation published in JAMA Dermatology reveals an immunologic overlap of helper T-cell subtypes 17 and 22 in erythrodermic psoriasis and atopic dermatitis that belies the clinically and histopathologically indistinguishable erythrodermic state.

T-helper immune phenotype a clue to

‘paradoxical’ anti-TNF- therapy related psoriasiform dermatitis

Therapeutics targeting TNF-are effective for psoriasis, however, their mechanism of action has not been fully elucidated. Paradoxically, a subset of patients with psoriasis may worsen and psoriasiform dermatitis may arise de novo in patients treated for other disorders, such as inflammatory bowel disease. This senior-authored work demonstrates a skewed immune phenotype with Th1:Th2 reduction and Th17:Th1 increase in this patient

subset and supports a rationale for therapeutic targeting of IL17 and TNF- in combination.

Narrative Report I am a full-time Dermatopathologist in the Pathology Service at Massachusetts General Hospital. I completed my residency training here in anatomic & clinical pathology in 2008, followed by a Dermatopathology fellowship in the Harvard Combined Program prior to joining the Harvard Medical School faculty in 2009. I have carved a unique niche in the pathology of medical (non-neoplastic) dermatology encompassing inflammatory and fibrosing disorders of the skin. My goal is to expand on my established national reputation as an expert physician scientist providing insights into the pathogenesis and potential therapeutic avenues for inflammatory dermatoses and cutaneous fibrosis. In addition to clinical service and teaching responsibilities (80% of my professional effort), the position affords me dedicated time and resources to further develop this academic interest internationally. In addition to my clinical responsibilities as a full-time dermatopathologist diagnosing a high-volume of inpatient and outpatient cases, I serve as a consultant in the diagnosis of Nephrogenic Systemic Fibrosis (NSF), eosinophilic fasciitis, morphea, calciphylaxis, aberrant wound healing, and drug-related dermatitides, such as vasculitis. My keen interest in elucidating the pathophysiologic basis for inflammatory and fibrosing disorders has led to 30 peer-reviewed publications (including 13 senior-authored reports and 3 first-authored reports, with innovation of a quantitative method for the diagnosis of NSF), 4 book chapters, over 20 abstracts presented at national and international meetings, and 14 national and international invited lectures. As the first Junior Investigator Award recipient of the Global Fibrosis Foundation, I investigated TGFβ, Wnt and Hedgehog signaling pathway activation in NSF, scleroderma, hypertrophic scar, and keloid that may inform future targeted therapies. Funded by the Vickery Pathology Translational Research Grant, I studied whether TGFB1 gene polymorphisms associated with development and progression of fibrosis in several organ systems predispose patients for NSF. As an Eleanor Shore Fellowship award recipient, I investigated T-cell transcription factors and cytokine signatures in acute erythroderma and chronic dermatitis (psoriasis and eczema) in collaboration with Internal Medicine colleagues. More recently, I have furthered the findings in these investigations resulting in subsequent works that demonstrate novel insight into the ‘paradoxical’ relationship between

psoriasiform dermatitis and therapeutic blockade of TNF and offer a rational for Th1/Th2 immune regulation and Th17 inhibition in anti-fibrotic therapeutic strategy. My efforts have been recognized by selection of presentations for awards at national meetings, health news articles highlighting my research findings, recognition of published work among “Most Cited Articles” indices, and a Partners in Excellence Award for Leadership and Innovation. My publications to date have achieved a citation index of 21 (H-

index). I have also developed close ongoing collaborations with key investigators in Endocrinology exploring aberrant cutaneous wound healing in Vitamin D receptor deficient mice, investigators in Nephrology exploring calciphylaxis (a rare and enigmatic vasculopathic systemic disease), and investigators in Surgery studying histopathologic alterations in cutaneous wounds. I have served on the Medical Advisory Council of the Global Fibrosis Foundation since 2010 striving to support research in the prevention and treatment of fibrosing disorders, and have recently been elected President. My educational activities range from instructing in the former HMS Dermatology (IN757) and Immunology, Microbiology & Pathology (IN754) courses, to teaching gross and microscopic dermatopathology to HMS students, residents, and fellows, to invited lectures at national and international meetings. These efforts have been rewarded with superb evaluations for the quality of my teaching contributions. I have mentored seven residents in their intradepartmental Molecular Pathology and Gross Pathology Research Conferences and was an NIH Career Development Award Advisor to an MGH post-doctoral fellow. My mentorship has led to 12 peer-reviewed publications, 8 book chapters, and over 20 abstracts, several of which have been recognized with awards. I have developed Dermatopathology educational materials including an Immunofluorescence primer for MGH trainees, MGH Fellows Handbook, and a curriculum-based glass-slide teaching set. Locally, regionally, nationally, and internationally I participate in educating my peers through invited lectures at CME courses and the annual meeting of the American Society of Dermatopathology and the American Academy of Dermatology, including as forum leader. My other activities include service as Associate Editor for the Journal of Cutaneous Pathology (medical dermatology, drug reactions, and inflammatory diseases section), Editorial board member of the American Journal of Clinical Pathology, ad hoc reviewer, and fostering quality improvement initiatives, including an approach to nail-plate processing published in JAAD, and interdepartmental clinicopathological correlations. I serve on the Continuing Education and Research Committee of the American Society of Dermatopathology. I am also a contributing member of the International Calciphylaxis Consensus Panel. Locally, I judge the Massachusetts State Science and Engineering Fair encouraging academically precocious high school students to enter science and technology careers. Regionally, I participate in pipeline initiatives involving Massachusetts Institute of Technology students and Boston public high school students to generate awareness of and interest in diverse careers in medicine. Globally, I volunteer as teledermatopathology consultant providing web-based diagnostic and educational support to physicians practicing in East Africa through Remote Access for Health Professionals, an international program funded by the Aga Khan Development Network, which has led to two publications and a recent senior-authored investigation on molecular pathways underlying HIV-associated Kaposi sarcoma published in Human Pathology. In addition, I direct programs in diversity and inclusion as a Departmental Diversity Leader. The supportive environment of the MGH Dermatopathology Unit, the resources provided by the MGH Pathology Service, physician leadership training I have received through the Massachusetts General Hospital Physicians Organization, and the expertise shared by my clinical collaborators has allowed me to establish dermatopathology of inflammatory and fibrosing disorders as my academic area of excellence. To my knowledge, this represents the sole medical dermatopathology area of expertise among dermatopathologists throughout the teaching institutions of the Harvard Medical School. Through my accomplishments to date, I have demonstrated an enduring commitment to academic dermatopathology with established excellence in teaching, clinical care and research activities, and have developed a regional and national reputation for my contributions to the understanding, diagnosis and treatment of inflammatory and fibrosing diseases in the skin.
