OUR APPROACHPeople visit The London Rectal Clinic, at 108 Harley Street, for a variety of reasons; most commonly, they have been referred by a medical practitioner because an abnormality has been noticed and a surgical opinion is required.

The clinic is dedicated to minimising patient anxiety. The colorectal consultants are currently available on Thursday and Fridays, so appointments can usually be offered within a maximum of 72 hours.

The clinics held in our relaxed and friendly premises, are designed to lead to a rapid, yet accurate, diagnosis. During an initial meeting with the consultant surgeon, a thorough history will be taken followed by expert physical examination. Facilities are available onsite, at 108 X-Ray and Imaging, should x-ray or ultrasound investigation be required. Other tests can be undertaken at partner facilities.

Once diagnosis has been confi rmed, appropriate treatment can be recommended. This treatment will be discussed and agreed with each individual patient, it may involve surgical intervention, or ongoing medical management of the condition.

The Consultant team at The London Rectal Clinic offers the best possible assessment of colorectal and general surgical conditions with the support of radiologists, pathologists, oncologists, nurses and other experienced health professionals;

CONSULTANT SURGEONMr Gordon Buchanan MD MSc FRCS (Gen) Gordon Buchanan is Consultant Colorectal and General Surgeon at Charing Cross and Hammersmith Hospitals, London. He is experienced in carrying out a wide variety of Colorectal and General Surgical procedures.

He graduated from St. Thomas’ Hospital, London and undertook colorectal and general surgical training in South East England. He also received advanced colorectal training at St. Mark’s Hospital, Harrow, St. Thomas’ Hospital, London and Mount Sinai Hospital, Toronto, Canada.

During additional laparoscopic colorectal training fellowships, in Colchester and Yeovil, Gordon Buchanan undertook laparoscopic appendicectomy and day case laparoscopic cholecystectomy, as well as benign and malignant laparoscopic colorectal resection. He also undertook research at St. Mark’s Hospital and Imperial College, London. He has published papers on many aspects of colorectal surgery, in particular investigation of anorectal conditions and fi stula-in-ano. He is presently undertaking collaborative research on laparoscopic resection of colorectal cancer.

CONSULTANT SURGEONMr Emin Carapeti MBBS MD FRCS (Gen)Emin Carapeti is Consultant General and Colorectal Surgeon at Guy’s and St. Thomas’ Hospitals. He is a Member of Council of the Section of Coloproctology of the Royal Society of Medicine, a member of the Association of Coloproctology of Great Britain and Ireland and an examiner in surgery at the Royal College of Surgeons of England.

He graduated from Guy’s Hospital, London and undertook his training in London and South East England. His specialist training was at St. Mark’s Hospital in London and Mount Sinai Hospital in Toronto, where he developed his interest and expertise in the management of complex infl ammatory bowel disease and anorectal disorders.

His thesis and main research was in the treatment of haemorrhoids and fi ssure-in-ano at St. Mark’s Hospital and he was awarded the Doctor of Medicine (MD) degree in 2000. He has published widely on anorectal disorders and pilonidal sinus disease and has pioneered new surgical techniques in complex recurrent pilonidal disease and day case surgery for haemorrhoids.

His special interests are rectal bleed, colonoscopy, colorectal cancer, management of fi ssures, haemorrhoids and pilonidal sinus disease, and he is a specialist in the surgical treatment of Crohn’s disease and ulcerative colitis.

CONSULTANT SURGEONMr. Ahmed R Ahmed BSc(Hons) FRCS (Gen)

Mr Ahmed is a Consultant Surgeon at Charing Cross Hospital, Imperial College London. He set up and heads the bariatric and metabolic surgery unit which is now a leading unit in the UK designated as an NHS preferred provider and provisionally recognised as an International Bariatric Surgery Centre of Excellence ASMBS.

He trained at University College London and Middlessex Hospital School of Medicine. Prior to his Consultant appointment he undertook extensive training in laparoscopic (keyhole) surgery in both the UK and USA.

Ahmed’s main areas of interest are;• Laparoscopic weight loss / bariatric surgery• Laparoscopic general surgery eg gallbladder / hernias etc• Emergency surgery eg appendictomy• Endoscopy

MULTI-DISCIPLINARY TEAM& MEDICAL ADVISORY COMMITTEE

Every patient’s case is assessed, managed and audited by the Consultant Team at 108 Harley Street; all are leaders in their fi eld and specially selected for their expertise and experience in different aspects of disease and cancer care.

The Consultants all work with our specialist nurses and other support staff in integrated multi-disciplinary teams, to ensure that each patient receives the best possible treatment required for their own particular problem.

In addition to the routine day to day team working, there are regular meetings which cover clinical, best practice and educational updates and reviews, as well as detailed case audit.

The Medical Advisory Committee regulates the admitting rights and the clinical governance of Consultants working at 108 Harley Street, as well as disciplinary matters.

WHAT TO EXPECT I RECEPTION

YOUR FIRST VISIT TO THE LONDON RECTAL CLINIC

ReceptionOn your fi rst visit to the clinic, you will be greeted by a receptionist, who will collect any referral letters, x-rays, test results or other information that you have brought with you. These will be passed to the consultant surgeon. It is helpful if you can arrive at least 15 minutes prior to the time of your fi rst appointment, so that any outstanding personal information can be obtained and so the consultant has time to review any information that you have brought with you.

You can then take a seat in the waiting room, where a medical secretary may see you, if any personal information is still required.

A nurse will take you to meet the consultant surgeon. A spouse, partner, relative or friend can accompany you, or the nurse can stay with you during your consultation and examination.

INITIAL CONSULTATIONWhether you have referred yourself, or been referred by a GP or other medical specialist, the consultant surgeon will discuss the nature of your problem and any other relevant information, such as your general medical history and family history. A thorough examination will follow. The nurse can be present during this examination.

The consultant surgeon will discuss your care with you, and may request investigations and tests to assist with the diagnosis.

An appointment will be arranged for a follow up consultation, to discuss the outcome of the investigations and any ongoing treatment plan.

The consultant surgeon will write to your GP and any other referring doctor and relevant specialists, summarising your consultation and the planned investigations. Copies of the letter can be sent to you, if requested. Letters will also be sent after all subsequent consultations, and as the results of investigations become available.

FOLLOW UP APPOINTMENT The consultant surgeon will discuss with you the results of the investigations carried out. He will recommend appropriate treatment and you will then have the opportunity to ask questions and discuss the benefi ts and any contra-indications of the treatment proposed.

If surgery is indicated, the proposed operation will be explained. This may be as a day patient, at 108 Harley Street, or may require admission to hospital. All patient care is based on informed consent; you will always know exactly what operation you will be undergoing, before admission and will not be asked to agree to further or alternative procedures. Surgery can be arranged for a mutually convenient date and confi rmed upon pre-authorisation by medical insurers, where relevant.

If no surgery is required, you will be discharged back to the care of your GP and other referring specialists but you may return should the problems recur.

Following surgery all patients are offered follow up appointments, as required.

WHAT TO EXPECTI FOLLOW UP APPOINTMENT

SURGERYIf an operation is required, it can be arranged immediately, for the fi rst mutually convenient date for you and your surgeon. Surgery, under general anaesthetic, takes place at a partner hospital and The London Rectal Clinic staff will make the booking for you. The partner hospital is recognised by all medical insurance companies, however it is important that you confi rm with your insurers that the hospital is included among those covered by your policy. Each patient is given a full estimate of the professional fees for the surgery, including anaesthesia and an indication of the length of inpatient stay required.

Patients are usually admitted to hospital on the morning of operation. On arrival, a hospital administrator will ask for contact information and confi rm your method of payment. Insured patients should bring insurance company and policy details with them. Once admitted to your room, a nurse will confi rm your personal details and answer any questions concerning hospital procedures. A bracelet is attached to each patient’s wrist with their name and hospital number. Any further investigations, such as blood tests or x-rays requested by your consultant surgeon will be undertaken.

Before the operation, the surgeon will visit, answer any further questions and ask the patient to sign a consent form for the procedure; he will visit again post-operatively. The consultant anaesthetist will also make a pre-operative visit to ask relevant questions and explain the method of anaesthesia, he will also visit again after surgery.

Insured PatientsAll the consultation, investigations and procedures at 108 Harley Street are routinely covered as outpatient treatment and are paid by medical insurers subject to policy terms, benefi t limits, conditions, exclusions and excesses.

The Consultants at 108 Harley Street are able to request these investigations as part of the original claim so a further pre-authorisation is not usually required but you should check with your insurer if this is their policy.

Patients who have been referred by a Doctor not based at 108 Harley Street should also be covered under their original claim reference but again it is better to check with the insurer.

Own AccountIf you are paying your own account, you are requested to settle all invoices on receipt this will usually be at the time of treatment.

An indication of investigation fees can be given in advance of any appointment based upon standard outcomes but these are for guidance only and are not binding.

Sponsored PatientsIf you are being sponsored by your employer or other third party please bring a letter of guarantee signed by your sponsor to your fi rst appointment with a Consultant. The letter should outline the scope of cover being paid for on your behalf e.g. consultations, investigations, surgery or provide a monetary limit. If this is not provided you will be regarded as a self paying patient and asked to settle the account at the time of treatment.
