RESPONSIBILITY:

The Registered Practical Nurse will provide medication, alcohol administration, nursing support, care plan delivery and therapeutic services to the residents of the Harm Reduction Home and other affiliated programs as required.

QUALIFICATIONS:

This position requires a minimum 2 year post-secondary Diploma or Canadian equivalent in

practical nursing. Two years’ experience working in the mental health/addictions field or one year

experience working with marginalized populations is an asset.

Language requirements: Fluency in English is required. Fluency in French is an asset. Priority will be

given to candidates who are bilingual in French and English as per our French Language Service Plan.

Professional designation/certification: College of Nurses of Ontario.

SALARY RANGE: 41,934.52 - $52,418.15 ($23.0410 – $28.8012 hourly)

UNION MEMERSHIP: This position is included within the collective bargaining unit, (Ontario Public Service Employees Union, Local 666).

POSITION SUMMARY:

Reporting to the Manager of the Harm Reduction Home (HRH), the Registered Practical Nurse (RPN) will work within the context of a managed alcohol program, provide person-centered care, harm reduction, and clinical care within a residential program Within an interdisciplinary team, the RPN will provide medication and alcohol administration, nursing support, care plan delivery and therapeutic services to the residents of the Harm Reduction Home.

The RPN will be required to work day shifts and evenings throughout the week, weekends and holidays along with on-call duties as required.

ESSENTIAL JOB RESPONSIBILITIES:

1. Resident Care

Dispensing Alcohol & Medication as ordered

Health teaching surrounding side effects and how to manage the symptoms thatinterfere with quality of life.

Transcription of orders and maintenance of medication records.

Collection of weekly assessments (vital signs and weights); equipment checks,clinical rounds as assigned.

Monitoring compliance to the care plans

Assist individuals with personal hygiene when necessary.

Demonstrate harm reduction and recovery oriented practice (ROP) philosophy andpractices within daily work and interactions

Intervene to crisis as necessary and work to intervene before a crisis escalates andprovide appropriate follow-up.

Participates in the safety of the program participants at all times.

Communicate with the Registered Nurse when acute medical issues arise.

Attends and contributes to medical/resident rounds as required

Client community integration

Definition of the Harm Reduction Home Includes:

The Harm Reduction Home (HRH) is a supportive residential program which will be serving 15 individuals from the local community that are impacted by homelessness/risk of homelessness along with alcohol dependence. The Harm Reduction Home operates within the context of a managed alcohol, resident centered, harm reduction, residential program that provides multi-disciplinary support to meet the daily needs of individuals. The HRH is intended to offer a supportive living environment and tools and opportunities for social and skills development. The program requires co-operative living where the residents, in conjunction with staff, will share the responsibility of managing the residence.
