Making an appointmentin Radiology
A requisition form must be completed by the physicianfor any appointment in ultrasound, x-ray, nuclear medi-cine, or for a CT-Scan. Results will be mailed to the referringphysician using the address that appears on the consultationrequisition form.

St. Mary’s Laboratories
Physicians wanting to send their patients for a blood testat SMH must fill out a requisition. Requests written on aphysician’s prescription pad are also accepted. Requestsmust include the patient’s full name (for a woman pleaseinclude maiden name) and medicare number. The physi-cian must include as much clinical and diagnosticinformation as possible on the requisition forms. Resultswill be sent back to the physician by fax, or by mail to theaddress that appears on the form. For offices withdedicated fax machines, same-day reporting of routineheamatology and biochemistry results is the standard.Dedicated fax reporting is strongly encouraged. Physician’ssignature must appear on all laboratory requests as requiredby the Collège des médecins du Québec.

The Medical Day Center
In order to serve urgent community needs, a Medical DayCenter has been established to diagnose and treat illpatients who do not require overnight care. Our aim is toprovide rapid testing and treatment for extremely illpatients of community physicians when the usual channelsprove slow or logistically difficult. St. Mary’s Oncology Department provides comprehen-sive continuous care to cancer patients, with emphasis oncancers of the lung, GI tract, breast and lymphoma. Itcomprises Therapeutic Units (out-patient/ in-patient), aPalliative Care Service and a Home Care Service.Individual care plans are determined at weekly TumourConferences.
