To receive toy assistance, you must apply IN PERSON with the following items in hand:

1. PICTURE ID FOR APPLICANT

Options: Driver’s license, passport, state ID, Matricula Consular

2. VERIFICATION OF CURRENT ADDRESS

Options: Lease, Medicaid card, utility bill

3. VERIFICATION OF INCOME OR BENEFITS

Options: Medicaid letter, food stamp card

4. VERIFICATION OF AGE FOR CHILDREN

Options: Birth certificate for children ages 12 & under
