In the United States, an estimated 4.5 million outpatient visits annually are attributed to adverse drug events. Identifying a significant problem in medication errors, the Institute of Medicine spearheaded the creation of the Universal Medication Schedule (UMS).

The UMS eliminates the confusion of terminology, such as “take one tablet twice daily,” to include more precise descriptions that people can clearly interpret.

The UMS utilizes references to the time of day versus the frequency of use. For example, “twice daily” is described as “morning” and “bedtime.” “Noon” and “evening” are also employed to clearly communicate time periods. To ensure customers do not misread directions, a carriage return indicates the end of one dose and the beginning of another. Further, UMS visual illustrations reinforce written descriptions regarding different time periods.
