Most Important Qualities of the Doctor- Patient Relationship | Communication is the key in any doctor-patient relationship. At FemSmart and AndroSmart—we treat the patient, not the lab results. needs and goals. In addition to this, we have the leading edge of technology in hair restoration, cellulite treatment and body sculpting.

Best Advice to Patients | My advice to patients would be to always be an open book when talking to your doctor—don’t hold back. Let them know exactly how you are feeling, so that we can help. You deserve to feel and look great. This is the reason I specialize in hormone therapies and hair restoration. This is the reason I have assembled such a diverse team of physicians and surgeons.

Why Choose Me | I not only offer the latest and greatest technologies and treatments to help patients look and feel as young as they deserve—I hold myself and my staff to the highest level of patient care. We strive every day to better the lives of our patients. It is notjust about how long you live or how good youlook. It’s about having the best quality of life along the way.

Professional Memberships / Licenses Charles L. Powell, M.D., moved to the Dallas/Fort Worth area in 2005. His philosophy of delivering expert comprehensive care stems back to his original training as a family doctor in 1998. He is currently board certified by The American Board of Cosmetic and Aesthetic Medicine, as well as the American Academy of Family Medicine. He continues to maintain his board certification in Family Medicine, because he feels a well-rounded medical approach is equally as important as the surgical care his patients receive. Dr. Powell founded The Specialty Clinics of FemSmart and AndroSmart in 2008. He has special interests in hair transplantation, body contouring, cellulite treatment, hormone

therapy, as well as the growth and develop-ment of the FemSmart/AndroSmart team. About TrueMedicine | TrueMedicine is a multi-physician group specializing in adult primary care. With a special emphasis on compassionate care that is patient-centered, patients have flocked to this practice. True-Medicine is dedicated to improving the value of healthcare by focusing on outcome effec-tiveness, increasing service and convenience, as well as controlling costs. Their objective is to offer the best patient centered “Medical Home” model, using a team-based approach. All staff are continuously trained on the im-portance of communication to establish an effective continuum of care for their patients.

our Philosophy | The TrueMedicine phi-losophy is to address the “TRUE” needs of the patient, which oftentimes is not the same as the given reason for a patient’s visit. The providers are given additional education and training on the importance of identifying psychological and social stressors which contribute to symptoms, and how to address these in a costeffective way. Through emphasis on developing close relationships with patients, TrueMedicine providers and staff are attempting to maintain the process of patient care as close to “the way it used to be” as much as possible. Their founder, Dr. Jonathan Matthews, states, “Taking care of patients is an honor and a privilege. We should care for every patient as though they are our own family member—and that is what TrueMedicine is really about.”

Why Choose us | TrueMedicine employs a state of the art Electronic Medical Record sys-tem giving them the ability to deliver consistent patient care with heavy emphasis on accurate communication. All employees are required to become proficient in many aspects of commu-nication, leadership skills and teamwork, by attending in-house training on a monthly basis.

[Blue stone Dental]Specialty | Comprehensive family dentistry including general dentistry, dental implants, laser dentistry, PerioLase® laser gum treat-ment, cosmetic dentistry, orthodontics and TMJ dysfunction.

Most Important Qualities of the Doctor-Patient Relationship | You know what youwant! Many times, patients’ wants and goals are not truly heard by their healthcare provider. Listening to your concerns, about your smile, teeth and gums help build an effective course of care.

Why Blue Stone Dental | Most of your oral healthcare needs and TMJ concerns can be treated under one roof. The end result—truly comprehensive care in one convenient loca-tion. This allows our dental team to focus on the specific strengths and individual training of each doctor to cover most areas of dentistry. All of your family members’ wants and needs can

be coordinated in one location—evidence that Blue stone Dental is committed to exceeding your expectations by providing personalized attention and giving quality care in a family-friendly environment.

Professional Memberships / Licenses American Dental Implant Association, Amer-ican Association of Functional Orthodontics, International Association of Orthodontics, American Orthodontic society, American Academy of General Dentistry, Texas Acad-emy of General Dentistry, American Academy of Cosmetic Dentistry, American Academy of Craniofacial Pain, American Dental As-sociation, Texas Academy of High Tech and Laser Dentistry and the American Academy of Sleep Medicine.

Dr. Jay Corley | He is one of only a few doctors in Texas who has hands-on training in the new FDA approved Laser Periodontal Therapy with the PerioLase laser, which places him in a rare category of dentists who use laser

dentistry effectively and on a daily basis. His ex-tensive training in sedation dentistry allows pa-tients the most comfortable visit. In addition, he has spent extensive time attending continuing education courses in Implant dentistry with a focus on the placement and restoration of den-tal implants—replacing one or multiple missing teeth and anchoring loose fitting dentures.

Dr. Rebecca Lauck | she has attended more than 2,000 hours of continuing education classes since graduating from dental school to best serve her patients. Her extensive training in orthodontics, sleep apnea and TMJ as well as general and cosmetic dentistry brings a unique skill set to the dentistry that she is able to provide.

Specialty | Creekside Pet Care Center is a full service veterinary medical facility. It has steadily grown into one of the area’s premier veterinary hospitals. They provide full service veterinary care, deluxe animal boarding, doggy daycare, grooming and training. Their staff seeks to pro-vide the best possible medical care, surgical care and dental care for their highly-valued patients. They are committed to promoting responsible pet ownership, preventative health care and health-related educational opportunities.

Services include: • Veterinary Clinic • Pet Boarding• Grooming • Training

Most Important Qualities of the Doctor-Patient Relationship | Dr. Rob Hance and his team have mastered the subtle art of animal rapport. “At other vets, I’ve had to drag them in,” says longtime client, Elaine Henley. “Dr. Hance and the other vets are so kind and caring. They genuinely love the animals. The

front office is always happy, helpful and upbeat. The dogs can’t wait to go there.”

Why Choose us | “Creekside is a very unique and special place, but more impor-tantly it is a team of special people that make the difference,” says Dr. Hance. “Our goal is to provide the best pet care and client expe-rience possible. We are in the business of de-veloping relationships with families, including the four-legged members. The fact that all pet care needs can be met in the same location with the same team is a huge bonus.”

Professional Memberships / Licenses | One primary distinction is their accreditation by the American Animal Hospital Association (AAHA). An elite few, 15 percent of all veteri-nary practices in the United States and Canada, are accredited, which requires them to pass reg-ular evaluations based on 900 quality standards of pet care, including surgery, record keeping, dental care, emergency services, cleanliness,

imaging, anesthesiology and more. AAHA standards are internationally recognized as the benchmark for quality care in veterinary med-icine and provide an added layer of assurance for patients and their custodians. Their accred-itation demonstrates their commitment to the highest standards of veterinary care.

Why We Stand out | Dog and cat owners feed, nurture and lavish attention on their pets. Our highly qualified kennel attendants do the same when you can’t be there. Guests of Creekside’s pet boarding facility are all lavished with attention during their stay—brushing, cuddling, treats, supervised outdoor playtime on two acres of fenced pasture. Creekside also has new suites that offer all of the comforts of home.

Creekside Promise | We promise to exceed our client’s expectations in their service and in the care of their pets, everyday.

Specialty | Eyelid plastic and orbital surgery, including eyelid and brow lifts, correct lid malpositions (such as turning in or turning out), lid ptosis, tear duct problems, skin cancer of the eyelids, thyroid eye problems and orbital tumors. In addition to these treatments, I also offer numerous cosmetic services such as Botox, Dysport, Radiesse, Restylane, Juvederm, Prevelle Silk, CO2 laser resurfacing, Thermage, IPL and DermaPen. DermaPen is a new micro-needling technique that treats skin texture and wrinkles all with minimal downtime. I also incorporate the Arque- Derma Artistic Restoration Lift technique with Juvederm, if appropriate to the patient’s needs.

Most Important Qualities of the Doctor- Patient Relationship | Listening. To fully understand each patient’s concerns, I must spend enough time with each of my patients to carefully discern their unique needs and goals.

My Best Advice to Patients | Don’t ever feel rushed into any treatment. Take ample time to ask questions before considering all of the options available to you. Then, choose the plan of action that feels right for you.

Why Choose Me | I trained as an ophthalmologist. What this means to my patient’s is that I have a great deal of experience and comfort working around the eyes. And because physicians in my field actually pioneered the use of Botox, I have had 15 years of experience with cosmetic Botox and 20 years experience with its medical use. Through all of my professional experiences, my foremost goal is always to preserve the natural function of the lids in order to avoid any possible eye problems.

Professional Memberships / Licenses I am board certified in Ophthalmology and licensed to practice medicine in Texas. I am also a member of the American Medical

Association, American Academy of Ophthal- mology, Texas Medical Association and the Texas Ophthalmological Association.

Greatest Professional Accomplishment To be able to say that I enjoy the respect of my fellow physicians, so much so that several of them have come to me for their own eyelid surgery—which is the ultimate compliment. I have worked hard to build a successful practice as a solo practitioner, recognizing that it is not common in today’s industry.

Charitable Work | I am proud to serve as a Girl Scout leader and as the fundraising chair for a local Boy Scout troop. I also actively support the Lion’s Club, as well as participate in the Komen Race for the Cure annually in honor of my grandmother, who died from breast cancer.

Medical Philosophy | I strive to be honest and always put the patient’s best interest first.

What Sets You Apart | The best gift we can give our patient is not only a healthy smile, but a smile and experience that they will feel good about. Ultimately, the way we care for our patients is what seperates us from other offices.

Most Important Qualities of the Doctor-Patient Relationship | We listen to our patient’s needs and concerns and come up with a treatment plan that they are comfortable with and trust.

Best Advice to Patients | Don’t let fear stand in your way of getting what is necessary to improve your oral health and ultimately, overall health. We strive to make every appointment comfortable and easy for our patients.

Why Choose us | We believe that dental care should be a fun and positive experience for everyone. I understand how uneasy some patients feel about their dental visits and how we make a difference in providing a warm, relaxing and comfortable environment. Our practice is family-friendly. When we see one family member, we are seeing the entire family (from grandparents to parents to children). It is important that we get everyone involved and motivated about their health because good dental care shouldn’t end at the dental office, it should extend to home. We strive to be thorough in everything we do—taking the time to get to know our patients, making sure they are well informed and educated of dental procedures, and that they receive quality dental care from the moment they walk through our doors. Our team is dedicated to providing our patients with excellent personalized care and service to make every visit as easy and pleasant as possible. We provide a broad spectrum of dental care for our patients from preventive to restorative to cosmetic dentistry. To ensure that we stay committed to our high standard of care, we continue to educate ourselves through advance continuing education courses.

Specialty | Comprehensive restorative/crown and bridge dentistry with an emphasis in Cosmetic Rehabilitations. All cases are accomplished with comfortable oral sedation, so we can be very efficient with our patient’s busy schedules, as well as putting the most anxious patients at ease.

Important Qualities of Doctor-Patient Relationship | A patient should have the final say as to the level of care that they want to pursue. Decisions made by insurance companies tend to best serve insurance companies. Our practice has no managed care philosophies in place. We are relationship driven. We know our patients by their name, not their insurance coverage.

Professional Memberships / Licenses American Dental Association, Texas Dental Association, Fort Worth District Dental Society, American Academy of Cosmetic Dentistry, Christian Medical Dental Society and the Dental Organization for Conscious Sedation.

Why Choose Me | We don’t offer patchwork dentistry. We offer a level of care that is infection free to support overall health. For 28 years, our practice had seen patients from all over the continental USA, as well as many foreign countries. Dr. Gist’s experience and approach to overall systemic health, as it relates to restorative dentistry, is the difference that sets this practice apart.

Charitable Work, Honors or Awards | Started in 1984, Dr. Gist has his own charity

program, Give a Smile for Life. His practice has restored hundreds of smiles at no charge to those in need. Dr. Gist has been named a “Top Doc” by Fort Worth, Texas magazine and a “Best Dentist in America” by Consumer Research Council. Dr. Gist also received the Quintessence Publishing Award at Baylor College of Dentistry for outstanding clinical work in the field of Periodontics and Crown and Bridge Dentistry.

our Mission | To deliver the best service and highest quality of dental care in the world, to patients that are looking for this level of care. To accomplish this in a fun environment, surrounded by the most talented and nurturing support staff in the world. In short, to be a blessing to every patient that God brings us.

Whether managing medical problems, weight issues, thyroid dysfunction or life circumstances we help our patients live a full life. We offer the newest and most innovative anti-aging, anti-oxidant anti-inflammatory treatment—the Cryo-Sauna—as seen on Dr Oz, The Doctors, ESPN and The Today Show. You can learn more about the amazing benefits of this state-of-the-art technology by visiting the website www.cryousa.com. We also offer metabolic testing with Indirect Calorimetry (measures your metabolic rate) and Athletic Biomarker analysis for athletes (measures important biomarkers that can affect performance).

Why Choose Me | Life’s Medicine is a highly attentive medical practice that focuses on making you better. Whether addressing high blood pressure, diabetes, thyroid issues, fatigue, insomnia, weight or athletic performance issues each patient gets the attention they need and deserve.

Most Important Qualities of the Doctor-Patient Relationship | I truly want to help my patients live a full and happy life. Sometimes that means taking them off meds. Sometimes it means putting them on others, but no matter what the plan, it is something the patient and I will discuss, evaluate and decide together. I really love to educate patients, so that they can truly understand what is happening to their body. This helps them be more compliant with their treatment plans and taking any supplements or medications correctly. Helping patients be the best that they can be is a true passion of mine. What Separates You From others in Your Field Doctors have a desire to help their patients. It has been my experience that insurance makes it difficult to build a well-rounded practice. Life’s Medicine is a fee for service practice allowing me to spend significantly more time with my patients, i.e. be highly attentive. I limit the number of patients I have in order to spend the time the patient needs. This allows time to really listen to my patients and take into account what they’re feeling, what their lives are like and how they take care of themselves. These factors affect their overall health. I won’t shove them into some pre-determined diagnosis and ignore obvious signs or symptoms. I want my patients to leave their appointment excited, optimistic and hopeful about their future health.

About Cryotherapy, Metabolic Testing and Athletic Biomarkers | Life’s Medicine offers Whole Body Cryotherapy. This hyper-cooling process lowers a person’s skin temperature to approximately 40˚ F for a short period of time. It is achieved by enveloping the body with extremely cold air for up to 3 minutes. The cryosauna devices are proven, innovative and cutting-edge technology. You can enhance and improve your athletic performance, as well as, accelerate the body’s natural recovery processes while offering overall benefits to your beauty, health and wellness. Metabolic Testing (indirect calorimetry) measures a patient’s true basal metabolic rate. Athletic Biomarker analysis is a blood test that will help identify nutritional deficits that affect performance.

You Should Know | This is not your conventional medical practice. We’re not contracted with health insurances but can bill insurance for ancillary services (labs). We are dedicated and committed to helping our patients reach their goals. We invest in our patients so they can invest in themselves and see ample returns! Living a full and optimal life is good medicine, come get it!

Specialty | Spine Team Texas is a true, com-prehensive spine center specialized in treating simple neck or back strains to complex spine disorders. We offer the full spectrum of services in one, convenient location with non-surgical and surgical options. We are dedicated to treat-ing patients conservatively through education, spine-focused physical therapy and non- surgical treatments. If surgery is the last or only option, we utilize the latest minimally invasive spine surgery techniques whenever possible. In fact, approximately 90 percent of our patients are treated successfully without surgery. Our team includes spine-focused physical thera-pists, physical medicine and rehabilitation phy-sicians, an anesthesiologist fellowship trained in spine pain management, neurosurgeons and an orthopedic surgeon specially trained in minimally invasive spine surgery techniques.

Services | Full-spectrum, full-time spine ser-vices include spine-focused physical therapy, general diagnostic x-ray, EMG/Nerve con-duction diagnostic testing, epidural steroid injections for acute spine pain management, facet blocks, radio frequency ablation, minimally invasive spine surgery, spinal cord stimulator

implants, Botox injections for back/neck mus-cular pain and trigger point injections.

Best Advice to Patients | If faced with back or neck pain choose a multidisciplinary spine center that offers a comprehensive, con-servative approach with the most innovative techniques for treating spine conditions.

Why Choose us | In the traditional spine care model, patients with back or neck pain are referred to several specialists in different locations for testing and treatment with mul-tiple office visits, delayed access, and little to no communication or collaboration amongst those specialists regarding the best course of treatment for the patient. To address these issues, the Spine Team Texas concept pro-vides all of the necessary diagnostic testing and treatment in one location. This optimizes convenience, efficiency and effectiveness for patient outcomes and quicker return to qual-ity of life. Our conservative approach to spine care utilizes surgery as a last resort. This pro-vides patients with an opportunity for effective treatment with decreased interruption to their busy lifestyle.

Charitable Work | Spine Team Texas has hosted fundraising events to benefit the Make-A-Wish Foundation of North Texas and Wednesday’s Child Benefit Corpora-tion. We partner with the Carroll Medical Academy of CISD to provide summer in-ternships to students aspiring to be in the medical profession.

Awards | “Top Docs” in Fort Worth, Tex-as magazine; “Healthcare Heroes” Physi-cian Award from Dallas Business Journal as nominated by their patients; U.S. News & World Report “Top Docs” as nominated by peer physicians in the specialty of neuro-surgery; “Best Places to Work” by Dallas Business Journal.

Medical Philosophy | Treat all patients as you would your own family member.
