1. In DAINIK SWASTHYA PARIKSHAN PRATIVEDAN i.e Sceering of Student, page initially, select Village Name, School or Anganwadi Name than enter Nodal Teacher Name of School or Anganwadi

Center and also enter Mobile or Phone No. of Nodal Teacher of School or Anganwadi Center.

After this entry, now select Class No. than enter Total Student i.e (Male/Female/

transgender), Total Inspected Student i.e (Male/Female/ transgender) , Total Disease found in

Students (Male/Female/ transgender) , Total Treatment Students (Male/Female/ transgender) , Total

Students Referred (Male/Female/ transgender) and also enter if any Remarks. These process is

repeated in different class.

2. In REFERAL ENTRY page initially on selection of School or Anganwadi, Year, Month, the screen opens enter Students Name, Gender, Class, Age, MCTS Id, Mother/Fathers Name, Contact No.,

Janmjat Vikriti, Bimari, Vikas Sambandhi Dosh, Kishor Awashtha Vishisht Awashtha than Any Other

disease, Referred Health Center and Remarks, These process is repeated for every student whose

referred by visited team.
