    Watchful Wait-ing for Prostate

    Cancer?DEAR DR. DONOHUE: I am an 80-year-old

    male whose annual physical exam was good. My wife and I have a good sex life. We've been mar-ried 61 years.

    However, my PSA rose to 5.7. My family doctor sent me to a urologist, who suggested a biopsy. The urologist said my prostate gland is normal for my age and had no hard spots or lumps. My dad died at 87 due, in part, to prostate cancer. A brother also had difficulties. I read that at my age, I should enjoy life without such procedures. Your opinion, please. -- B.M.

    ANSWER: What to do when the prostate spe-cific antigen (PSA) blood test is higher than nor-mal is a controversial subject. Every year, about 192,000 men are diagnosed with prostate cancer. Close to 70 percent of those cancers are low-grade, meaning they don't pose a serious threat to life. Such cancers, depending on the man's age and his health, may not require any treatment other than scheduled monitoring.

    One of the ingredients that goes into the mix for making a decision for "active surveillance" is what's seen on the biopsy. The pathologist exam-ining the prostate tissue discovers if any cancer is present and assesses the cancer's stage and its po-tential to be an aggressive cancer. If the cancer is in its early stages and if its appearance is one that's not threatening, then the decision for treatment is something that can be discussed with the doctor. Quality of life is as important as extension of life. However, I believe you should have the biopsy. It has some rare complications, but it provides infor-mation not obtainable in any other way.

    You are a very young 80-year-old. You have no other health problems that might shorten your life. You have a family history of prostate cancer. If the biopsy shows a low-grade cancer, then talk to the doctor about your wishes. One of those wishes could be active surveillance.

    The trick to the perfect hot sauce is using a combination of pep-pers with a balance of sweetness, fruit and heat. Fruity peppers like the Aji Amarillo Chili, the Mexican Mirasol Pepper or the Yellow Peruvian Chile (which is a deep yellow, sometimes orange, 4 to 5 inches long) have an intense spice with a fruity flavor. A Mustard Habanero pepper retains the heat found in many Habanero varieties but has fruity overtones. This pepper is dark-yellow with hints of orange and a pointed tip.

    HOT STUFF HOT SAUCE3 tablespoons extra-virgin olive oil1 large yellow onion, sliced2 teaspoons salt2 large jalapeno peppers, diced2 medium chile peppers, such as poblano, New Mexico or Anaheim, diced (see Tip below)2-4 habanero or other small, hot chile peppers, stemmed, halved and seeded (see Tip)4 cloves garlic, diced1 large carrot, tip and root end removed, chopped1 pound tomatoes, diced (about 3 cups) or 1 (28 ounce) can fire-roasted diced tomatoes1-3 teaspoons sugar or stevia1 cup distilled white vinegar or apple-cider vinegar

    1. Heat oil in a large saucepan over medium-high heat. Add onions and salt, and cook for 3 to 5 minutes, until onions begin to soften. Add in peppers, garlic and carrots. Cook, stirring, until onion begins to brown, about 3 to 4 minutes. (Note: This should be done in a very well-ventilated area! The fumes from the cooking peppers are strong, so do not lean over the pot, or you may inhale the acrid steam.)

    2. Reduce heat to medium. Add tomatoes and sugar or stevia. Bring mixture to a boil, then return heat to medium. Continue to cook, stirring occasionally, until the tomatoes begin to break down,

    about 15 to 20 minutes. 3. Remove from heat and allow mixture to steep until it comes

    to room temperature. Carefully transfer pepper mixture to a food processor or blender. (Use caution when pureeing hot ingredients.) If youre using a blender, place the lid on loosely and cover it with a dishcloth to allow any steam to escape. Puree mixture for 15 seconds. With food processor or blender running, add vinegar through the feed tube or opening in the lid in a steady stream.

    4. Puree until smooth. Set a fine-mesh sieve over a medium bowl; pour the pureed mixture through the sieve, gently pushing on the solids with a wooden spoon to extract all the liquid. (Discard solids.) Let the sauce cool to room temperature, about 1 1/2 hours. Taste and season with more salt, if necessary.

    5. Transfer hot sauce to a sterilized, pint glass jar or bottle and secure with airtight lid. Refrigerate. The hot sauce tastes best when aged at least 2 weeks. Shake bottle to recombine the liquid before using. Can be stored in refrigerator up to 6 months. Makes 1 pint.

    Tip: The membranes that hold the seeds are the spiciest part of chile peppers (thats where the capsaicin is). The seeds pick up some spiciness by association. You can adjust the heat of the peppers and the spiciness of the hot sauce by using some or all of the seeds along with the flesh of the peppers. Be sure to wash your hands thoroughly after chopping hot peppers, or wear rubber gloves.

    way on Memorial Day weekend, meaning that four of his 13 career victories at the Sprint Cup level have occurred at the 1.5-mile track.,

    where he also won the Sprint All-Star Race in 2008.But Kahne, who is from Enumclaw, Wash., has never finished better than eighth

    (2006) in the point standings. This year, Kahne's first at Hendrick Motorsports, began slowly, but now he has reeled off consecutive finishes of seventh, eighth, fifth, fourth, eighth and first, advancing from 31st to 15th in the standings. The latest CMS victory was important in terms of one of two wild-card slots in the Chase for the Sprint Cup, but, with 14 races remaining in the regular season, Kahne is only 42 points behind 10th place Carl Edwards. The top 10 automatically advance into the Chase.

    "The win's really important for us to make the Chase and keep moving up in the points," Kahne said. "You've got to win to make the Chase.

    "We just need to stay after it and keep doing the things we've been doing."Kahne and teammate Jimmie Johnson are the hottest drivers in the sport. Although

    Roush Fenway drivers Greg Biffle and Matt Kenseth are atop the point standings, it would be premature at this point to suggest Hendrick isn't still NASCAR's best team.

    "Our team is solid," Kahne said. "Our car is solid. Mr. Hendrick gives us every-thing we need to win races and run up front. Finally we were able to put it all together.

    "But next week (Dover) will be a new show. We have to figure it out again next week in practice. It's just tough. The competition is stout. It's hard to just move through the field and pass cars because everybody is so fast. The cars are so competitive right now, it makes it difficult."

    Too many veterans have waited far too long for mental-health care.

    The Office of the Inspector General at the Depart-ment of Veterans Affairs was asked to look at whether the Veterans Health Administration is accurately re-cording the wait times for mental-health services for both existing and new patients.

    Policy states that if a veteran calls in and asks for help, he or she is to be seen within 24 hours.

    If after the initial evalu-ation the veteran is not immediately given ser-vices (hospitalization or outpatient care), a more-comprehensive evaluation must take place within 14 days. This would include a diagnostic and treatment plan.

    VHA is to calculate wait times by figuring the days from the desired date of care to the date of the actual appointment. This assumes, of course, that the right date is entered into the system.

    The VAOIG determined that: 1) VHA doesnt have a reliable way to determine

    whether its giving services on time; 2) VHA doesnt provide initial appointments within that 24-hour window; 3) Veterans are waiting far past that 14-day window for their treatment plan appointments.

    As always, the devil is in the details.The VAs own 2011 performance report claimed that

    95 percent had their evaluation with 14 days. Not so, said the VAOIG. The VAs inaccurate calculations were based on the time between the actual appointment date and the time the evaluation was complete ... often on the same day, which looked like a wait of zero days. It wasnt calculating how long it took to get the appoint-ment, which might have been weeks.

    The VAOIG looked at selected records and deter-mined that only 49 percent of veterans had their evalu-ation within 14 days. The average wait was 50 days to get a full evaluation.

    Fifty days is a long time for a veteran who is seek-ing help.

    PICKS OF THE WEEKSherlock Holmes: A Game of Shadows (PG-13) --

    Robert Downey Jr. puts his swagger and eccentricity into Sherlock Holmes in this second installment of Holmes as directed by Guy Ritchie. Dont expect a slowly unraveling mystery -- this movie is more like the adventures of Iron

    Man if he didnt have his suit and lived in Victorian-era England.

    Just like last time, it has more explosions and brawl-ing than you might remem-ber from the original stories,

    but the resulting experience is more fun than you expect. Besides action, Downey and Jude Law (back as Dr. Wat-son) have plenty of quips. The story also mixes in more brain with its brawn, as Holmes is pulled into the mind games of Moriarty.

    Good Deeds (PG-13) -- When Tyler Perry steps out of the fat suit and wig, hes all business. This movie star-ring, written and directed by the prolific Perry is one of his serious ones, and the movie will not let you forget it. Perry plays Wesley Deeds, an uptight, wealthy businessman who has had his whole life handed to him since he was born. While Deeds questions whether his life is the one he wants, he forms a bond with a destitute single mother who cleans his office. The central relationship isnt a romantic one, but a centerpiece for the scripts meditations on morality, fair-ness and charity. Its ultimately a thoughtful, sober drama.

    In Darkness (R) -- As the Nazis march through Po-land, an anti-Semitic sewer worker finds himself taking money from Jews in exchange for hiding them in the city sewers. The film is based on the true story of Leopold So-cha. As he realizes how much risk hes taking just to get money out these desperate people, he becomes uncertain of his motivation. Could it be money, fear or humanity?

    Come on. Take the one of the kookiest men in Holly-wood, cast him as a superhero with a flaming skull in a sequel directed by the guys who did Crank and Crank: High Voltage, and you would expect something more ex-citing -- or at least something notably more insane than its banal predecessor. Nope. This movie rides right out of your memory with the spirit of vengeance.

    A: Dont worry -- there will be 10 new episodes of Lifetimes Army Wives starting Sunday, June 24, at 10 p.m. ET/PT. Note the new time slot, as Wives will now follow Drop Dead Diva. Upon the shows return, Susan Lucci will be back in her guest-starring role.

    Q: I watched the most wonderful documentary on PBS

    the other night on the life of Johnny Carson. He is and always will be the best late-night talk show host, and I was very interested to learn about this usually private man. Will it air again soon? If not, can I buy it on DVD? -- Paul F., Altoona, Pa.

    A: I know the documentary you are talking about, because I watched it too and absolutely loved it. In the late 80s and early 90s -- when I was finally allowed to stay up late -- I watched The Tonight Show with Johnny Carson reli-giously, so I was thrilled to sneak a peek into this talented mans life. You can stream American Masters Johnny Carson: King of Late Night on your computer at pbs.org/americanmasters. However, if you can wait until July 17, you can purchase the DVD (or Blu-ray) of the film, which will include outtakes and bonus material, through PBS Distribution.

    Q: My wife and I loved the show Cold Case, and still watch the reruns. We wonder whatever happened to Kathryn Morris, who played Det. Lily Rush. We have seen all the other lead characters on other shows, but not her. -- Ron C., Roanoke, Va.

    A: The gorgeous 43-year-old just finished the short film Sundays Mother, and most recently co-starred with Denise Richards and Jim Belushi in the direct-to-DVD movie Cougars, Inc. She also played a small role in last years Oscar-nominated film Moneyball; however, her part ended up on the cutting-room floor. I havent seen the DVD extras yet, but hopefully we can watch her there. I think your wife and you will agree that we need to see Kathryn in a leading role on another hit series stat!

    Q: My two favorite new shows of this

    past season were both on Fox: Touch and Alcatraz. Will they be back for another season? -- Sienna F., via e-mail

    A: Ive got good news and bad news. Want the bad news first? Fox has opted not to pick up Alcatraz for a second season. While it debuted pretty strong in the ratings, each week saw a steady decline in viewers, and it lost more than half its initial viewers by seasons end. Touch, however will be back for a second season. And, in case you were wondering, American Dad, Bones, The Cleveland Show, Fringe, Glee,

    Kitchen Nightmares, New Girl, Raising Hope and The Simpsons will all be back for new seasons come fall.
