Provide the name of the allegeddiscriminatory institution, and/orhis/her official job title, and adescription of the issues. Please includethe names of witnesses or anyone whocan clarify the circumstancessurrounding your complaint.

party write and sign the complaint onbehalf of complainant, include that person’s

providing a written statement, a verbalcomplaint of discrimination may be made to
the Title VI Program Coordinator. Underthese circumstances, the complainant willbe interviewed, and the Title VI Program
Coordinator’s office will assist thecomplainant in converting the verbal
allegations to writing.
Complaints received by fax or e-mail will beacknowledged and processed, once the
Complainant has been identified andhis/her intent to proceed with thecomplaint have been established.
Title VI Program complaints also may behand delivered to DDS' Title VI Program
For allegations received verbally or bytelephone, the complaint must be reduced
to writing. A Title VI Program complaintform will be forwarded to the complainant
for completion and signature. Thecompleted form may be returned to the
Title VI Program Coordinator forprocessing.
Within five (5) days of receipt of thecomplaint, The Title VI Program
Coordinator will send an acknowledgementletter to the complainant and begin
processing the complaint.
The Title VI Program Coordinator will notify
Office for processing.

Any person, or group of persons, or entity thatbelieves they have been subjected to
discrimination prohibited by Title VI Programnondiscrimination provisions may file a
written complaint with Georgia Department ofDriver Services (“DDS”) Title VI Coordinator.
A person may also choose to file a separatecomplaint with the Federal Motor Carrier
Safety Administration, Office of Civil Rights, ifthe complainant is not satisfied with DDS’
response.
A formal complaint must be filed within 180calendar days of the alleged occurrence or
when the alleged discrimination becameknown to the complainant and must meet the

nondiscrimination authorities, which protectsindividuals from discrimination based on race,color, national origin, sex, age, disability, low-income and limited English proficiency (LEP) in
programs that receive Federal financialassistance.
