a pharmacist hospital-wide program for monitoring and adjusting anticoagulation regimen’s, to promote adherence to institution guidelines, improve healthcare utilization, and improve patient outcomes.

- DTI per pharmacy protocol - Enoxaparin per pharmacy protocol 2004 ICU Sedation Pharmacist Rounds: In conjunction with critical care specialists, designed and

implemented a pharmacist hospital-wide program for monitoring and adjusting sedation practices in the ICU’s to promote adherence to institution guidelines, improve healthcare utilization, and improve patient outcomes.

2004 Pharmacy Discharge Counseling Service: In conjunction with internal medicine and

ambulatory specialists, physicians, and nurses developed a process to target appropriate patients to receive counseling on medications prior to discharge and also receive follow-up post-discharge.

2004 Multidisciplinary Heart Failure Monitoring Service: In conjunction with cardiomyopathy

physicians and nurses, developed a multidisciplinary process whereby acute decompensated heart failure is monitored and improved in patients admitted to Boston Medical Center.

2004 Antibiotic Management Team: Coordinated development of multidisciplinary team to

proactively monitor antibiotic use within Boston Medical Center. Members of team include ID pharmacy specialist, pharmacy residents, clinical pharmacists and an ID physician.

2003 Pharmacy Nutrition Support Service: Coordinated the development and establishment of

pharmacy services within the multi-disciplinary nutrition support service for Boston Medical Center.

2002 Decentralized Pharmacist Program: Developed and implemented training program designed

to prepare staff pharmacists from a central distributive to a decentralized model of practice. 2000 Heparin Dosing Protocol: Developed and implemented an hospital wide heparin
