Transforming Care is about helping people live the best lives possible, in the right place and with the support they need. It is about helping people lead fulfilling and safe lives in their communities whenever possible, with less need for long stays in hospitals and similar places. What is a ‘Local Area Emergency Protocol (LAEP) review? A LAEP Review is a meeting or teleconference (telephone call) that happens when it is thought a person may urgently need to go into hospital. The aim is to make sure the person gets the best possible care. The people invited to take part in the review will include those that are involved in the person’s care as well as others from health and social care who organise services for people with learning disabilities and/or autism. The person who may need to go into hospital can be invited to take part as well as their family.

Transforming Care Local Area Emergency Protocol review

Information to help you decide

For the person being reviewed and family carers

Transforming Care is about improving the health and care

system for people with learning disabilities or autism

who also have mental health conditions or behaviour which

challenges. Transforming Care is about helping people live

the best lives possible, in the right place and with the support

they need. It is about helping people lead fulfilling and safe

lives in their communities whenever possible, with less need

for long stays in hospitals and similar places.

What is a ‘Local Area Emergency Protocol (LAEP) review?

A LAEP Review is a meeting or teleconference (telephone

call) that happens when it is thought a person may urgently

need to go into hospital. The aim is to make sure the person

All the meetings are private and confidential. Anything we write down will be kept in a safe place.

The things we want to find out are:

1. Why might the person need to go to hospital?

2. Is there anything that could be put into place in the

community so the person can stay at home?

3. Is there somewhere else the person can be cared for that

is not a hospital if they cannot stay in their own home?

4. If a hospital admission is required, what would be the aim

of the admission?

What happens afterwards?

A decision will be made about:

- whether or not the person needs to go to hospital

or

- what other support should be put in place in the

community.

A report will be written about the review.

Do I have to take part in the review?

It is your choice to take part and you can change your mind at

any time. We want you to feel happy, safe and in control of how

you take part, and we will do whatever we can to help.

We will work hard to make it a good experience for you.
