1. Identify and enhance practices which connect programs and services for children, families, schools and communities.

2. Develop linkages between school and these multiple partners to provide access to resources and information that can enhance the outcomes of the transition process.

3. Create traditional, as well as nontraditional alliances with community groups and organizations in order to fully expand options available.
