CLAIMS DETAILS Notice of Claim Written notice of claim must be given to addresses mentioned below within thirty (30) days after the occurrence or commencement of any loss covered by this policy or as soon thereafter as is reasonably possible. Claims Department – Travel Insurance

Claims Details for Common Carrier Travel Accident Insurance; Written notice given by or on behalf of the Insured Person to the Insurer or to any authorized Agent of the Insurer with the information sufficient to identify the Insured Person shall be deemed notice to the Insurer. (a) Claims Forms and Proof of Loss Upon receipt of a written notice of claim, the Insurer will furnish to the

claimant claim forms for filing proofs of loss. If the duly completed claim forms are not returned to the Insurer within (30) days from the date of furnishing the claim forms, the claimant shall be deemed to have failed to comply with the requirements of this section. Insured person shall provide the proof of loss within the stipulated time in this policy for filing proofs of loss as stated below to the Insurer.

Written proof of loss must be furnished to the Insurer within ninety (90) days

after the date of loss. Failure to furnish such proof shall not invalidate nor reduce any claim if it was not reasonably possible to provide proof within such time, provided such proof is furnished as soon as reasonably possible by the Insured.

All evidence, proof, information, accounts, receipts, certificates, statements,

reports and any other documents required by the Insurer shall be furnished at the expense of the claimant and shall be in such form and of such nature as the Insurer may prescribe.

The Insured Person shall as soon as possible after the occurrence of a loss which is likely to give rise to a claim under this policy obtain and follow the advice of a duly qualified registered Physician and the Insurer shall not be liable for any consequences arising by reason of the Insured Person’s failure to obtain or follow such advice and use such appliances or remedies as may be prescribed.

The Insurer at their own expense shall have the right and opportunity to

engage a Physician of their choice to examine the Insured Person whenever Insurer determines as required.

In the event of a death of the Insured Person, reasonable notice shall be

given to the Insurer as soon as possible before the burial or cremation and the Insurer may require to be present at a post-mortem examination on the body of the Insured Person. Immediate notice of time and place shall be given to the Insurer before the holding of an inquest. Time is the essence of this condition.

(c) Payment of Claim Indemnity for loss of life will be payable to the estate of the Insured Person.

All other indemnities for any other class of injury will be payable to the Insured Person.

If any indemnity of this policy shall be payable to the estate of the Insured

Person or to an Insured Person who is a minor or otherwise not competent to receive an indemnity, the Insurer may pay such indemnity to his trustee. Any payment made by the Insurer in good faith pursuant to his provision shall fully discharge the Insurer to the extent of such payment.

Benefit shall only be payable under one of the benefits shown in Section

2.2.1, Table of Benefits.

Claims Details for Travel Inconvenience Insurance; All information and evidence required by the Insurer or their agents shall be furnished at the expense of the Insured Person or his or her personal representative, no later than thirty (30) days after the event giving rise to the claim to addresses mentioned above. The Insured Person shall complete a Claim Form and submit it together with the following documents: (a) Photocopy of charge form verifying that relevant air ticket was

charged to a Citibank Credit Card Account; (b) Photocopy of Airline or travel agent's receipt for air ticket; (c) Photocopy of air ticket; (d) Photocopy of Airline boarding pass; (e) In respect of missed flight connection, flight delay or denied boarding,

a written confirmation from the Airline indicating the duration of delay and the reason(s) for it;

(f) In respect of delayed or lost luggage, a signed and dated copy of the

Property Irregularity Report obtained from the Airline; (g) Original receipts relating to the expenses incurred and details on

quantities and description of items purchased; and (h) Letter from the Airline indicating amount of compensation paid, if

any. Benefits payable under the Policy for valid claims will be credited to the Insured Person's Citibank Credit Card Account.

COMMON CARRIER TRAVEL ACCIDENT

AND

TRAVEL INCONVENIENCE POLICY

SECTION I - DEFINITIONS AND INTERPRETATIONS (1) Airline means an airline listed in the Official Airline Guide and updated from

time to time and the holds a valid certificate, licence or similar authorisation for scheduled air transportation issued by the relevant authorities in the country in which its aircraft is operating.

(2) Citibank Credit Card member means the holder of a valid PremierMiles

Platinum/ Titanium under Visa/MasterCard Card franchise (whether basic Citibank Credit Card member, or supplementary Citibank Credit Card member) issued by the Policyholder and issued in the United Arab Emirates.

(3) Citibank Credit Card Customer means a Citibank Credit Card member with

one or more Citibank Credit Card (basic card or supplementary card) issued by the Policyholder. For the purpose of Premium Calculation and Payment, the holder of supplementary card and holder of basic card shall be considered as one Citibank Credit Card Customer.

(4) The Company (Insurer) means American Home Assurance Company.

(5) Concessionary Fare includes: (a) Discounted tickets offered by a Public Conveyance operator; (b) Tickets offered on special promotions, including 'free tickets' given

together with purchased tickets, the cost of which have been charged by the Citibank Credit Card members to their Citibank Credit Card Account, provided both purchased tickets and 'free tickets' were issued in the names of Insured Persons;

(c) Tickets offered as part of Citibank Rewards Programs or similar

programs offered by Citibank with or without additional cost chargeable. In the event that cost is chargeable, 100% of such cost must be charged by the Citibank Credit Card members to their Citibank Credit Card Account.

(6) Country of Residence means the country in which the Insured Person is currently residing and holds a valid residency visa.

(7) Family Members means the following: -

(a) The Citibank Credit Card member's legally married spouse, and

(b) All the Citibank Credit Card member's legally dependent children under the age of 23 years and who are unemployed and unmarried.

(8) Full and Total Fare means the 100% fare payable by the Citibank Credit Card

member on standard fare or Concessionary Fare.

(9) Hazardous Activities means sky diving, parachuting, hand gliding, bungee jumping, scuba diving, mountain climbing or the like.

(10) Injury means bodily injury caused solely and directly by violent, accidental,

external and visible means occurring during the policy period and requiring treatment by a Physician.

(11) Insured Person means the Citibank Credit Card member or anyone of his Family Members as defined herein.

(12) Loss means the act or instance of losing and / or the disappearance of

something cherished and / or a measurable reduction in some substance or process.

(13) Master Policy means the insurance policy issued to Citibank N.A., UAE (14) Physician means a legally licensed practitioner acting within the scope of his

license practicing medicine, and concerned with maintaining or restoring human health through the study, diagnosis, and treatment of disease and injury. The attending Physician may not be:

(a) the Insured Person; nor (b) the Insured Person’s Relative

(15) Policyholder means Citibank N.A., UAE

(16) Professional Sport means a competitive sport used as a source of livelihood.

(17) Public Conveyance means any Scheduled Flight, or any land or water

conveyance which is duly licensed for the transportation of fare-paying passengers and shall exclude any private hire or rental vehicle or any conveyance operated for the purpose of sightseeing or tour.

(18) Relative means a spouse, parent, parent-in-law, grandparent, step-parent,

children, grandchild, brother, brother-in-law, sister, sister-in-law, daughter-in-law, son-in-law, fiancée, fiancé, half-brother, half-sister, aunt, uncle, niece or nephew of the Insured Person.

(19) Scheduled Flight means anyone of the following: (a) A flight on an Airline over a scheduled route (b) An additional flight operated by an Airline to meet demands over a

scheduled route (c) A chartered flight on an Airline for which tickets are issued through

appointed agents and available to the public at large Departure times, transfers and destination points shall be established by

reference to the Insured Person's Scheduled Flight ticket. (20) Sickness means any fortuitous somatic illness, sickness or disease. SECTION II – COMMON CARRIER TRAVEL ACCIDENT INSURANCE 2.1 Description of Events and Benefits The events against which insurance is granted under this Section are such injuries

occurred or the death occurred during a one-way or round trip taken by the Insured Person between the Point of Departure and the Destination (both as designated in the Insured Person's ticket) on or after the date of the ticket purchase; provided, such injury is sustained or the death occurred under the circumstances specified as follows:

(i) Such injury or the death occurred while riding as a passenger, and not as a

pilot or crew member, on or boarding or alighting from or being struck by an air, land or water Public Conveyance operated under a licence for the transportation of passengers for hire, provided the Full and Total Fare for such travel has been charged by the Citibank Credit Card members to their Citibank Credit Card Account.

(ii) In respect of such trip by air, such injury or the death occurred while riding as

a passenger in a private registered four-wheeled motorised vehicle or Public Conveyance operated under a licence for the transportation of passengers for hire, but only

(a) when going directly to an airport for the purpose of boarding an airworthy, licensed and scheduled common carrier aircraft on which the Insured Person is covered by the policy, or

(b) When at the airport terminal itself; or (c) When leaving directly from an airport after alighting from such

aircraft until they reach their next destination Provided that the Insured Person's Full and Total Fare for such trip by air has been

charged by the Citibank Credit Card members to their Citibank Credit Card Account. However, should such trip is part of a package tour, cover shall apply only if a

minimum of 80% of the total cost of the package tour was charged to the card. 2.2 Benefits If within one year from date of the accident such injuries sustained by the Insured

Person under Section II shall result in any loss as specified below, the Insurer will pay the sum as set out in Section 2.2.1, Table of Benefits provided that not more than one of these sums (the greater) shall be payable for such injuries resulting from any one accident.

Benefits for the Citibank Credit Card member's legally dependent children shall be

limited to a maximum of 25% of the Capital Sum Insured for the Citibank Credit Card member.

Indemnity provided hereunder will not be paid under circumstances for more than one of the losses, the greatest, and sustained by the Insured Person as the result of any one accident.

2.2.2 Benefits Extensions Major Burns sustained during Public Conveyance travel Major Burns means third degree burns covering at least 25% of the surface area of

the Insured Person's body, provided such Major Burns was sustained by the Insured Person during Public conveyance travel.

Corrective Plastic Surgery necessitated by injuries sustained during Public

conveyance travel Corrective Plastic Surgery Benefit reimburses the medical expenses incurred for

Corrective Plastic Surgery arising from injuries sustained by the Insured Person during Public Conveyance travel.

2.3 Maximum Limit of Indemnity

In no event Insurer will cover duplicate or multiple Citibank Credit Cards issued to an Individual Insured Person. Only one Citibank Credit Card will be eligible for the insurance coverage provided by the Insurer.

2.4 Exposure and Disappearance When by reason of an accident covered by this Policy, the Insured Person is

unavoidably exposed to the natural elements and as a result of such exposure, suffers a loss for which indemnity is otherwise payable hereunder; such loss shall be covered under the terms of this policy.

If the body of the Insured Person has not been found within one year of the

disappearance, sinking or wrecking of the vehicle in which the Insured Person was riding at the time of the accident, it will be presumed subject to the terms of this Policy, that the Insured Person suffered loss of life resulting from bodily injury caused by an accident at the time of such disappearance, sinking or wrecking.

If at any time after payment have been made by the Insurer for such claim, the

Insured Person is found to be alive, the benefit paid shall be refunded in full to the Insurer.

SECTION III -TRAVEL INCONVENIENCE INSURANCE 3.1 Operation of Insurance Coverage

The insurance coverage for the Insured Person shall operate only when he/she is taking a trip during the Period of Insurance on a Scheduled Flight as a fare-paying passenger and when the entire air fare for travel on such Scheduled Flight has been fully charged to a Citibank Credit Card Account maintained by the Citibank Credit Card member with the Policyholder.

However, should such trip is part of a package tour, cover shall apply only if a

minimum of 80% of the total cost of the package tour was charged to the card. 3.2 Benefits (i) Missed Flight Connection If the Insured Person’s (a) confirmed onward connecting Scheduled Flight is missed and no

alternative forward or onward functioning air transportation is made available to the Insured Person within four hours of the actual arrival time of his or her incoming flight; or

(b) Confirmed Scheduled Flight is delayed for four hours or more,

cancelled or the Insured Person is denied boarding of the aircraft due to overbooking and no alternative transportation is made available within four hours of the scheduled departure time of such flight

The Insurer will indemnify the Insured Person for reasonable and necessary

valid expenses incurred at the place of departure or missed flight connection in respect of:

(a) Restaurant meals or refreshments, telephone calls; and

(b) Where accommodation is required:

i. accommodation costs; ii. Transportation costs to and from accommodation; and iii. Emergency purchases of essential clothing and requisites if

luggage has been checked in up to the limit of indemnity as stated in the Summary of Cover. (ii) Luggage Delay Expense If the Insured Person's accompanied checked-in luggage with the common

carrier is not delivered to him or her within six hours of the Insured Person’s arrival at the scheduled destination point of his or her flight, the Insurer will

indemnify the Insured Person for incurred expenses at such scheduled destination in respect of the emergency purchase of essential clothing and requisites up to an indemnity as stated in the Summary of Cover.

(iii) Luggage Loss Expense If the Insured Person's luggage is lost or if the Insured Person's accompanied

checked-in luggage is not delivered to him or her within 24 hours of the Insured Person's arrival at the scheduled destination point of his or her flight, such luggage will be assumed to be permanently lost and the Insurer will indemnify the Insured Person for charges incurred within 120 hours of his or her arrival at such scheduled destination in respect of the emergency purchase of essential clothing and requisites in addition to those payable under item 3.2 up to a limit of indemnity as stated in the Summary of Cover.

3.3 Maximum Limit of Indemnity

Duplicate or multiple Citibank Credit Cards shall not obligate the Insurer to pay in excess of the limit stated for expenses incurred by any Insured Person as a result of any one incident covered under the policy. In the event that a Citibank Credit Card member is accompanied by his or her Family Members who are not supplementary Citibank Credit Card members, cover for all charges incurred under that Citibank Credit Card Account as the result of any one incident will be limited to the maximum amount stated under Summary of Cover.

4. General Exclusions This "policy does not cover any loss caused or contributed to by:

1. Pre-existing medical conditions;

2. Intentionally self-inflicted injury, suicide or any such attempt while sane or insane;

3. War, civil war, invasion, insurrection, revolution, use of military power or usurpation of government or military power; The use, release or escape of nuclear materials that directly or indirectly results in nuclear reaction or radiation or radioactive contamination; The dispersal or application of pathogenic or poisonous biological or chemical materials; The release of pathogenic or poisonous biological or chemical materials;

4. Any period an Insured Person is serving in the Armed Forces of any country or international authority, whether in peace or war, and in such an event, the Insurer, upon written notification by the Insured, shall return the pro rata premium for any such period of service;

5. Loss sustained or contracted in consequence of an Insured Person being intoxicated or under the influence of any narcotic or drug unless administered on the advice of a Physician;

6. Any Loss of which a contributing cause was the Insured Person's attempted commission of, or wilful participation in, an illegal act or any violation or attempted violation of the law or resistance to arrest by the Insured Person;

7. Any Loss sustained while flying in any aircraft or device for aerial navigation except as a passenger; exclusions include, but are not limited to, pilot, operator or crew member;

8. Travel in, to, or through any country the following countries: Afghanistan, Iraq, Cuba or Democratic Republic of Congo;

9. Failure to obtain the required visa;

10. Any costs incurred due to fluctuation in exchange rates;

11. Any Loss incurred where payment has been made using reward schemes;

12. Any expenses incurred in the Insured Person’s Country of Residence;

13. Bacterial infections except pyogenic infections which are caused by an accidental wound;

14. Hernia;

15. Flying in any aircraft owned, leased or operated by or on behalf of (a) the Insured or any subsidiary or affiliate of the Insured, or (b) an Insured Person or any of his Relatives;

16. Coverage with respect to flying is limited to Injury sustained during such trip while the Insured Person is riding as a passenger (but not as a pilot, operator or member of the crew) in or on, boarding or alighting from a scheduled airline.

17. Driving or riding as a passenger in or on (a) any vehicle engaged in any race, speed test or endurance test or (b) any vehicle being used for acrobatic or stunt driving;

18. Any claim caused by opportunistic infection or malignant neoplasm, or any other sickness condition, if, at the time of the claim, the Insured Person had been diagnosed as having AIDS (Acquired Immune Deficiency Syndrome), ARC (AIDS Related Complex) or having an antibody positive blood test to HIV (Human Immune Virus);

19. Any Loss sustained while the Insured Person is participating in any Professional Sports, Hazardous Activities or skiing whether indoor or outdoor;

20. Mental, nervous or emotional disorders including, but not limited to anxiety disorders, eating disorders, psychotic disorders, affective disorders, personality disorders, substance use disorders, somatoform disorders, dissociate disorders, psychosexual disorders, adjustment disorders, organic mental disorders, mental retardation and autism;

21. An Insured Person traveling against medical advice or to seek medical attention or advice or with a terminal condition which was diagnosed prior to their trip or when he is unfit to do so;

22. Any cardiac, vascular, cardio vascular or cerebral vascular Sickness or conditions or sequelae thereof or complications that, in the opinion of a Physician appointed by the Insurer, can reasonably be related thereto.

23. Any medical condition related to or arising from hypertension, if the Insured Person has received medical advice or treatment (including medication) for hypertension within the two (2) years prior to the commencement of their trip;

24. Services, supplies, or treatment, including any period of hospital confinement, which were not recommended, approved, and certified as medically necessary by a Physician;

25. Routine physicals or other examinations where there are no objective indications or impairment in normal health, and laboratory diagnostic or X-ray examinations except in the course of a disability established by the prior call or attendance of a Physician;

26. Elective, cosmetic, or plastic surgery, except as a result of an Injury;

27. Dental care, except as a result of Injury or Sickness to sound natural teeth caused by accident while the Policy is in effect;

28. Congenital anomalies and conditions arising out of or resulting there from;

29. Expenses incurred in connection with weak, strained or flat feet; corns, calluses, or toenails;

30. The diagnosis and treatment of acne;

31. Deviated septum, including sub mucous resection and/or other surgical correction thereof;

32. Organ transplants that competent medical professionals consider experimental;

33. Well child care including exams and immunizations;

34. Private hospital or medical care within the Country of Residence where public funded services or care is available.

35. Treatment provided in a government hospital or services for which no charge is normally made;

36. Eyeglasses, contact lenses, hearing aids, and examination for the prescription or fitting thereof, unless Injury or Sickness has caused impairment of vision or hearing;

37. Pregnancy and resulting childbirth, miscarriage or disease of the female reproduction organs and all related conditions, including services and supplies related to the diagnosis or treatment of infertility or other problems related to inability to conceive a child, birth control including surgical procedures and devices;

38. Sexually transmitted diseases and the conditions commonly known as AIDS or HIV and/or any related Sickness or condition including derivatives or variations thereof, howsoever, acquired or caused;

39. Any terrorist or member of a terrorist organization, illegal drug traffickers, or purveyor of nuclear, chemical or biological weapons.

40. Confiscation or quarantine or requisition by customs or other Government authorities

41. Failure of the Insured Person to take reasonable and necessary measures to save or recover lost luggage

42. Failure to notify any and all relevant airport/airline authorities of missing luggage at the destination point and obtain a Property Irregularity Report

43. Abandonment of luggage to the Airline or to its assigns or to its agent

5. Geographical Limits The Insurance afforded under this Policy shall apply anywhere in the world unless

otherwise amended or endorsed. 6. Termination The Insurance cover provided for any individual Citibank Credit Card member will
