Date and Time Stuck: 29-April-2009 @ 19:30 Date and Time Cement Pumped: 04-May-09 @ 17:OO Hole Conditions: Back reaming out of the hole through a ruble zone and experiencing total losses. Had

drilled through several faults Fishing Attempts: Attempted to jar on the fish from 20:OO on 29-April-09 until 09:OO on 30-April-09. Parted

drill string above the jars and came out of hole. RIH with a set of fishing jars and an accelerator to try to

fish the BHA. Engaged fish and jarred on fish for 12 hours with no results. Decision made to P&A.

Comments (what happened to get stuck?, etc.,): At 13:OO (13,465 ft. MD) noticed torque starting to ramp

up. Decided to pull out of hole to do a wiper trip to 12,000 ft MD. While back reaming out of the hole

experienced tight spots at 12,600 ft MD and 12,140 ft MD. Continued to work the tight spots, however

Became stuck at 12,020 ft MD. Rigged up for fishing operations.

The following is a summary of NRC and/or Agreement States regulations that must be followed when abandoning an irretrievable well logging source(s). The specific regulations are found in 10 CFR 39.15 and equivalent regulations in Agreement States. An irretrievable well logging source means any licensed radioactive sealed source that becomes lodged in a well and cannot be retrieved after reasonable efforts have been made to recover the source(s).

1. If a well logging source is irretrievable, the following requirements must be implemented.

a) condition of the Texas Railroad Commission (others occasionally).

The source(s) must be immobilized and sealed in place with a cement plug. The cement has to be dyed red in Texas as a

b) A mechanical deflection device must be set at some point in the well above the cement plug to prevent inadvertent intrusion on the source, unless the cement plug and sources(s) are not accessible to any subsequent drilling operations. The mechanical device can be devices such as a whipstock, old drill bit, etc. For LWD, drillpipe and/or collars left on top of the BHA usually are approved as a deflection device.

c) be mounted at the surface of the well unless the mounting of the plaque is not practical (i.e. subsea completion).

2. The STC RSO (or his designee) will notify the NRC or Agreement State of the abandonment plan developed by/with the client. The NRC or Agreement State must approve the abandonment plan prior to implementation. The federal and/or state oil and gas well permitting agency must also approve the abandonment plan. The contact with the well permitting agency is the responsibility of the well owner/client but we should advise himlher of that fact as a courtesy.

3. If any changes must be made to the abandonment plan submitted to the NRC or Agreement State, the STC RSO must be informed so that he/she can get approval of the modification, as appropriate. The actual abandonment must not begin until any abandonment plan or modification to that plan is approved by the appropriate agency.

Detailed Well Schematic depicting location and depth of tool(s), source(s), drill pipe, plugs, deflection device etc.

Any information pertinent to the abandonment that the STC RSO may not have for hidher final report.

e) If all data sent to the STC RSO is still applicable for the final report, an e-mail or fax is to be sent to the STC RSO confirming that fact so that he/she c a n be ensured that the data sent to the appropriate agency is totally accurate. Most facilities send a completely new report since many these documents often are incomplete or are poor quality fax reproductions.

If there are any questions regarding these procedures, discuss them with your Operations Manager.

FACILITY MANAGEMENT MUST ENSURE THAT THE FINAL ABANDONMENT REPORT IS SUBMITTED TO THE STC RADIATION SAFETY OFFICER.

7” liner shoe is at 10,630’, 6 1/8” hole drilled to 13,466’ with top of fish at 10,800’ Planned sidetrack calls for 2 runs, motor with milltooth bit to open hole sidetrack and RSS with PDC to drill to TD. Review paper on Shublik Phenomena.

Currently in the hole jarring with fishing assembly. When SFT arrives spot SFT outside of PBL in the annulus and allow to soak while continuing to jar for an additional 4 hours. If fish does not come free a call to town will be made to determine course of action. (If no movement probably abandon fish, if any movement probably continue to jar for another hour)

Schlumberger e-line is on the hook for a colliding sever tool. Call Schlumberger at 659-2487 or 659-2434, Blake is engineer, give Schlumberger a two hour heads up. Continue to jar until Schlumberger shows up.

Perform Derrick inspection and get ready to rig up Schlumberger E-line.

Rig up Schlumberger E-line and run in with colliding tool, CCL, and umbrella centralizer to sever HWDP just above drilling jars at -1 1,830’. Mix 50 bbls LCM pill as per Mud Engineer, to be spotted after backed off.

Pick up 20’ with E-line to position tool just above sever point. Pick up 15’ on Drill pipe to verify pipe is parted. Check for free pipe movement in open hole, if no movement down then do not pickup higher than the 15’. Pull out to surface and rig down Schlumberger E-line.

Pump LCM pill down and spot at cut point. Close pipe rams and pump down drill string to squeeze LCM into loss zone. Call Schlumberger cementers, give them a 2 hour heads up, Todd Jaggers is the cementer 659-2434 or 659-2487.

Attempt to circulate at 300 GPM and monitor for losses. Well must be healed up and support full drilling rate before moving onto the next step. If well is still loosing at full drilling rate prepare to spot a second LCM pill and squeeze down to loss zone. Have cementers rig up and blow air down to Schlumberger out of critical path.

RIH to 11,830’ and prepare to spot cement plug. Mix 60 bbls of 9.8 brine and load on Vac truck. Mix 25 bbls Fresh Water Hi Vis Biozan sweep.

Hold a PJSM with rig crew and cementers. Goal is to pump 200’ cement plug with red dye on top of fish.

a. Schlumberger to pump 5 bbls of water and test lines to 4,000 psi

b. Swap over to rig and pump 25 bbl Hi Vis Spacer

c. Swap back to Schlumberger and pump 10 bbls 17.0 ppg Class “G” cement plug with one gallon red dye (200’ plug with 30% excess)

d. Schlumberger to chase cement with 10.5 bbls fresh water

e. Rig to displace with 55 bbls of 9.8 ppg brine from truck followed by 44.3 bbls of seawater

f. Open bleeder and pull out of pill at 1.5 minutes per stand allowing cement to fall out of pipe. POH to 10,800’, drop nerf ball in drill pipe, wash down to 11,200’ and circulate a bottoms up.

11. PU 6-1/8” MX-R18DX bit and Schlumberger AIM motor with 1.83 degree bent housing, MWD with IIFR+MS and DIR/GWRES as per DD and MWD. RIH, dogleg required from this BHA is 12 degrees/lOO’. RIH filling every 20 stands. DO NOT FORGET TO INCLUDE PBL WHEN PUlMU BHA.

12. RIH to 10,750 and wash down with minimal pump rate to the open hole sidetrack point, less than 100 GPM (watch out for green cement). Open hole sidetrack as per Schlumberger DD at -1 1,200’.

13. Drill ahead as per DD to cross fault 3. Once across fault 3 a decision will be made when to come out and pick up RSS/Periscope. Watch for losses.

14. Pump out to the shoe, monitor well, record MW in and out and status of well in IADC. POH to pick up RSS assembly.

15. PU 6 1/8” MSX-513M-A2BI Hycalog PDC bit and Schlumberger Rotary Steerable with PeriScope/Azimuthal Density Neutron/VPWD/CCV as per DD and MWD. Run PBL circulating sub in case of losses. RIH fill every 20 stands. Attempt to work /wash past doglegs with out rotating if possible, doglegs above rotary limitations of RSS, MWD, and LWD tools.

16. Drill ahead to TD of -14,470’ MD, 8,646’ TVD. Pump sweeps if needed. At TD, circulate a sweep at maximum possible flow rate and DP rpm as allowed by Schlumberger DD.

17. Back ream to BHA build section doglegs, no speed limit as there will be no mad pass. Pump out to shoe, open circ sub, pump bottoms up at max rate, pump dry job.

18. If the hole is not in good condition do not open circ sub. Make wiper back to bottom.

19. Circulate sweep, possibly spot liner running pill. POH to 7” csg shoe and open circ sub. Circulate at max possible rate to clear casing of cuttings. Pump dry job and continue POH for 4.5” liner.
