2014 Pharmacy Day

By Binh Tran, Pharm.D.

Pharmacy Day at Skaggs School of Pharmacy and Pharmaceutical Sciences at UCSD is an awaited activity for many schools in San Diego. This year, the event was organized on February 22, 2014 for 132 students from Sweetwater School, East Lake, University, Hilltop, Southwest, Madison, and Morse High Schools. Crystal Zhou gave the welcome, and as usual, James Colbert, Associate Dean of Experiential Education gave an excellent presentation of the program of study at Skaggs School of Pharmacy, and acknowledging the support of coordinators from the San Diego schools and the wonderful participation of Pharmacy Day mentors and teachers on the campus. In the morning session, there were Safe Meds, Immunization, Compounding, Intravenous procedures, and in the afternoon: specialty pharmacy such as Diabetes, Hypertension and Asthma. For Safe Meds, the teachers projected slides explaining the safe use of medications, and as hands-on, students put a patient’s jumbled up drugs into the correct slots of her drug box, reading the directions from the drug vials. During lunch, a very informative session on organ donation was offered to all students in the main auditorium. In the afternoon, the diabetes session involved a large group of students and teachers. After the slide presentation and demonstration of the blood glucose test, everyone went outdoors for the Relay Race exercise activity: a mock step by step glucose test, a determination of glucose content in five beverages, trivia questions, and a labelling of food items on a plate. The last station was

exercise with jumping jacks (10), hula hoop twirls (10) and jumping ropes (10), timed by pairs of students. The teachers were also invited to add more excitement. Posters were made at the close of the session before everyone reconvened in the main auditorium for Dr. David Bao’s inspirational thoughts on his career in pharmacy.

Student Pharmacist Participates in the Organ Donation Awareness Challenge

By Francis Wang, P-3

Every year the American Society of Transplantation (AST) supports an annual event called “The Organ Donation Awareness Challenge”. The Challenge relies on collaboration with pharmacy schools and student chapters of pharmacy professional organizations to design and conduct at least one organ donation awareness event. This is the second year that University of California San Diego (UCSD) Skaggs School of Pharmacy and Pharmaceutical Sciences participated in AST’s Organ Donation Awareness Challenge. Our Organ Donation Awareness Committee (ODAC), under the guidance of Dr. Shirley Tsunoda was comprised of 27 student pharmacists. Our overall goal was to increase San Diego community’s awareness of the need for organ donors. At the beginning of the year, the ODAC planned our first event at the Vietnamese Federation Health Fair. In preparation for this event, we created a poster outlining the importance of becoming an organ donor, myths and facts about the organ donation process, and shared three stories from previous organ recipients. The student pharmacists educated patients about the organ donation process, including how to register to be a donor, the misconceptions of organ donation, and the growing need of a larger registry. Volunteers were able to discuss these misconceptions and hand out English and Vietnamese language fliers. If patients wanted to sign up, our team was able to help register them on the spot. We educated 53 patients at the health fair. Our second event was a school-wide event called Pharmacy Day. This annual event brings high school students to the UCSD campus to educate them on the profession of pharmacy. Our committee prepared an hour-long presentation that highlighted stories of organ donors and recipients, the organ donation process, the growing need for organ donation, the misconceptions, and how to register to be a donor. The high school students were very engaged during the presentation and asked many questions. We realized that many students had misconceptions about the organ donation process, and we were able to discuss these topics with the students. The ODAC was able to educate over 150 high school students at Pharmacy Day. Our next event was Triton Day, a university-wide event where incoming undergraduate freshmen and their families visit UCSD for an annual open house. Our committee had posters, flyers, and ribbons that we handed out to the students and their parents. We informed the UCSD undergraduate population about the growing need for registered donors. If people wanted to register to become a donor, volunteers were able to register them on the spot. We spoke to a total of 70 patients at Triton Day. Our biggest event of the year was the Annual Organ Donation Awareness Symposium. Our featured speaker this year was Dr. Kristin Mekeel, one of our UCSD transplant surgeons. She spoke about the process of donating, the type of organs that can be donated, and the misconceptions people have about donating. In addition, 5 organ transplant recipients shared their personal stories and the impact of receiving a life-saving organ. These deeply personal stories were moving and provided a uniquely emotional perspective. In total, 80 pharmacy and medical students attended. After the presentation, students had the option of signing up to become organ donors.

Lastly, during the month of April, our committee initiated a social media campaign. Every other day, the UCSD-CSHP Facebook page posted a statement about organ donation. For the entire month, we posted myths and facts, the donation process, and instructions on how to become registered donors. We posted examples of misconceptions such as: ‘MYTH: "If I am in an accident and the hospital knows that I want to be a donor, they won't try as hard to save me." FACT: The medical team treating the patient is completely separate from the transplant team. The transplant team is not contacted until the patient has died and the family has consented to donation.’ We also added posts that help support registered donors and encourage others to become registered donors “If you are already an organ donor, please like this post to show your support. AND if you are not a donor and have decided to register this month to be one, please like this post! Your decision to become an organ donor can save as many as 8 lives.” In addition to the Facebook page, everyone in the committee changed their Facebook profile picture to the green ribbon to symbolize being an organ donor. In total, we had 15 posts on the Facebook page and 123 total likes. Our social media campaign was a huge success. In conclusion, each of the five parts of the ODAC campaign was successful. We reached out to our pharmacy school, university, and community to provide much needed awareness on organ donation. We educated over 350 people. In addition, we added 23 people to the organ donation registry. Next year, we hope to continue and expand our efforts to educate the greater San Diego community on organ donation.

UCSD CPNP Chapter Conquers Phoenix

By Ibrahim Muradian, P-3 & Sadaf Rowshanrad, P-3 The College of Psychiatric and Neurologic Pharmacists (CPNP) chapter at UCSD may only be two years old, but that didn’t stop Skaggs students from leaving a mark at the annual CPNP conference this past April. The conference is intended to provide networking opportunities for both professional and aspiring neuropsychiatric pharmacists across the nation and was held in Phoenix, Arizona for 2014. Sadaf Rowshanrad (P3, CPNP UCSD Chapter President) and Ibrahim Muradian (P3, CPNP UCSD Chapter Community Affiliate) were among 17 students nationwide to be awarded the CPNP 2014 Annual Meeting Travel Grant. Recipients were selected by merit and CPNP Foundation President Barbara Wells, PharmD, FCCP adds “These students represent our future and the future of neuropsychiatric pharmacy.” Also in attendance was Conor Bowman (P2), representing Skaggs as one of only two research trainee award finalists and Poster Presenter for his study on “Caffeine Augmentation on Electroconvulsive Therapy Seizure Duration in Depressive Disorders”. Ibrahim Muradian was another participant in the poster presentation session, presenting his study on “Promoting Appropriate Prescribing and Reducing Polypharmacy within a Correctional Setting”. Andy Valine (P2) continued to show his enthusiasm and involvement in CPNP UCSD Chapter by attending his second annual meeting in a row. Dr. Kelly Lee, PharmD, BCPP, founder of CPNP UCSD chapter, was in attendance at the annual meeting and provided mentorship and encouragement for Skaggs students.

Global Advanced Pharmacy Practice Experience in Vietnam

By Jennifer Thu Pham, Pharm.D. During the last week of school for the Class of 2014, four P4s presented to deans, faculty, and fellow colleagues about experiences of their respective international rotations. Three P4s, Karen Hoang Kuusniemi, Jennifer Pham, and Thu Truong reported on their adventure in Vietnam. The affiliation with the Ho Chi Minh School of Medicine and Pharmacy allowed the team to forge connections in the settings of academic, clinical, as well as analytical pharmacology. The rotation started with a strong academic focus where our students observed the Vietnamese pharmacy education curriculum and system, led a therapeutic discussion on Parkinson’s Disease, attended therapeutics lectures and SOAP conferences, and presented on the topics of Diabetes, Hepatitis C, and Hospital Acquired Pneumonia in Vietnamese! The UCSD team is highly impressed with the depth and scope of the curriculum in regards to therapeutics, mechanisms, pharmacology, and beyond. Most unique is an added component of Eastern medicine and herbs that is largely absent from the curriculum in the states. Beyond the classroom, our team attended continuing education networking events at the Ministry of Health. Our team had the opportunity to rotate and round with clinical teams at several hospitals where we conducted clinical case

reviews and forums to discuss on antibiotic use. Due to non-adherence of guidelines; lack of stewardship programs, hand hygiene, and infection control; inappropriate selection of empiric therapy; and antibiotics being available over the counter; antibiotic resistance is rampant and is the focus of many discussions during this rotation and will likely continue to be at the forefront of conversation for the next several years. Most unexpected was a partnership forged with the Hospital of Tropical Diseases (HTD) and Oxford University Clinical Research Unit (OUCRU). In efforts to improve antibiotic stewardship, the UCSD team and Dr. Jennifer Le initiated implementation of vancomycin therapeutic drug monitoring at the HTD. Our team had the opportunity to help clinical pharmacists master how to initiate and adjust doses based on levels as

well as help the lab in one stage of HPLC optimization. As Vietnam is a developing country with limited resources, immunoassay was not available at this site. Methods are being devised by the OUCRU Pharmacology lab to quantify vancomycin in the blood with HPLC. One observation made by Truong during her rotation was that, “Since I got a chance to see how a private hospital works (very similar to the US system actually), I feel like the situation is not as bad as how the public hospitals portray

Page 12: UC San Diego Skaggs School of Pharmacy & … 2014 Spring Newsletter | 1 UC San Diego Skaggs School of Pharmacy & Pharmaceutical Sciences Newsletter . Spring 2014. Congratulations to

the healthcare in Vietnam […] though since the medical service provided at a private hospital is for those who can afford it, the system is still lacking in many ways.” During this rotation, we were not only students, but very much ambassadors of pharmacy in the United States. The message that our presence brought to the sites we visited was that several decades ago, clinical pharmacy in the United States was in its infancy stage as it is now in Vietnam and is a peek into the future of clinical pharmacy. For us, it was an irreplaceable experience where we were only one rotation from graduating and were transitioning into experts on clinical topics where we could lead academic discussions, evaluate literature to its limit as the prevalence of antibiotic resistance in Vietnam is not captured by typical guidelines we use in America, but most importantly we feel that we made an impact. We worked tirelessly to promote a culture to aim for constant progress and hope that we left our mark. We would like to extend our sincere thank you the following for their continuing guidance and support of this program: Dean Palmer Taylor, James Colbert, David Adler, Drs. Jennifer Le, Kathryn Nguyen, Tuan Dung Nguyen, and Binh Tran. For more information or copy of the presentation mentioned above, email [email protected].

John Robertson Class of 2010 is Ramona Hero John Robertson graduated from SSPPS in 2010. He is a community pharmacist in Ramona, California. Robertson was honored with the “Real Hometown Hero” award. John noticed one of his elderly patients did not pick up his prescriptions. He worried that something might have happened to him so he visited his home. When the man did not answer the door he became very nervous and contacted the Sheriff’s Department. They found the elderly man on the floor where he had been for up to two days. The senior citizen was sent to the hospital to recover. Ray Cordona, the Director of the Ramona Senior Center says, “To have someone else backing us up checking on our seniors, it’s a great thing.” John Robertson doesn’t think he was a hero, but we at SSPPS sure do! Faculty News Dr. Jonathan H. Watanabe and colleagues Dr. Rashid Kazerooni and Dr. Mark Bounthavong recently published their article "Association of Copayment with Likelihood and Level of Adherence in New Users of Statins: A Retrospective Cohort Study" in the Journal of Managed Care Pharmacy in January 2014. The authors evaluated the change in medication adherence as well as the likelihood of adherence for those paying copayments versus without copayments. Patients without a copayment for their statins had better adherence than those that paid a copayment. They also compared statin adherence for patients that have the same copayments for their statins, but differ in receiving some of their other medications for free. This revealed that patients who have copayments for their statins, but receive other prescriptions for free had worse statin adherence, albeit non-statistically significant, than patients who had a copayment for all of their prescriptions including statins. This suggests that patients may be determining the relative cost of their prescriptions they are taking and filling the prescriptions based on this relative cost. Medication out-of- pocket costs clearly influences whether a patient takes their medication as directed by their prescriber and should be a consideration for pharmacists to ensure that their patients' medication adherence is optimal. Below is an excerpt from the article: "Patients who did not pay a copayment for their statin medications were more likely to have adherence rates of ≥ 0.8 MPR and ≥ 0.9 MPR relative to the No Copayment Group with odds ratios (OR) of 1.19 (95% CI = 1.03-1.37) and 1.28 (95% CI = 1.11-1.48), respectively. The second analysis applied the VHA exposure categories of SC Copayment, NSC Copayment, and No Copayment. Using the 0.8 MPR or greater adherence threshold, the No Copayment group was associated with an increased likelihood of adherence versus the SC Copayment category as reference group with an OR of 1.31 (95% CI = 1.10-1.58). The NSC Copayment was associated with a non-significant increase in odds of adherence at the 0.8 MPR level or greater with OR of 1.12 (95% CI = 0.98-1.39)."

Highlights from the Winter 2014 Applied Pharmacoeconomics and Outcomes Research

Forum

By Jan Hirsch, R.Ph., Ph.D. The 10th Applied Pharmacoeconomics and Outcomes Research Forum was held February 10th, 2014 at the University of California San Diego, Faculty Club. The event was hosted by the Skaggs School of Pharmacy and Pharmaceutical Sciences and supported by grant funds from Allergan, Partnership for Health Analytic Research, and Ventegra. The topic for the Forum was “Biosimilars: Challenges and Opportunities for All”. Continuing our new theme of kicking off the Forum by testing the audience knowledge of somewhat little known facts we challenged the audience to name the Top 10 Most Important Drugs. “Most Important” was defined as“… how the drug changed whole infrastructure of the drug industry and the practice of medicine.” (John Swann, Historian FDA) Can you name the top 10 most important drugs? (Answers are on the last page of this newsletter.) This Forum we had our largest turn out so far – about 90 participants! Our speakers covered a wide range of content regarding the growing field and controversy surrounding biosimilar products. - What are biosimilars? - Why is there controversy surrounding this evolving class of drugs? - What is happening with biosimilar products in Europe? - What is likely to happen in the US regarding approval and marketing of biosimilar products? - What do pharmaceutical manufacturers and healthcare payers see as current and future challenges and

opportunities associated with biosimilar products? The link below will take you to the speaker slides with answers to these questions and more. http://pharmacy.ucsd.edu/faculty/pharmacoeconomics_forum.shtml

Purpose of Applied Pharmacoeconomic and Outcomes Research Forums The number of individuals in the Southern California region with interest in pharmacoeconomics (PE) is growing rapidly across the healthcare system - from those involved with creation of PE data within pharmaceutical and biotech companies to those incorporating results into decision making within a plethora of managed care organizations. The region provides an excellent opportunity to gather individuals to debate issues, and propose solutions that are vetted from multiple perspectives – not just individual silos defined by employer. The Applied Pharmacoeconomic and Outcomes Research Forum was created to facilitate this cross perspective communication. The goals of the forum are to: 1. Discuss commonly encountered obstacles to conducting or utilizing results of applied pharmacoeconomic studies

and explore solutions from various perspectives of the health care system. 2. Create an environment and foundation to foster the creation of a Southern California Pharmacoeconomic and
