DESCRIPTION

David was speaking about the UK Transitions Taskforce, which the STF currently stands as the Scottish representative of. David talked about why the Transitions Taskforce was needed and the on-going work and vision. Their aim or vision “is that all young people with life limiting or life threatening conditions will make the transition to adult services and live their lives as independently as possible according to their wishes, while receiving the care and support they and their families/carers need.” David then went on to discuss what the Transitions Taskforce has managed to achieve so far and invited people to become involved in Scotland. Scott suggested that he allows those who were down to attend the day as an email list to sign them up to the Network and those in attendance seemed in agreement to this. David then discussed with delegates the earnest work that was needed and other obstacles and future directions they hope to move in to enable the network to succeed. This included funding to continue the work. David also introduced everyone to Richard Whateley development worker for the UK Transitions Taskforce.

Transition Taskforce Vision

Our vision is that all young people with life limiting or life threatening conditions will make the transition to adult services and live their lives as independently as possible according to their wishes, while receiving the care and support they and their families/carers need.

Transition Taskforce Aims

For young people with life limiting or life threatening conditions

Build a UK-wide forum to drive the development and implementation of a strategy to improve transition.

Raise awareness and influence the policy and commissioning agenda and develop a strong and united voice on transition.

Stimulate new service development and partnerships between adult and children’s service providers at national, regional and local levels as appropriate.Develop a UK-wide hub of information, training, research and good practice in relation to transition, producing new resources as appropriate.
