UltiPro Enrollment Instructions
Newly Eligible, Qualify Life Event Changes or Open Enrollment

PLEASE READ
Dear Employee:
This booklet is an enrollment guide to provide step by step instructions on enrolling for
your benefits for newly eligible employees; employees with a qualifying life event or
annual Open Enrollment. New employees are provided 30 days from date of eligibility to
enroll in benefits or 30 days from the qualifying life event to enroll or un-enroll you and
your dependents from your benefits in UltiPro system. During Open Enrollment season
employees can review their benefits and make changes to their plans during a limited
time period. Employees can either elect new benefits or decline any benefits options
that they no longer wish to be enrolled in for the next year.
Please thoroughly review the Benefits Guide to determine which benefits plan which you
and your family would like to enroll in. Once you’ve decided please enroll your selections
into the UltiPro system. Please follow the instructions that is provided.
If you have dependents that you would like to enroll please also upload your dependent
documents into the system. The instructions are provided in this booklet or email it to
[email protected] or [email protected]. Fail to provide dependent
documentation will hinder in adding your dependents into your benefits.
The company provides an employer paid Basic Life Insurance and Basic AD&D Insurance.
Please ensure you assign a beneficiary. If you do not name one or more beneficiaries, the
proceeds will go to your estate at your death. Proceeds paid to your estate are subject to
probate and will incur all of the expenses and delays associated with settling an estate.
Named beneficiaries will receive proceeds immediately after your death, and probate is
bypassed.

Representative or the Benefits department for assistance.

Click on your Menu. Hover over the Myself tab and click on the option that applies to you, Life Event or Open
Enrollment.

Select the drop down to choose the reason for the life event.

Verify Beneficiary And Dependent Information
To verify or change dependents and/or Emergency Contacts:
1. Select the name link for the individual
2. Edit the necessary information, as needed
3. Select save
To add a dependent not already listed:
1. Select add (green plus (+) sign button)
2. Enter the contact information, as needed, including social security, date of birth and gender
3. Check the "Dependent" and/or "Beneficiary" check box as applicable.
4. Select Save

Confirm Your Elections OR Changes
This summary reflects the elections that will be deducted from your upcoming paycheck(s). If you need to make any
edits you can do so by selecting the plan type or plan description hyperlink to return to the election page.
Once you have reviewed your New Benefit elections click Submit

Confirm Your Elections Or Changes
This page contains personal information as well as benefits selected and declined. You can compare current benefits to
the newly selected benefits. Selected plan details include covered family members, pay period cost, and beneficiaries,
when applicable.
1. From the Confirm Your Changes page, review the selected and declined benefit type and plan details including
covered family members, plan beneficiaries, and cost information.
2. Return to the applicable page to make any changes, if needed.
3. Select Draft to continue the election at a later time or select Submit to complete your elections now. If you
select Submit, you cannot make any changes without the assistance of your Benefits Administrator. You can
print the Confirm Your Changes page. If you select OK, the Confirmation page appears.
4. Select Print to print a summary of your elections.
5. Select Close.
Please verify your changes carefully before submitting. If you need to make any edits you can do so by selecting the plan
type or plan description hyperlink to return to the election page. When you are satisfied with your changes, please click
the Submit button on the toolbar.

Add Employee Document
To add an employee document in UltiPro i.e. social security card for a dependent you will need to take a picture of the
document and save it as a file on your computer or your phone for easy access to upload.
The following files supported are: doc, docx, pptx, ppt, xlsx, xls, xlt, xltx, txt, rtf, tif, png, jpg, jpeg, gif, bmp, pdf, mht, db,
zip, xml, log, dpt, odt, ods
Once that has been completed follow the steps provided below.
Log into your UltiPro account.
Click on your Menu. Hover over the Myself tab and click on Life Event

Under Things I Can Do select Add Employee Document
Add Document
Select Choose File to find the document to upload and select Open.

Enter the title of the document.
From the drop-down menu choose the category the document belongs to.
If there is an expiration date provided on the document click on the calendar icon to indicate the expiration date.
Otherwise leave blank.
Select Save to complete the upload.

To view all documents uploaded to the UltiPro account click on the Menu. Hover over the Myself tab and click on
Employee Documents.

Add Additional Beneficiary / Dependent
Log into your UltiPro account.
Click on your Menu. Hover over the Myself tab and below Personal click on Contacts.
Click Add

The Contact is Active box is checked (default selection). Uncheck the box to create an inactive contact.
Personal Enter the contact's personal information listed.
Designation From the drop down select the contact's relationship to the employee. Designate the contact as a dependent,
beneficiary, or emergency contact. More than one option can be selected.
Address

The address section will automatically default to the employee’s address. If the address is different from the employees
check mark the box Address is different from employee. Enter the contact's address, telephone, and e-mail information.
Click Save to complete.
