Northwest Family Services & North Clackamas School District• We served over 200 in 11 schools. • The schools provided child care.• Free production and hosting of a video podcast• Bilquist Elementary was a good example of a school/church

partner providing snacks, refreshments, and family board games to give away. They also had a couple interested in becoming mentors who went through our mentor certification process.

• Our contact person moved to a new school district and now both are partners.

OFA – HMRF Grants award extra points for partnerships.Our opening paragraph:This community-centered approach provides six of the eight allowable Healthy Marriage and Relationship activities in four of Oregon most populated counties in the greater metropolitan Portland area. The collaborative approach between NWFS as the lead agency partnering with Multnomah County Health Department (MCHD) and Catholic Charities of Oregon (CC) will provide comprehensive services: relationship and marriage education, employment, and other support to low-income couples and indivduals.

Affordable Health Care Act• The Community-Based Care Transitions Program,

created by Section 3026 of the Affordable Care Act, tests care delivery models for improving care transitions from the hospital to other settings and reducing readmissions for high-risk Medicare beneficiaries.

• Up to $500 million in total funding is available for 2011 through 2015 to community-based organizations partnering with hospitals and other providers to provide care transition services to effectively manage Medicare patients' transitions and improve their quality of care.

the Hospital Engagement Networks, have pledged to join this Partnership in a shared effort to save thousands of lives, prevent millions of injuries and take important steps toward a more dependable and affordable health care system. They include:

• Hospitals and national organizations representing physicians,

nurses and other frontline health care and social services providers committed to improving their care processes and systems, and enhancing communication and coordination to reduce complication for patients.
