Use the below instructions to get your previous 6 completed months of quality measures from Care Watch. Not all of the Quality Measures are in care watch.

You will need to get the rest from Medicare.gov

1) Goto Medicare.gov

2) Click on Find Nursing Homes

3) Under the location search field enter the city name in which your facility is located and click search.

4) Scroll down until you see your facility name and then click on your facility name.

5) Click on the tab that says “Quality of Resident Care”

6) Scroll down and you will find your most recent Long Stay and Short Stay Quality Measures. Enter only the quality Measures not included in care watch. DO

NOT enter any other quality measures from Medicare.gov other than the ones not included in Care Watch. i.e., ED Visit—Short Stay, Hospital Readmission-

Short Stay, Successful Discharge-Short Stay

Click on “Reports” , Then “Quality Measures”, then

“QM Benchmark”.

Complete the Quality Measure fields below by entering your

current percentage for each quality measure QM Date Range

previous 6 completed months. Example: Todays date 5/2017

and date range should be set 11/1/16-4/30/17.

Check the box that says “Show CMS 5-Stars QMs only”

Check the box that says “Exclude Medicare Other”

Click Display

These are your QM’s you enter into the dashboard.
