ENROLLMENT FORMS OVERVIEW Certain Treasury Management services require additional information to complete the setup. If assigning access to these services to a user, you must also create and submit an enrollment form to provide the additional information needed for the setup.

The following services require enrollment forms:

• Image Lockbox

• File Transfer Manager

• eStatements

• International Trade Online

1. First add a new user or modify an existing user within the User Maintenance widget to add one of the entitled services listed above. (See the Add a New User or Modify a User steps in the User Administration Guide)

2. Scroll down to the Enrollment Forms widget on the User Maintenance Page. To create a new enrollment form, click Add Enrollment Form.

3. Select the user that needs access to the services and select the service for which you want to submit the enrollment form and click continue.

4. Fill out the enrollment form and click submit. (click Save, to save the information you started)

5. Please continue to follow each step on the individual forms.

IMAGE LOCKBOX FORM Image Lockbox allows clients to review receivables, resolve exceptions and view financial reporting in as

soon as 30 minutes after deposit processing. Clients are able to view images of deposited checks and

documents via the internet, and if subscribed to the additional Real Time Decision service, are able to

“Accept”, “Reject” or provide missing information to rejected items. All accepted items will be processed

and deposited with the same day’s work, saving the client multiple days of float.

LOCKBOX ENROLLMENT FORM

First Time User Set Up

Now that the user is set up for Commercial Services, the user needs to be set up with Lockbox as well. This form will be submitted to gain user access to the Lockbox system.

1. Select your user

2. Select Enrollment Form: Image Lockbox

3. Click Continue

4. Select User Type:

a. If the user is new click the radio button First Time User Setup

5. Select Settings:

a. Custom Settings- If you want to define the criteria and do not have a user with this set up

b. Model After An Existing User- select this option if you want to set up the user with the same setup as an existing user

If you selected Custom Settings:

6. Enter Lockbox Number (1) - if you have more than one you can append by adding more with the Add Another Lockbox Number

7. Select radio button on whether you want Decision Rights

a. Yes

b. No

8. If you have multiple lockboxes and a Master Company - type in the Master Company name in this field

9. Additional Information - please provide any information/details that are important to the set up

10. Click Save to save the form if you do not want to submit yet

11. Click Submit if you want to submit the form through to the set up area

If you selected First Time User Setup and Model After an Existing User:

1. User Type and Select Settings- are already selected 2. Select the drop down for: Select Existing User

a. All users will be shown, select the user to model permissions after 3. Click Submit if you want to submit the form through to the set up area

Note: Click Save to save the form if you do not want to submit yet

Modify Existing User (adding service(s) to an already set up user)

Now that the user is set up for Commercial Services, the user needs to be set up with Lockbox as well. This form will be submitted to gain user access to the Lockbox system.

4. Select User Type:

a. If this is a modification to a current user, (ie: adding the service to an existing user), click the radio button Modify existing g user.

5. Select Settings:

a. Custom Settings- If you want to define the criteria and do not have a user with this set up

b. Model After An Existing User- select this option if you want to set up the user with the same setup as an existing user

If you selected Custom Settings:

6. Enter Lockbox Number (1) - if you have more than one you can append by adding more with the Add Another Lockbox Number

7. Select the radio button to Add or Remove the provided Lockbox Number

a. Add Number to User

b. Remove Number to User

8. Select radio button on whether you want Decision Rights

c. Yes

d. No

9. If you have multiple lockboxes and a Master Company - type in the Master Company name in this field

FILE TRANSFER MANAGER FORM File Transfer Manager allows clients to send and retrieve receivables and payment files with M&T

Bank. M&T Bank automatically encrypts all incoming and outgoing files with PGP encryption.

FILE TRANSFER MANAGER (FTM) ENROLLMENT FORM:

Now that the user is set up for Commercial Services, the user needs to be set up with the FTM application.

This form will be submitted to gain user access to the FTM system.

First Time User Set Up

1. Select your user

2. Select Enrollment Form: File Transfer Manager

3. Click Continue

4. Select User Type:

a. If the user is new click the radio button First Time User Setup

5. Select Settings:

a. Custom Settings- If you want to define the criteria and do not have a user with this set up

b. Model After An Existing User- select this option if you want to set up the user with the same setup as an existing user

If you selected Custom Settings:

6. Select Accounts: Select the file (service) the user needs to be set up for

a. Account Recon – Incoming

b. Account Recon – Outgoing

c. ACH- Acknowledgement – Outgoing

d. ACH – Incoming

e. ACH Reporting – Outgoing

f. ACH- Returns – Outgoing

g. BAI – Outgoing

h. Big Batch Wire Incoming 80 Record Length

i. Big Batch Wire Incoming 94 Record Length

j. Commercial Credit Card – Outgoing

k. EDI – Incoming

l. EDI – Outgoing

m. Lockbox

7. Click Save to save the form if you do not want to submit yet

ESTATEMENTS FORM eStatements allows users to access Checking, Savings and Account Analysis statements online. Up to

seven years of historical statements are available for users to view, save and/or print

ESTATEMENTS ENROLLMENT FORM:

Now that the user is set up for Commercial Services, the user needs to be set up with the eStatements

application. This form will be submitted to gain user access to the eStatements system.

First Time User Set Up

1. Select your user

2. Select Enrollment Form: eStatements

3. Click Continue

4. Select User Type:

a. If the user is new click the radio button First Time User Setup

5. Enter Account Number:

a. Select the file (service) the user needs to be set up for

6. Select Statement Type

a. Checking/ Savings

b. Billing (analysis)

7. Select Statement Delivery

a. Electronic

b. Electronic and Paper (receive both)

Note: An additional fee for the delivery of paper statements may be incurred.

8. For additional account (s) that need to be set up for eStatements click Add Another Account

Number ( ) additional rows will append up to 10 accounts

9. Click Save to save the form if you do not want to submit yet

10. Click Submit if you want to submit the form through to the set up area

Modify Existing User (adding service(s) to an already set up user)

INTERNATIONAL TRADE ONLINE FORM International Trade Online (ITO) allows clients to manage their Letters of Credit and Documentary

Collections. Furthermore it allows clients to create, submit, and track trade instruments and transactions

through a web-based interface. Clients will be able to send and receive trade-related messages through the

site, generate reports about their organization's trade activities, and maintain reference data.

INTERNATIONAL TRADE ONLINE (ITO) ENROLLMENT FORM:

Now that the user is set up for Commercial Services, the user needs to be set up with the International

Trade Online (ITO) application. This form will be submitted to gain user access to the International Trade

Online system.

First Time User Set Up

1. Select your user

2. Select Enrollment Form: International Trade Online

3. Click Continue

4. Select User Type:

a. If the user is new click the radio button First Time User Setup

5. Select Settings:

a. Custom Settings- If you want to define the criteria and do not have a user with this set up

b. Model After An Existing User- select this option if you want to set up the user with the same setup as an existing user

If you selected Custom Settings:

6. Select Profiles

a. Create and Authorize b. Create/ Input c. Review and Authorize d. Inquiry and Reporting e. Custom (For customer security profiles please call 410-244-4800 or 410-244-4802) f. Please click on the Security Profile Permission Details caret to see all permission details

7. Select to: Receive International Trade Online E-mail Alerts

a. Select radio button Yes or No
