Aim of the courses In 2011, six CME-accredited training courses were held on the subjectof complex perianal fistulas, and in particular, on the new mini-invasiveVAAFT (Video Assisted Anal Fistula Treatment) technique.
The first course saw the participation of Italian surgeons, but by thesecond course, the participants came from overseas countries, such asRomania, Malaysia, India, Spain…
Thus, the technique has spread to numerous other countries and, up tonow, there have been numerous tutoring requests.
In 2012, the aim will be to enrich the scientific content of the coursesand extend participation to a greater number of surgeons, given thelarge number of requests coming from all over the world.
Therefore the courses will be international.
The official language will be English.

Scientific significance
Perianal fistulas are one of the most significant proctological disorders, and their treatment has constituted and still continues to constitute one of the major problems for both patients and surgeon.
Eminent surgeons of the past used to say they would have preferred to perform a complex surgical procedure on the pancreas rather than treat a complex perianal fistula, never mind a relapsing disorder.
Complex perianal fistulas. A social problem.
This “reputation” that perianal fistulas have acquired results from the fact that conventional surgical treatments have never been certain of cure, and indeed however, may have been and continue to be a potential cause of relapse. This may be seen in the high rate of relapse and the exponential increase in complications, such as faecal incontinence.
Fistulas are commonly classified according to their relationships with
the sphincter, but we wish toemphasise that even a simple fistulabecomes complex if it doesn’t healafter the first treatment.
The number of patients affected byperianal fistulas is high. Many ofthese, after having been subjected tosurgery on several occasions withoutsuccess, refuse other procedures,and frequently forego treatment,especially if the alternative is
creation of an ostomy, without any certainty of cure.
In any case, the only solution for definitive treatment is surgery.
Traditional techniques, such as fistulotomy and fistulectomy have been shown to be effective in a high percentage of cases of simple fistulas. In the case of complex fistulas, the results depend on the relationship with the sphincter and whether or not there are secondary tracks or abscessual cavities.

In recent years, mini-invasive techniques have been proposed which, besides safeguarding sphincter integrity, minimise the extent of surgical scarring, thus improving patient quality of life. This implies simplifying the surgical procedures themselves in addition to the use of synt
biologically-derived materials. The only drawback is that, just as with the traditional techniques, the procedure is performed “blind”. The innovative aspect of VAAFT lies in the direct vision inside the fistula, both during diagnosis (fistuloscopy) and treatment. A new way to consider a great problem.

In 2011, there were 6 CME-accredited VAAFT courses (9 credits per course). 6 participants were envisaged per course.
Applications for participation in these courses, now fully international in nature, have been so numerous that there is talk of organising an extra international course in December 2011.
In 2012, given the high number of applications for participation in the courses, the aim is to “raise” the quality of the courses by increasing both the number of participants and inviting highly qualified and well-known figures in the field of coloproctology as teaching staff.
The courses will not be CME-accredited (a condition that has proved restrictive with the other events) and the location will be Rapallo, for both the theoretical and practical sessions.

The courses location will be “Nostra Signora di Montallegro” Hospital in Rapallo. Starting this year, the theoretical session will be
conduct in the auditorium and the practical session in the operative theatres on the first floor, with direct links to the auditorium.
Sponsoring companies involved shall be Karl Storz, Johnson & Johnson, Covidien, Nathura, SigmaTau, SapiMed, GEM and other.
Registration fee: to be communicated soon (approx. € 500). There will be 4 events (the dates are indicated on the cover page): they have been decided based on the national and international events calendar.
For each course, one or two well-known international coloproctology specialist shall be invited to teach, such as for example John Nicholls, Yehael Ziv, Herand Abcarian, Tomas Skricka, Francis Seow-Choen. The specialists will be asked to give a 40 minute to one hour “lecture” where there will be the opportunity to interact and ask questions.
The courses will be held on Fridays (theoretical sessions) and Saturdays (practical sessions).
Considering the close proximity of Rapallo with Santa Margherita Ligure and Portofino, the accommodation and dining facilities will remain in Santa Margherita Ligure, with a shuttle service provided for course participants and teaching personnel.
A potential layout of the course program is shown below along with the names of the specialists teaching the upcoming sessions.
The final program will be showed toward the first fortnight of November 2011.
