For OJT – A training schedule of major operations & tasks to be learned, including number of hours required for the duration of the program

For Apprenticeships - meet state criteria for standards of apprenticeship published by the Department of Labor pursuant to 29 U.S.C. 50a

• Designation of Certifying Official form - this form is completed to designate the individual(s) who will be responsible for the accurate and prompt certification of enrollment and changes in enrollment for eligible trainees

• Training agreement
