WHAT IS TRANSITION?

IDEA (Individuals with Disabilities Education Act) federal special education law requires that school districts help students with disabilities make the transition from school to work and life as an adult. Transition services provide students with disabilities opportunities to develop strengths, interests, and preferences.

The transition plan is a component of a child’s IEP (Individualized Education Program) with services coordinated by the Transition Coordinator and the student’s IEP team.

During the students’ transition years (ages 14-21), three main goal areas are discussed with students and their families. These areas are:

Employment Education Independent Living

EMPLOYMENTSchool Based: (6-8 weeks)School based training provides the opportunity for students to learn and practice employability skills in the school setting. Some examples include: laundry services for athletes, career and technical programs, die cutting service, recycling, and mail delivery. We also have work experience classrooms that focus on a variety of skills (i.e., fine motor, social, sports team management).

Community Based: (6-8 weeks)Community based training is a non-paid opportunity for students to practice employability skills in a real employment environment. We partner with many community businesses to offer a variety of experiences. Some examples include: Goodwill Industries, St. Vincent de Paul, Schlow Centre Region Library, PAWS and Discovery Space.

Paid Employment:Paid employment is for students who demonstrate proficient employability skills. The IEP team determines the student’s participation and job coaching services. Some examples of employment placements are: Penn State Redifer Dining Commons, IHOP, Eat’n Park, and Penn State Commons mailroom.

INDEPENDENT LIVINGLifeLink Apartment: Students take turns living in this apartment with an apartment coach, who oversees their stay and aids in the teaching of various life skills. The lessons at school are then based on outcomes from experiences at the apartment and from dialogue with the students, parents, and transition coaches. This opportunity is for students who are junior status standing or older, or as determined by IEP team.

CATA Bus Training: (6 weeks)Students can sign up for the CATA bus training during the Extended School Year summer program. Students are assigned a one-on-one coach, who helps them learn how to read bus schedules, find stops, and understand transfers.

EDUCATIONStudents and parents can gain information about post-secondary education through in class activities, school visits and presentations. There are parent post-secondary luncheons, countywide post-secondary night, and monthly transition days. Some schools that we visit are:
