1:1 Online Tutoring ServicesTutoring by 1:1 Online Tutoring Services is done on the online platform. For those students who do not have a laptop or access to the Internet we provide them with the same free of cost; the student's all work is done on the computer so the student is not required to bring any materials. Our virtual classroom has a whiteboard to promote communication, collaboration and understanding among students. The main features of our tutoring program include One-to-One instruction, live-tutoring, use of state-aligned curriculum, flexibility, high quality researched material, experienced tutors, continuous monitoring of student’s progress and involvement of teachers and parents at each step of the student’s development. Typically our classes are conducted 2 to 3 times a week

with each class being of 60 to 90 minutes duration provided before and after school, weekends and during summer. We provide a minimum of 30 hours of tutoring per subject to each student, in any one of the subjects i.e. Reading, Math or Science. Our services will be offered before/after school, weekends and summer.

We begin our services by administering a diagnostic assessment whose results are used to formulate the learning plan. We strictly follow the individual learning plan during the program. Students take sessions mostly from the comfort of their homes, but they may be able to join our sessions wherever they have an access to internet. So, no transportation is required. All tutors who work with us have at least bachelor's degree, and most tutors have experience in tutoring Title I students. We have expert tutors who use their skills to teach IEP students effectively. For students with disabilities we offer the ability to change frequency of the sessions as well as the length of session. Instructional materials are available in verbal and non-verbal modes. For LEP students, while our materials are in English, we offer Spanish translators to assist students. In the years 2011-13, we have collected data on over 1400 SES students (K-12) nationwide. Average student improvement across grades levels in reading, math and science is one grade level equivalent.

24/7 Online Education24/7 Online Education provides students with an interactive web-based software program. Since 2010, we have served about 700 K-12 students in Reading, Math and Science. The average student improvement showed an improvement of 40% in Science, 35% increase in Math, 40% increase in Reading. We provide laptop and internet connection free of cost to every student; thus, students can take tutoring sessions from the comfort of their homes, and no transportation is required. For LEP students, while our materials are in English, we offer a computer program to translate Spanish - English to assist students. For students with disabilities, we offer the ability to change frequency of the sessions as well as the length of session. Instructional materials are available in verbal and non-verbal modes.

Our program begins with a student diagnostic test. The diagnostic test results are discussed with classroom teachers and parents to formulate a learning plan. Students will complete lessons and activities per the learning plan. We provide a minimum of 30 hours of instruction per subject in Reading, Math or Science. The student can choose only one subject during the program. The student needs to complete assigned activities every week. Typically, the student would spend 4-5 hours a week using the tutoring program. Each student is able to access our program anytime (i.e, before and after school, weekends, and summer) and is assigned a tutor who works with him/her by appointment in the program. Tutors interact with students once a week to discuss skills and concepts. The instruction is provided by tutors one to one. Tutors are required to have a four-year college degree and a minimum of one year experience dealing with Title 1, ELL, and/or students with special needs. In an effort to ensure that our tutors are capable of improving student’s academic achievement, we hire tutors with teaching certifications and teaching experiences who are subject matter experts in the discipline they tutor. 24 Hours Tutoring is teaches students through animations and interactive lessons on the computer, combined with frequent remedial interactions with tutors. Scores in math of students performing 2 grade levels below their current grade level increased by 15%, and students performing 1 grade level below increased by 21% from the pre-assessment to the post assessment. Scores in reading

We provide a minimum of 30 hours of tutoring per students in reading or math. Based on the student’s performance, we will develop an individualized learning plan, and the tutoring will be based on that plan. Our teacher student ratio is always 1:1, and instruction is available throughout the year, 7 days a week from 8 am to 8 pm; students take only 4 sessions a week, and each session is for 1.5 hours each. As our sessions are conducted via Internet, they are generally available from the student’s home. All our instructors a minimum of a bachelor’s degree and at least 2 years of successful teaching experience. All our services are provided online, so the student does not need to bring any additional supply. Working parents can get their children tutored without having to commute to and from tutoring sessions. Our computer program is available in Spanish, and our tutors are bi-lingual. For students with disabilities our software has developed special strategies that:• Provides students with chapter outlines or study guides that cue them to key points in their readings.• Keeps oral instructions logical and concise. Reinforces them with brief cue words. Repeats or re-words complicated directions.• Frequently verbalizes what is being written on the whiteboard.• Paces instruction carefully to ensure clarity.• Uses plenty of examples, oral and otherwise, in order to make topics more applied.

5 Star Tutors 5 Star Tutors has provided Supplemental Educational Services to over 800 K - 12 students since Oct 2012. We pretest the students, then develop a Student Learning Plan based on the pretest results. We instruct the students according to the ILP. We have a strong record of success with all students. Students completed our reading program with average growth of 31%. Students completed our math program with average growth of 37%. For both reading and math, our minimum growth target is 25%. We tutor students in a respectful, supportive learning environment individually (one-to-one) and in small groups of one tutor to 5-10 students. We provide each of our students with at least 30 hours of tutoring in either Reading or Math. We typically offer 1 to 2 sessions per week with each session being 1 to 1.5 hours in length. We provide tutoring every day of the week, before school, after school, weekends and during the summer to meet the needs of parents’/families’ schedules. We generally hire tutors who have either a bachelor's or master’s degree in education. The minimum qualification required by 5 Star Tutors is 60 college credit hours in education or a related program. 5 Star Tutors provides all instructional materials and technology (including computers/tablets and internet connections) needed for tutoring.

Our tutoring sessions are customized to help each student build on his or her strengths and meet their specific challenges. We offer special scheduling and assignment extensions to support students with disabilities. In tutoring students with a disability, we provide comply with student's individualized education program (IDEA) or the student's

individualized services under Section 504. For Spanish-speaking students, we provide Spanish-speaking tutors and Spanish-language material; however, our emphasis is English-language development, so we tend to use the Spanish-language materials as supplemental to the English-language material. Our tutoring sessions may take place at student’s schools and nearby community locations (libraries, community centers, places of religious worship, and non-public schools). By tutoring at students’ schools or nearby locations, it makes transportation easier for parents/families. 5 Star Tutors does offer transportation when we are able to contract with a local vendor.

A+ Tutoring Service A+ Tutoring Service provides students in Kindergarten through 12th grade with small and medium group tutoring in reading and math. Based on our pre-test and post-test data from more than 2400 students that completed at least 15 hours of our reading and/or math course since 2004, those students show an average grade level growth for grades K-8 greater than one year. Our students often begin the program below grade level, and many have learning disabilities (example: ADHD and ADD) and speak little English. Before students begin tutoring, they are tested to tell us the correct level at which to begin tutoring, and this testing provides information which leads to goals in the student's learning plan. For our Spanish-speaking students we have Spanish-speaking teachers, and for students with very limited English we will supplement our materials with Spanish language materials. For our students with disabilities, we provide assistive equipment, adjustments in the physical setting and presentation of materials or non-written formats. We also provide an aide to assist, under some circumstances.

A+ Tutoring Service offers a minimum of 30 hours per subject of instruction in reading and/or math. We provide 1 instructor for a maximum of 12 students (Small to Medium Groups). Our average group size is 8:1. We ensure our services are available to students with working parents by offering year-round instruction before and after school, weekends, and during the summer. Our session scheduling ranges from 2 - 5 sessions per week for 60 – 120 minutes each. A+ Tutoring Service seeks to minimize the need for transportation by providing services on-site at students’ schools. We collaborate with school districts at all times to ensure that students have safe passage to and from the tutoring site. All of our instructors possess a minimum of a bachelor’s degree. 75% of our instructors have advanced degrees and the majority of our instructors are state certified teachers. We provide ALL instructional materials and equipment necessary for tutoring sessions; students and families never have to supply any materials or equipment.

Above & Beyond LearningAbove & Beyond Learning provides aggressive supplemental education that is consistent with district practices and structured to meet the needs of students in reading or mathematics in grades K-12. Each student is evaluated prior to tutoring using an assessment that identifies skill gaps which are incorporated into the student’s learning plan, which serves as the basis for tutoring. Since 2010 Above & Beyond Learning has successfully serviced over 3000 students in 8 states with an improvement of at least 15% from pre to post test and a program completion rate of 95%.

Our services can be provided in multiple settings with an emphasis on individualized instruction for at least 30 hours per subject in Reading or Mathematics. Our main program is the Individual tutoring program where we send one of our highly trained

instructors for a direct, individual and intensive learning experience. Our program takes parent choice and ease into consideration, and we strive to conduct tutoring in locations that are easy to reach (school, place of worship or community location) and by providing transportation services where possible. Individual tutoring will be offered weekdays from 6 AM until 8 AM or 4 PM until 7 PM and weekends from 9 AM until 7 PM year round. Individual tutoring will be conducted at least once a week and no more than 2 times a week for 1-2 hours per session. Group tutoring will be offered weekdays from 4 PM until 6 PM during the school year. Group tutoring will be at least once a week for 1-2 hours per session and no more than 2 times a week for 1 hour per session. We offer group tutoring programs with one instructor for no more than 10 students. In the online tutoring program students are provided with a computing device (e.g. netbook or tablet computer), internet connection (as necessary), and secure access to computer-assisted instructional lessons; a live Tutors via online chat or phone is also providing instruction. In all cases we supply all materials and equipment needed.

Above & Beyond Learning requires all tutors to have at least 60 semester hours or 90 quarter hours with multiple years of instructional experience and strives to work with only the best state certified teachers. Our staff has ample experience working with all special populations and LEP students. While we accommodate for setting, presentation, response, format, and scheduling/time we only offer equipment/technology accommodations to vision impaired students. While our materials are in English we supply bilingual instructors for the following languages: Spanish, French, German and Italian. Achieve HighPointsAchieve HighPoints (AHP) An effective provider in the SES market since 2006, Achieve HighPoints (AHP) is an online math tutoring program for students in grades 2-12. AHP teaches students in a 1:1 tutor-student ratio. We also have live tutors available if needed. We require these virtual tutors to have a degree in math or related fields. Our program is available in English only. However our SES Handbook is available in Spanish, and our tutors are bilingual when required. Students go online for 3-4 one-hour sessions per week with a minimum of 30 hours per student. Students can attend online tutoring sessions before and after school, 7 days a week between 7:00am and 11:30pm. Since students can work from any internet-enabled computer whenever it is convenient for them, there are not the usual time and transportation barriers found with traditional tutoring. We may be able to loan families a laptop for program use. Since the whole program is based on the computer students do not need to bring any additional materials.

The process starts with students taking an assessment to identify their strengths and challenges. Based on the results of the assessment, an Individual Learning Plan is created and all tutoring is based on this plan. Since 2005-06, over 16,500 SES students have used our platform and experienced a 17% increase between pre and post-assessment results, where a gain of at least 12% would be significant. In addition 83% of our students reported that their math grades went up in school. In our home state of Georgia, 65% of students passed/exceed the state math tests where the average of all students is 47%.

African American Images has successfully tutored over 1250 students since 1988. Those students include 450 SES students since 2006. Those 450 SES students ranged from K-8th grade averaged a 1.8 grade level equivalent increase in math and 1.9 in reading. We give students a pre-test to determine their strengths and weaknesses. We then develop a learning plan using our Open Court curriculum to address those weaknesses. We continue bi-weekly testing for diagnostic purposes and utilize our curriculum to address continued weaknesses. Our goal upon conclusion is to have all weaknesses eliminated. We offer a minimum of 30 hours each in reading and math. The exact number of hours is determined by the school district based on their financial resources. We utilize a student tutor ratio of 5:1. We will tutor 3 days per week for one hour after school. We also provide tutoring during summer months. We will tutor inside the school. All of our staff are certified by the state of Illinois. We will provide all curriculum materials, paper and pencils. The student is not required to bring anything. We do not provide transportation, fortunately because we tutor in the school right after class, the only transportation needed is going home.

All Students Succeed/Sylvan Learning Centerl Students Succeed/Sylvan Learning CenterAll Students Succeed, a Sylvan Provider, offers both reading and/or mathematics instruction; we will provide a minimum of 30 hours. Parents may choose either subject, reading or math, or both subjects depending on district funding. All instruction is carried out in small groups. As a Sylvan provider we use the same Sylvan and Ace it! curricula and pedagogies as all other Sylvan franchises. Nationally, since 2008 Sylvan has served over 45,000 K-12 students in Reading with 81% of them showing at least a half of a grade level improvement on our post assessment after attending a minimum of 18 hours of tutoring. In Math, we have served over 18,000 students grades 1-12 with 83% students performing better by at least half of a grade level after a minimum of 18 hours of tutoring with our program.

Our teaching allows us to personalize our program for each child because we use detailed assessment to identify the educational needs of every child. Students with disabilities can be given smaller assignments, given extra time, and be given a smaller teacher to student ratio, and accommodations will be made for physical limitations. Tutors will have an Associate's Degree; however, we prefer they possess a teaching certificate or at the very least a Bachelor's Degree. Student-to-teacher ratio is no more than 12 students per teacher. But the average is 8 students to one teacher. Parents are invited and encouraged to attend a conference to discuss progress toward program goals every 15 instructional hours. Instruction can take place at a location most convenient to all parties (parents, students, schools, districts). The most common place for instruction is the school where transportation is less likely to be needed to get the child to the program. Some districts and/or schools may provide transportation home. Instruction time is most commonly held immediately following the school day, typically two to four hours a week spread across the weekdays but could also include Saturdays, before school and/or summer if a need for that is presented. Another location possibility would be at our Learning Centers. All transportation to and from any of the learning centers is the responsibility of the parent/guardian. The schedule for in-center services is based on our hours of operation and parental schedule. We are also capable of providing instruction at community centers as long as it is approved by the district we are serving. In all locations all instructional materials and equipment are provided.

ATS Project Success ATS Project SuccessATS Project Success is an AdvancED NCA accredited organization that has tutored students since 2003. The majority of ATS students begin our program more than one grade level equivalent behind their grade level; we have great success bringing them up to and above grade level with an average of 30 hours of tutoring. In 2011/12 school year, 7000 grade K-8 students achieved an average grade level equivalent gain of 1.05 in Reading and 0.89 of an grade level equivalent gain in math. In the 2011/12 school year, 500 grade 9-12 students achieved an average growth of 20% in Reading/Language Arts and 24% in math from pre to post test.

ATS offers eligible K-12 students a minimum of 30 hours per subject in reading and/or mathematics. We offer services online in the student’s home with a loaned, pre-programmed computer; however the home must have a land line telephone, existing high speed Internet or be in a T-Mobile or Verizon wireless coverage area in order for the student to participate. The student is first assessed to determine his/her starting level. Based on the assessment, a timeline, the curriculum and other program elements will be set in an Individual Learning Plan, which will drive the tutoring. While students set their own schedule, they are encouraged to work a miniumum of 2 hours per week in order to complete 4 to 5 activities. Each tutor works one-on-one with their students through weekly online monitoring of student progress and weekly telephone calls to the student and parents (English/Spanish/Hmong/Somali). The program is available 24 hours a day, 7 days a week. While our materials are in English, we offer translators in Hmong, Somali and Spanish to assist students. For students with disabilities we offer the ability to change frequency of the sessions as well as the length of session. All tutorial staff are high school graduates and either have an Associate’s degree (or its equivalent). Most tutorial staff are certified in the subject area being taught or have passed the Michigan Subject Area Test for Teacher Certification. Also, 49% of our tutors have 2 or more years of experience tutoring SES students in the program. When hired, tutors complete a 60-hour training program, which includes training in curriculum and working with at-risk students. Tutors engage in professional development throughout the school year.

AYP Services Inc.  AYP Tutoring offers individual instruction to meet individual student needs. Since 2007, we have served about 3,000 K-12 students in reading, with an average an increase of 8.6 points from pre-test to post-test, where an increase of 5 or more points is significant; in the same period, we have served over 2,500 K-12 math students, with an average increase of 9.1 points from pre-test to post-test, again with a 5-point threshold.The program begins with a assessment to identify each student's specific learning needs. AYP Tutoring uses the results to create a learning plan to guide each student's reading or math instruction.

AYP Services Inc.The AYP Tutoring program includes a minimum of 34 hours of instruction per subject in reading or math. We have a maximum of 8:1 student-teacher ratio. All instructional

materials are provided by AYP Tutoring. Students are not required to provide any curriculum materials. Tutoring usually occurs after school two times per week, one hour to one-and-a-half hours per session. AYP Tutoring classes are held at the child’s school. We also offer SES during summer hours. Parents need to make arrangements to pick up their children after the tutoring sessions. For our LEP students, we offer our materials in Spanish, and we have bi-lingual teachers. For students with disabilities, we offer extended time, shorter assigments, adjustment for the physical setting, assistive technology, and the option for nonwritten presentation of material and student responses. AYP tutors must at a minimum have a high school diploma and an associate’s degree or its equivalent. However we have a preference for elementary and high school certified teachers. We also have a hiring preference for individuals who have classroom experience with children.BabageBabbage Babbage Net School’s educational program consists of an instructional management system supported by standards-based assessment tools for grades 1-12. The program begins with an assessment in each subject area. After the assessment’s scoring, an individual learning plan is created to compile a set of lessons that will meet the targeted educational needs of each individual student. Students are presented with a specific concept and then use that information to work through sets of practice problems.

We offer a minimum of 30 hours per subject in reading and/or math, all with a 1-to-1 ratio of instructors to children. Ours is a computer-based tutoring program; therefore, the curriculum is available to the student at any time they log in to their computer. However, students who have questions can contact a live tutor via our toll-free Hotline from 12pm to 8pm Monday through Friday. Any lesson may be revisited at any time and at any place to increase a student’s mastery of that subject or skill. Students work through the program at their own pace, according to their own level of skill and comfort. The Babbage Net School tutoring program is available year-round to serve students. All of our tutors have a minimum of a bachelor's degree and are retired or current teachers. For our LEP students, we prefer our tutors to have experience and certification with ESL populations. We have tutors who are fluent in Spanish. Our teaching materials provided on the laptop or tablet are only available to students in English; therefore a student should have some knowledge of English. The program is provided on a free pre-installed laptop or tablet to the student at the beginning of the program. No Internet access is required, and to access the toll-free hotline, the student only needs access to a phone. Since the services are provided on a pre-installed laptop or tablet, no special transportation is required for participation. Babbage Net School maintains a strong record of effectiveness. As of 2011 we have served over 25,000 students Nationwide in Reading and Math for grades Kindergarten through 12th grade. Students in our program realized achievement gains on average of one grade-level equivalency through the course of tutoring.Brain HurricaneBrain HurricaneBrain Hurricane uses a direct-model of instruction using hands-on learning activities tied to content standards. Additionally, instructors may engage students with adaptive electronic content from Brain Hurricane’s online database using supplied tablet devices. Our program begins with each student taking a diagnostic test, which is used to help create an individual learning plan; the plan is the basis of the tutoring for that child.

Studies have shown gains averaging about two-thirds of a grade level in Reading, while the gains in Math were consistently an entire grade level or more. Depending on the per-student allocation, we provide between 10-30 hours of tutoring per student, per subject, in math and/or reading. Our maximum group size is 12 students, but typical groups are between 3-6 students. We typically provide service two days per week for two hours at school sites. We require a minimum of 4 students within a 3-grade range in order to provide services at a schools. Our staff members have at least an Associate’s degree or its equivalent and many of our instructors have advanced degrees and/or teacher certifications.

Brain Hurricane provides all instructional materials, including lessons and manipulatives. Brain Hurricane provides service to LEP students whose native language is Spanish. We provide instructors who are fluent Spanish speakers, and while most of our materials are in English, we do have some modified materials for the youngest children we serve that are in Spanish. Additionally we provide services to students with disabilities. We work with schools to provide the appropriate modifications and accommodations as dictated by the student’s IEP or 504 plan, and uphold the spirit of this by minimizing the impact of their disability and by maximizing their access to our general education curriculum. Specifically, we provide assignments that are smaller in length and give more time for the completion of those assignments. We provide materials in non-written formats and provide opportunities to respond in non-written manners, and we make adjustments in the physical setting of our tutoring. We typically provide services at school sites to minimize the need to transport students. Brain Hurricane may provide transportation depending on circumstances.BrightStar OnlineBrightStar OnlineBrightStar Online offers online tutoring in Math or Reading/ELA to students in grades K-12. An internet ready computer is provided upon request however the parent must provide the internet access. Parents and students have great flexibility in choosing their tutoring schedule as tutoring is done online in the student’s home. Transportation is not required. Teacher/student ratio is 1:1. Sessions are conducted 2 to 3 times a week for 60- 90 minutes per session before or after school, on weekends and/or during the summer. We provide a minimum of 30 hours of tutoring per subject, in any one of the subjects (Reading/ELA or Math). Students will be provided with the necessary materials and equipment. For LEP students, we offer bilingual instruction in Spanish and provide verbal and non-verbal materials in Spanish as well as English. Our program addresses the varied learning needs of all students including those with different learning styles and specific learning disabilities. The IEPs for students with disabilities will be consulted. Student will communicate with the teacher by phone, email, text and/or by electronic whiteboard. Access to our software will be available for one year. Our tutors have at least a bachelor's degree some of those maybe certified teachers. They receive extensive and ongoing training in effective instruction, assessment, and progress monitoring. Between 2010 and 2012, we provided tutoring to 2284 K-12 SES students (reading and mathematics) in South Carolina and Texas. In all cases, except 11th-grade reading, the average student improvement was equal to or greater than one grade-level equivalent improvement, and for those 11th-grade students, the increase was slightly less than one grade-level equivalent improvement.Brilliance Academy

Brilliance Academy Brilliance Academy offers programming to students in both communication arts (reading and writing) and mathematics. Between 2004 and 2012, Brilliance Academy served over 7,600 students nationwide in grades Kindergarten through 12th in both English and Math. Through the course of our service, students realized achievement gains on average of 18 percentile levels between pre- and post-testing. For example, if a student were initially tested in the 60th percentile it would mean they scored higher than 60 percent of other students taking the same assessment. With an 18 percentile level gain, they would be scoring higher than 78% of the assessment's standardization pool of nearly 2500 students .

Brilliance Academy uses a pre-test for student academic needs. The results of the test are used to create an individual learning plan, and the plan is the basis of the tutoring. Our program offers a minimum of 30 hours per subject in reading and/or math. Our maximum ratio of students to instructors is 8 to 1. Typically, sessions are 1 to 2 hours long, and we offer 1 to 4 sessions after school (Monday through Friday) per week yielding 4 to 8 hours of tutoring weekly. Services are ideally provided at the local school facilities. This eliminates the need for transportation to the program. Parents would only be responsible for pick up at the conclusion of the program unless the district offers an after-school bus program. Services may also be provided at non-public schools, libraries, community centers and our business location. In these cases, transportation to and from the facility may be provided by Brilliance Academy. In addition to the after-school program, Brilliance also provides services on weekends and over the summer. Each student is provided with all necessary materials; the student need not supply any additional material or equipment.

All tutors must hold a minimum of a bachelor’s degree, are certified teachers, and must also be certified by the National Tutor Association. For our LEP students, we prefer tutors who have some experience and certification with ESL populations. Our teaching materials are available in English and Spanish, but we have tutors who are fluent in Spanish, French, Urdu, Guijrati, Malagasi and Hindi. Cambridge Educational Services Cambridge Educational Services Cambridge provides a minimum of 20 hours of tutoring per subject in Reading and/or Math. Tutoring is conducted at the student’s school twice a week, two hours per session, for ten weeks, with an instructor. All materials — including math books and reading books —are supplied by Cambridge at our expense. Cambridge has a maximum student:instructor ratio of 15:1 and averages four to six students per class. Parents are responsible for picking up children after the program. Whenever possible, Cambridge also makes sure that the tutoring schedule does not conflict with other after school programs (we also offer tutoring before school after school and during the summer). Students take a pre-test to measure current skill levels. The pre-test is used as the basis for a student's Individualized Tutoring Plan (ITP). All tutoring follows that tutoring plan. At the conclusion of the program, a post-test shows how each student has improved. Teachers are certified and have at least three years of classroom teaching experience. While written materials are in English, we have bilingual teachers who are fluent in English and Spanish for LEP classrooms. For some students with disabilities, we can provide a specially trained teacher to accommodate for scheduling and format of

Since 2004, Cambridge has served over 13,000 SES students in grades K-12 in reading and math. For grades K-8, based on the Iowa Test of Basic Skills, there has been an average improvement of 14.28 points from the pre-test to the post-test for reading, and likewise average gain of 15.67 points in math, where a gain of 6 points or more is significant. For grades 9-12, based on the ACT, there has been an average improvement of 1.6 points from the pre-test to the post-test for reading, and likewise average gain of 3.3 points in math, where a gain of 1 point or more is significant.

Carter, Reddy & Associates, Inc.Carter, Reddy staff teaches students to read textbooks, write sentences and paragraphs, and use mathematics content and strategies to solve mathematics problems. Students learn in small group (nor more than 10:1 student-to-teacher ratio)settings where they learn to use a specific process to make sense from text they read. Since our inception, we have serviced approximately 8,000 K-12 students nationwide. 73% of students in ELA and 74% of students in Mathematics have demonstrated average gains (based on pre-assessments and post-assessments) of .5 grade level equivalent growth or more.

In reading, students learn to use Reciprocal Teaching strategies, strengthen skills in phonics, vocabulary, comprehension, fluency, and phonemic awareness. In mathematics, students learn and master basic mathematics skills and concepts within whole numbers, fractions, decimals, percents, geometry, and algebraic concepts; students learn 6-8 specific problem solving strategies and practice applying these strategies under the guidance of the certified mathematics tutor. LEP students (Spanish, Creole, Arabic) and students with disabilities will receive modified programs based on assessments, school data, as well as parent and teacher inputs. Program modifications may include specialized instructors (bi-lingual, special education), modified lessons, extended sessions, additional equipment/technology and additional evaluation sessions to more closely monitor progress.

C&R offers 30 hours minimum per subject (one subject/year) in reading OR math based on funding available. Students are tutored using explicit instruction, guided and independent practice. Each session is one to two hour long (before or after school, weekend sessions, or summer). C&R offers its services at the school the student attends, community centers, faith-based centers, public libraries, company sites, colleges, and non-public schools. C&R strives to provide locations convenient to the parents within their neighborhoods if NOT on school grounds in case transportation is an issue. Where possible, all tutors are certified teachers. All staff have at least 60 semester hours or 90 quarter hours from an accredited institution or associate's degree. C&R provides students with all materials for instruction.Chess Academy LLC (Ho Math and Chess of Illinois)Chess Academy Chess Academy offers a different kind of reading and math program to students in grades 1-12. We offer word problem-solving and mathematical chess puzzles to further give students more opportunities to develop skills in critical thinking and problem solving. This type of quality program is often not available to low income earners and disadvantaged kids. Chess is used as a learning tool to improve students’ skills in

mathematics and/or reading. We have been using the Compass Assessment Tool and Achieve Assessment Tool since 2008 with a total of 1,844 Chicago Public School, South Berwyn and Cicero students in grades 1-8, who showed an average reading gain of 14.92 percentage points and an average math gain of 16.48 percentage points, where gains of at least 3.00 percentage points and 3.23 percentage points, respectively, are considered significant. For grades 9-12 we have served 25 students, who showed an average reading gain of 39.55 percentage points and an average math gain of 42.16 percentage points, where gains of at least 8.90 percentage points and 6.50 percentage points, respectively, are considered significant.

Chess Academy uses an assessment tool for both pretest assessment and posttest assessment. The pretest assessment is used to create an individualized learning plan to target students' learning gaps. Chess Academy will use the same curriculum as that used in the Ho Chess and Math Program in Canada. The model of instruction will be small group instruction with no more than 10 students for each teacher. The program will involve a minimum of 30 hours per subject in reading and/or math with an average of 3-4 hours of instruction per week. The program will take place before school, after school, on weekends and summer. All instruction will take place at the host school (which will minimize transportation issues with parents) or at a community center, YMCA, Library, non-public school or community college. All staff will have the minimum of a high school diploma and an associate’s degree, or its equivalent of 60 semester hours or 90 quarter hours of college credit. All instructors will be trained using the Chess Academy teaching method and curricula. Our program offers bilingual instruction (monolingual materials and bilingual tutors) in Spanish, Hungarian and Russian. We make accommodations to students with disabilities by giving them extended time, shorter lessons, non-written material and responses and adjustments to the physical environment. All instructional materials will be provided by Chess Academy. Students are not required to provide any instructional materials. The instructional materials provided to students will include a reading book with a chess theme, reading and math workbooks, paper, rulers, pencils and an official United States Chess Federation tournament style chessboard, pieces and carry-on bag. Chess Academy may supplement instruction with a hand-held tablet device preloaded with chess instructional software.Chi Tutorsz Chi Tutorsz The Chi Tutorsz model of instruction is a combination of online and face-to-face tutoring, and it begins with a diagnostic test. The diagnostic test reveals student strengths and weaknesses in reading and/or math, and this leads to the creation of an individual learning plan that will guide the tutoring each child receives. Chi Tutorsz has provided SES instruction since 2006 to over 2200 students in grades K thru 12th, with an average improvement of 23 points for reading/language arts between our pre and post tests. Our math students show an average improvement of 20 points. In both subjects, a gain of at least 5 points is considered significant. We offer a minimum of 30 hours per subject in reading or math. Our student-teacher ratio is a maximum 5 to 1.

Our services are offered before school, after school, weekends, and during the summer. For ESL/ELL students, we provide bilingual tutors that speak and write both Spanish and English fluently. We also have materials in Spanish. Recommended schedule for delivery of services is a minimum of 4 hours per week per student. The preferred location of services is onsite at the student's school; however, we do provide services at community

centers, libraries, private/non-public schools, community centers/colleges, and churches. The majority of our tutoring staff is composed of certified, highly qualified Illinois teachers. We also employ college students that are pursuing a degree in Education. They must have completed a minimum of 60 semester or 90 quarter credit hours with a minimum GPA of 3.0. We provide all instructional materials and equipment needed including a computer with Internet access.

The Chi Tutorsz program utilizes a web-based curriculum with in-person instructor guidance. We offer two options for delivery of our services: 1) web-based instruction on-site at schools (where available), with the instructor providing small group instruction in-person, which limits parent transportation responsibilities to getting children home after the tutoring; 2) for the convenience of working families who cannot accommodate on-site after school tutoring sessions, we also offer our web-based program in students’ homes with live tutors supporting students via phone and Internet.

Club Z! Inc. Club Z! Inc.Club Z! has provided tutorial services in both reading and mathematics since 1995, and has tutored over 100,000 students under No Child Left Behind (NCLB), including students with disabilities and English Language Learner students. Our instructional model allows for no more than ten (10) students per one (1) instructor. During the 2011-12 school year, Club Z! serviced 728 students in grades K-12 in Illinois, and the average gain in reading was 2.0 grade level equivalencies, and the average gain in math was 1.8 grade level equivalencies.

Upon being selected as a student’s SES provider, Club Z! administers a diagnostic assessment to pinpoint the student's academic deficiencies in reading and/or math. After the assessment has been administered and scored, Club Z! personnel meet with the parent(s)/guardian(s), classroom teacher(s), and appropriate school district personnel to formulate an Individual Learning Plan (ILP).

Club Z! offers flexible scheduling to accommodate working parents, including before school, after school, on weekends and during summer and other academic breaks. Club Z! does not provide transportation. However, all sessions take place at locations convenient to families and easily accessible by public transportation to ensure that transportation needs are minimized. Club Z! tutorial sessions can be held at public and non-public schools/district sites (where applicable), community centers, libraries, places of religious worship, community colleges, or other public locations directly after regular school hours, at times convenient for working parent(s)/guardian(s). Students are grouped by age, grade, and academic needs. All instructional materials and curriculum are provided by Club Z!. Tutoring sessions are provided at times convenient for parents/guardians and average between 1 to 3 hour sessions, 2 to 4 times per week dependent on student need. The length of the Club Z! program entails a minimum of 30 hours of instruction per subject, in reading and/or math, depending upon available district funds.

Club Z!'s instructional staff includes certified teachers, degreed professionals with Doctorate, Masters, Bachelors and/or Associates degrees, and individuals with a

minimum of 60 semester hours of college credit, or 90 quarter hours of college credit. Club Z! instructional staff have widespread experience in working with Title I and academically at-risk students, students with disabilities, and students with limited English proficiency. Preference is shown to applicants with teaching certifications, experience teaching reading and/or mathematics, and/or individuals with advanced degrees, as well as specific experience working with Title I students.

In the case of students with disabilities, Club Z! will provide necessary accommodations such as scheduling/time, format, setting, equipment/technology, presentation, or response and/or modifications such as simplified curriculum, prioritizing key concepts or shortened or alternative assignments. With respect to LEP students, while our materials are in English, Club Z! utilizes bilingual tutors to ensure effective tutoring can take place in a multitude of languages including Spanish, French, Italian, German, Haitian Creole, American Sign Language, Arabic, Cantonese, Chinese, Karen, Creole, Farsi, Hindi, Hmong, Korean, Lao, Portuguese, Russian, Somali, Vietnamese and Tagalog. In addition, instructors working with bilingual students are trained to utilize the philosophies and principles of the National Teachers of English to Speakers of Other Languages (TESOL) to assist them in educating English Language Learners and effectively communicating with their families.  

College Tutors of Glenview Comp Ed II, IncCompEd II, Inc. serves students in grades 3-12 in reading and math. Since 1996, CompEd II, Inc. has served approximately 400 students in grades 3-12 in reading and math. For students in grades 3-8, the average improvement has been 0.8 grade equivalent for language arts and 0.5 grade equivalent in mathematics. 2012-2013 is the first year we are serving grades 9-12 in SES. Therefore, we have no SES achievement data yet. At the beginning of tutoring services, we will give our students an initial assessment, to obtain baseline literacy and mathematics data. An individual learning plan will be developed based on the initial assessments, and tutoring will follow that plan.

Our instructors will provide educational services on-site, at the children’s school. There will be no more than 10 students for every instructor, and the students will be tutored at least 30 hours per subject in reading and/or math. We will use cooperative learning groups. Our schedule will vary, according to the availability of space. For example, sessions might meet twice a week, for two hours per session, after school. Another possibility is that sessions might meet twice a week for 1.5 hours per week. All of our instructors have bachelor's degrees. All instructional materials will be provided to the students. All of our materials are available in Spanish and English. For our special education students, we allow for non-written presentation of materials and student responses, the subdivision of assignments and tests into smaller units, and adjustments to the physical setting. We do not provide transportation; however, we minimize the need for travel, because we serve the students in their home schools.

These activities are designed for student engagement in small groups or independent learning. Skills are presented in a continuum (from basic to more difficult) that allows each student to enter the skill at his or her own level. In this way, teachers are able to tailor the instruction in order to address the specific individual needs of every student.

From 2007-08 to the present, students who have been enrolled in the CEC SES reading and math tutoring programs have scored significantly above the expected increase in the difference scores between the pre and post tests. In Reading our students' average difference score of 15.9 and in math a difference score of 15.9. The expected outcomes are 5.47 in reading and 8.12 in math.

Each student will receive a minimum of 30 hours of tutoring in reading (grades K thru 12) or math (grades 6 thru 12), and the ratio of students to instructor will never be greater than 10 to one. The schedule for delivery of services will vary depending on the needs of the student: for example, sessions may occur before school, after school, weekends, or during the summer months, usually delivered 2 or 3 days per week for an average of 90 to 120 minutes per day. Students will receive all materials and journals. Services will be provided in the schools of participating districts and occasionally at places of religious worship. CEC tutors are certified teachers.

All lessons are pre- and post- tested. Student progress is monitored throughout lessons to gauge the duse of extra supports and/or enrichment activities. Although our materials are in English, CEC provides bilingual teachers to assist Spanish-speaking students. Accomodations offered to students with disabilities include modified assignments, extra time, alternative ways of presenting material, and changes in the physical environment. After school tutoring is offered in schools where students can access the activity bus to take them home, or at a place of religious worship, in which case, no transportation is provided. Educate OnlineEducate Online has served more than 60,000 SES students in grades k-12 in reading and math since 2002. Average grade gains were about one year's growth in academic skills. All sessions are delivered online by a live teacher. Students may log on to our secure website from home, or they may elect to take sessions in a school computer lab or in a temporary lab in a community center. Educate Online provides computers and internet access to families who need them to participate in the program. Educate Online computers are shipped with security software embedded that only allows access to our website. Families incur no costs with the Educate Online program. When students take sessions from home, transportation is not required. Students taking tutoring sessions at school will only require transportation home.

Educate Online offers a minimum of 24 hours in reading or math. Students generally 15

take 2 to 4 one-hour sessions per week, but parents may choose the schedule that works best for their family. Sessions are offered after school and on weekends year-round. Sessions are delivered by state-certified teachers. Teachers may work with up to three students at a time, but each student logs on to his or her own virtual classroom, so their experience is entirely one-on-one. An online diagnostic test is administered at the beginning of Educate Online programs to determine each student’s individual skill gaps and provide a baseline measure upon which to determine each student’s program progress. Results of the pre-test are used to develop a student learning plan that is an individualized prescription to address that student’s specific skill gaps. At the conclusion of the program, a post-assessment test will be administered to identify mastery and areas of continuing need for instruction.

Educate Online can serve LEP and Special Needs students, assuming their ability to perform necessay computer functions. While our materials are all in English, we do have Spanish-speaking teachers who will work with students to learn the material. For our students with disabilities, we can make some accommodations: extended time to complete assignments or tests and the ability to have non-written communication between the student and teacher. Our experienced prescription monitors will incorporate available IEPs and 504 plans into their personalized learning plans.

Education Futures Corp. Educational MastersEducation Masters challenges and engages all students by addressing the wide variety of abilities and learning styles with its interactive and adaptive curriculum. We offer instruction to grades sixth through twelve grade, including second language learners, in reading or math. Education Masters has data in the 2010-2012 school year in Illinois for over one hundred students in grades 9-12 with an average grade level gain of one year and four months in reading/language arts and one year and nine months in mathematics. Through the use of our unique assessment techniques, the Basic Achievement Skills Inventory, we can identify the areas in which the individual needs assistance and tailor an Individual Learning Plan and curriculum to target instruction. These methods inspire, stimulate, and connect with the child. The lessons provide further adaptation through the use of dynamic questioning, a process that generates questions as a lesson progresses rather than preparing all questions in advance.

Our model of instruction is that we administer a diagnostic test to see where the students’ needs are and then we provide instruction to those needs with a minimum of 30 hours of reading or a minimum of 30 hours of math. We then re-assess the student to see how they progressed. Because we are an online program, our software program can be served 24 hours a day, seven days a week, including summer. Tutor support is available at least 9-5 Monday through Friday and may be available at other times. Our suggested time frame is for at least three sessions per week at a minimum of one hour per session. The student chooses when and where to do the tutoring program to meet their needs; however students must maintain a cumulative 2 hour average of tutoring per week or they may be dropped at Education Masters' discretion. Our staff is educated meeting the Illinois requirements with a minimum of an Associate's Degree. The student and parent will be able to continually monitor their progress by simply accessing the progress report on-line at a time and frequency of their choosing. The curriculum will be

accessed by the students on a personal computer and air card to connect to the internet all of which is provided to them at no cost. Students actually receive tutoring online with a ratio of students to instructors at one to one. An adult will respond to questions. The tutoring may occur at home. Since the student may log in at home, transportation may be minimized and help the working parent. For our Spanish-speaking students we will be able to translate text aurally, have graphics, students may control the pace of the tutoring, and Spanish support may also be received by phone.

Educational Consultants, Inc Educational Consultants, Inc. (ECI) begins its work by administering a diagnostic test. Each student has adopted small group instruction: pre-assessment, development of ILP, instruct and post assessment. We provide high quality before school, after school ,high school lunch hour, weekend and summer tutorial services to kindergarten through high school level students in the areas of Math, Reading or Science. One-hour to two-hour tutorial classes are conducted a minimum of one day a week for students (for a minimum of 30 hours per subject). Weekend sessions consists of one two to four hours on Saturdays for a total of 30 to 40 weeks at both the elementary and high school level. Summer sessions are conducted on Tuesdays, Wednesdays and Thursdays for two to fours hours daily (for a total of 40 to 81 hours). Each class consists of a maximum of 1:10 teacher/student ratio. Since 2008, ECI has served over 250 K-12 students in Illinois, Indiana and Wisconsin in reading, math and science and have shown a minimum 86% gain on their ISAT or ACT.

ECI will establish tutoring classes on the students’ home school campus thus, minimizing the need for transportation. We also may offer tutoring at Libraries, Community Centers and Places of Religious Worship and our business location. ECI recruits and trains qualified teaching staff who have a minimum of 60 college semester hours or 90 quarter hours or an Associate's degree. In addition, ECI hires and trains current and retired teachers.

ECI will provide all classroom learning materials and textbooks for student use. Parents are required to supply all notebook paper, spiral notebooks, notebooks, pens, pencils, crayons, scissors, glue, and any other miscellaneous school supply the student may need. All high school students enrolled in the math tutorial must have a scientific calculator (TI84 or better). All students are required to have a library card. ECI offers services to LEP students and students with IEP's; but does not offer support for students with severe learning disabilities.

EduPlus LLC EduPlus LLC has developed a K-12 reading and math academic improvement program that teaches students through animations and interactive lessons on the computer combined with frequent remedial interactions with our teachers. Tutoring starts once the students have taken the diagnostic test. We use the results of the test to create a learning plan which is the base for the tutoring of that child. Students do not incur any costs associated with the classes. We provide a laptop and Internet, free of cost, for students who do not have them. Each student will receive a minimum of 30 hours in either reading or math, usually conducted at the student’s home but available anywhere the student can login. This facilitates transportation by giving parents the broadest choices for location to fit their schedule. Sessions are of 90 minutes each and held two-

three times a week. Students can choose any day and time, all year round, between Mon-Sat, 6am to midnight. For English Language Learners, our software/learning materials are available in Spanish and we have bi-lingual tutors. For students with disabilities, we make accommodations in the length of tutoring sessions as well as frequency, and the instructional material will be presented both verbally and in a written format for their comprehension. Our tutors are all college graduates and 70% have a minimum of 1-year experience in SES. 523 K-12 students completed our ELA program and all of them showed significant improvement. 1003 K-12 students completed our Math program and 962 (95.9%) showed significant improvement in their post assessment scores. These students were from nationwide and served between 2010-2012.

EEE The goal of Empowerment Through Education and Exposure is to ensure that students increase theirproficiency in meeting the state's academic achievement standards in reading and/or mathematics forgrades K-12. From school years 2008-2009 through 2011-2012, we have served 1,274 students in grades 1-12 in reading and/or mathematics, with an average reading improvement of over 23 percent and with an average math improvement of over 23 percent. The students are given an assessment to identify their strengths and weaknesses at the beginning of program. From this data, an individualized learning plan is created, and instruction is based on each student's individualized plan. Our program provides a minimum of 30 hours per subject in reading and/or mathematics for grades 1-12. The typical tutoring schedule is two 2-hour sessions per week or four 1-hour sessions per week at public schools, community centers, non-public schools, public libraries, community colleges, our place of business, and places of religious worship. We provide individual and small-group sessions with a maximum of 7:1 student-to-tutor ratio. Also, we offer LEP services for Spanish-speaking student; Spanish-language materials and Spanish-speaking tutors. We are able to tutor before and after school, on weekends, and during the summer. All tutors are required to have at least an associate's degree with experience in tutoring; some tutors are also certified teachers. Our program will provide all instructional materials and equipment. When our services are offered at the public schools, parents are only responsible for transporting children home at the end of each session. If community centers, public library, community colleges and/or places of religious worship are selected, we will provide services close to each student's home to reduce travel time.  Empowerment LearningEmpowerment Learning has been providing SES since 2006. We provide high quality instruction generally in either reading and/or math to students in Kindergarden through 12 grade. Between 2009 and 2011, we served nationwide 268 students in reading, K-5, who had an average gain of 23.47 points between the pretest and posttest, which was significant. Similarly, we served 142 math students, K-6, with an average gain of 25.10 points, which was also significant.

All instruction takes place in person; students 6th grade and up use a computer instead of a paper workbook. We tutor in small groups and one-on-one (parents choose). We administer a pre-test, and the results of the pre-test lead to an Individual Learning Plan.

The learning plan is the basis of the tutoring. We offer a minimum 30 hour per subject in reading and/or math. Typically, there is 1 instructor for no more than 8 students. We provide all learning materials and equipment necessary to participate in our program; students and their families never have to supply any materials or equipment. Material is also provided in Spanish along with Bilingual tutors. Students with disabilities will be provided with extended time to complete assigments, shorter lessons, modified lessons, non-written materials and responses, specialized equipment, and alternative physical settings. We only employ trained educators who are committed to providing individualized instruction to meet students’ educational needs. We employ tutors who hold, at a minimum, a Bachelor’s degree.

We ensure our services are available to students with working parents by offering year-round instruction before school, after school, on Saturdays, and during the summer. Our students typically receive tutoring twice a week in 2-hour sessions. Empowerment Learning seeks to minimize the need for transportation by providing services at students’ schools. When no space is provided at students’ schools, we provide tutoring at nearby non-public schools, places of religious worship, community centers, and libraries. We are sensitive to students’ transportation challenges when we are selecting a tutoring site; we attempt to find places that are within walking distance from students’ schools and/or on mass transit routes. For students who are transported to school by school bus, we may be able to provide transportation through a contract with a school bus company.

Gateway Center for Education, Inc. Since 1990 Gateway has served over 3,000 K-12 students, including SES, in Reading and Math with an average increase of one grade-level equivalent in 15 hours per subject. Since 2004, we have served approximately 1150 SES students in reading and math with an average grade-level improvement in reading of 2.6 and in math of 1.5. The program is set up for each student to attend a minimum of 15 per subject in reading or Math. Typically our program involves one to four sessions per week for 1 to 3 hours before and after school, on weekends, and during the summer. Gateway offers individualized tutoring programs for students in grades K-12 in Reading/Language Arts or Math. We offer a small student:tutor ratio of 5:1, which allows us to give direct instruction to each student, reinforcing classroom lessons and filling skill gaps. Our program begins with students taking a standardized test that determines students skill gaps. Using input from parents/guardians and schools/LEAs, Gateway designs an Individual Learning Plan (ILP) outlining program goals directly correlated to the Common Core Standards. We prefer to tutor at the students’ schools, which helps with transportation issues. If a school site is not available, we may use a Public Library, Community Center or a Church. Some students may be able to attend our Tutoring Facility in Steger, Il. When services our offered at students' school only transportation home is required of parents; if services are offered at other sites, parents are responsible for taking students to and from the location. We provide all materials and supplies, including books, paper, pencils, etc. We utilize consumable materials. Gateway believes in "going green," we recycle materials whenever possible. All of our tutors have a High School diploma and an Associate's Degree or its equivalent.

Our services to Limited English Proficient (LEP) students, at the parents' choice, can either be totally in the native language, totally in English or begin with the native language with an emphasis towards English. Our services to Spanish-speaking students

include a bilingual teacher who translates our English materials into Spanish as well as some supplemental materials in Spanish. Gateway attempts to provide similar services for LEP students in German, French, Italian, and Farsi when teachers and materials are available. Gateway accommodates students with special needs on an individual basis. Extended time to complete assignments and/or tests, breaks during instruction and/or testing are allowed. Special seating and adjusted student grouping is allowed. Gateway may also provide written copies of orally presented materials, verbal, and/or written directions. Gateway will make every effort to provide a tutor who is proficient in sign language when required. Gateway allows students to respond and to receive information in a non-written form. Gateway does not provide large print or Braille materials, calculator, word processor, or augmented communication devices, but will integrate these devices for students who bring them to the tutoring sessions if the tutor is proficient in assisting the student while using these devices.

Grade Plus Tutors Our tutoring program has individualized 1:1 instruction, systematized and sequenced lesson plans, and problem based learning for students in Grade K to 12 in Reading, Math or Science. We conduct an assessment test at the beginning of the program, which helps us identify the student’s area of strength and weakness, on the basis of which we prepare the Learning Plan, in association with teachers and parents. The length of each tutoring session is 1.5 hours and the weekly frequency is 3-5 sessions. We provide a minimum of 30 hours of tutoring per subject to each student, in any one of the subjects i.e. Reading, Math or Science. We offer services before & after school, weekends & during the summer. All the tutoring sessions are 1-on-1 with a live online tutor. The instructional material is developed by our Curriculum team & include electronic teaching materials. Our program has the advantages of a self-paced program supported by full interaction and focused attention of experienced teachers. Our lessons are conducted at the student’s home and are very flexible in order to fit into the schedule of working parents. For those students, who do not have a computer & Internet connection, we provide the laptop / tablet computer free of cost. All our teachers have a post-graduate degree in their subjects and teaching experience.

We are able to serve low performing students, students with disabilities, English language learners "Spanish". For ELL students, we choose teachers who are fluent in both Spanish and English. For students with disabilities, we offer extra time to complete the lesson plan. The whole lesson plan is divided into smaller units. Strategies such as creating multi-sensory contexts, drawing visual connections, incorporating communicative approaches, modifying teacher speech, establishing predictable routines and lesson patterns, and creating comprehension pauses. We are able to serve children who suffer from mobility impairments, are hard of hearing, or have a chronic illness.

Our Program has tutored more than 950 students since 2009. Out of these, 374 were tutored in English who scored an average of 37% in Pre- Assessment test, and 61% in the Exit test; 291 students were tutored in Math who scored 45% average in the Pre Assessment Test and 73% in the Exit Test; and 285 students were tutored in Science who scored 41% and 77% in Pre-Assessment and Exit tests respectively.

At GradeCracker, we firmly believe that getting quality education should not be limited by the factors such as place and time. Hence, our online tutoring facility delivers the instructions to you; where you want, when you want. Our program has serviced more than 1900 students throughout the US, between 2009-2012 for Grade K-12. Out of these, 1750 were tutored under Supplemental Educational Services program. The average increase in score in post test result as compared to pre test was 38% for Math & 37% for Reading, where a gain of 20% is considered significant.

We use a test to assess the student’s strengths and weaknesses and develop student learning plans. We consult on the results of this assessment test and proposed student learning plans with the parents/guardians, students, and schoolteachers. The key features of our instructional program include one-to-one live tutoring, flexible time-tables, experienced tutors, and state-aligned curriculum. If the student does not already have a computer/laptop/tablet computer and/or Internet, we provide both free of cost. Being an online provider, students attend the tutoring sessions from the comfort of their home and avoid transportation hassles, we fit our tutoring to parents’ schedules. Each student will be provided a minimum of 30 hours of tutoring in Reading, Math or Science. We cater to students in grade K to 12. A standard tutoring session will be for 90 minutes, 2 times to 3 times per week, before & after school, during summer & weekends.

Our tutors are experienced and all of them have a college degree, and most of them have more than 3 years of teaching experience. Students are not required to buy any additional learning material. The instructional materials are provided free of cost and are aligned with Common Core Standards. To teach ELL students we use a dual language program approach. In this a well researched and established method; we can teach in Spanish, with specially trained bilingual teachers & Spanish language materials. The program has been specifically designed to suit the needs of students with disabilities. We divide the lesson plans into easily understandable parts; we deliver tutoring sessions through Internet chat; we provide all the tutoring sessions in a recorded format and we make the student either write or draw the responses on the electronic whiteboard (as preferred by the student).

Growing Scholars Educational Center, LLC Growing Scholars Educational Center provides tutoring in math and/or reading to students in grades K-12 for a minimum of thirty hours in each subject. An Individual Learning Plan is developed for each student based on diagnostic pre-testing in math and/or reading. The ILP states the specific instruction goals that we will follow in the tutoring. Data taken from our 2010-2012 school years showed that of approximately 1300 k-12 students enrolled. Growing Scholars Educational Center, the average student increased in both reading and math from the pre-test to the post by 43 percentile points, with 15% of students increasing 50 points or more. An increase of 10 points or greater is considered significant.

We will provide services to accommodate parents at public libraries, local churches, community centers, non-public schools, community colleges or our business location. Under limited circumstances, we may be able to offer bus services. We typically offer our first choice of service before school (3 days a week for one hour) or after school (2 days per week for 1½ hours to two hours, summer and weekends. Our program is also available through a temporary computer lab that we will create at the tutoring site.

We offer individual and small group instruction, with a student-to-teacher ratio no greater than six-to-one. Students are not required to bring any materials. We will offer services to LEP students by offering Spanish-language materials and bilingual tutors. We will serve students with disabilities by providing alternative settings, teaching in smaller units and times, alternative technology, alternative physical setting, and/or oral rather than written material and responses. We require our tutors to have a minimum of 60 semester hours or 90 quarter hours, however, we have a preference for certified teachers or other individuals with at least a Bachelor's Degree.

Huntington Learning Centers, IncEstablished in 1977, Huntington Learning Center is a leading provider of supplemental educational services (SES) with over 250 accredited learning centers across the country. Huntington offers a minimum of 30 hours per subject of high-quality tutoring in reading or math for eligible students. Our instruction is typically 1.5-2 hours per session, 2-3 times per week, before and after school, weekends, and summer. Instruction will be provided at a student's school, a community center, library, place of religious worship, or at one of our local Huntington centers. Each student is assessed at the beginning of the program, and an individualized tutoring plan (ITP) is developed based on the student’s needs. The ITP is the basis of the tutoring program. All of our tutors have a minimum of a bachelor's degree, most of our teachers are state certified, and some may be Huntington certified. Each certified teacher provides instruction to no more than six (6) students, but it is common for our teachers to work with four to five students. While our materials are in English, we have bilingual tutors who are able to help students that speak Spanish, Portuguese, Cantonese, Mandarin, Lao, and Arabic. In addition, for students with disabilities, we offer extended time to complete assignments and/or tests. Annually, we provide SES to over 2,000 students across the state of Illinois, and students participating in our SES program experience average academic growth of 1.2 years in grade equivalency. Huntington provides all of the necessary materials for the program.

Imagine Learning Academy During each session of Imagine Learning Academy, students use a computer program called Imagine Learning. Students use this program individually or, if necessary, in a computer lab (with no more than 8 students) created by Imagine Learning. This software provides tutoring that automatically adapts to the child’s needs. Since 2008, we have served approximately 8,000 students in grades K through 8 in reading. Those students have had an average improvement of 17 percent. Based on research conducted by assessment specialists at Imagine Learning, a significant gain is 8-10 percent or greater.

Every Imagine Learning Academy tutor is also a teacher at the child’s own school with a minimum of two years of teaching experience. Students will attend three one-hour sessions per week for about twelve weeks, for a minimum of thirty hours of reading instruction. Imagine Learning Academy is prepared to offer tutoring sessions throughout the year after school, before school, on weekends, or during the summer. Imagine Learning Academy not only helps English-speaking readers but also provides special support for English learners. Native-language support is available in Spanish, Portuguese, French, Haitian Creole, Mandarin, Cantonese, Japanese, Korean, Vietnamese, Hmong, Arabic, Marshallese, Tagalog, and Russian. These students will receive a curriculum customized to work with their native language, including audio translations. Parents of

English language learners will receive progress reports in their first language, when available. Imagine Learning Academy is also ideal for many students with disabilities. This includes extended learning time, possible support of an in-class support person, and verbal and non-verbal presentation of material.

iPad Tutoring Our program design includes carefully constructed and sequenced lesson plans online, individualized direct one-to-one instruction, and problem based learning for students in Grades K-12. Our lessons are conducted online at the student’s home under the supervision of parents/guardians. We provide 24X7 technical support for any technology related issue. For those students, who do not have a computer and Internet connection, we provide the iPad and Internet free of cost. Hence, no transportation is required, and we can serve all school districts in Illinois. Tutors are required to have a four-year college degree and minimum one-year experience dealing with Title 1, ELL, and/or students with special needs. We prefer to hire tutors with teaching certificates and teaching experience who are subject matter experts in the discipline they tutor.

We provide a minimum of 30 hours of instruction per subject in either reading, math or science. Our recommendation is that each student participates in 3-4 one-and-half-hour sessions per week. However, students can work on their lessons for as long as they want, at any time of the day or night. For English Language Learners, our materials are available in English and Spanish, and we have bilingual tutors. For students with disabilities we offer the ability to change frequency of the sessions as well as the length of session. Instructional materials are available in verbal and non verbal modes. Our program begins with a student diagnostic test. The test results are discussed with the classroom teachers and parents to formulate a learning plan. The student will complete lessons and activities per the learning plan. Students can replay instructions as many times as they want while attempting activities, quizzes etc. Media rich content and flash based activities help keep the learner engaged while grasping important concepts.

Our program has tutored more than 500 K-12 students nationwide. We analyzed a random sample of 90 middle grades and 45 high school students, with an average increase of 39% and 42% respectively, in Reading. We analyzed a random sample of 60 elementary and 100 high school students, with an average increase of 34% and 41% respectively, in Math. In each of these cases, a gain of 20% or more, is considered statistically significant.Jeremi Learning Illinois Inc.Kinetic Potential Scholars The Kinetic Potential (KP) Scholars program uses a multi-faceted approach that directly builds upon research on “scaffolding” the developmental process, in which students receive online instruction in a virtual classroom coupled with in-person instruction to acquire new skills. Since 2009 we have served approximately 2000 K-12 students in reading and math, and on average our students gained 0.7 grade level equivalency gain in math and 0.5 grade level equivalency gain in reading / language arts.KP Scholars provides students with a minimum of 30 hours per subject in either Math and/or Reading / Language Arts. From the outset, KP Scholars assesses student needs, skills, and specific learning gaps. Every student entering our program is provided with an individual Learning Plan based upon the assessment and the tutorial sessions will follow the learning plan.

The KP Scholars program is designed as a 10-20 week series of academic sessions with studentsattending 2-4 times per week in 1-2 hour sessions Monday through Saturday (Saturday sessions may extend up to a maximum of 4 hours) and all participants have access to the site’s multi-faceted eLearning platform 24 hours a day, 7 days a week via a secure username and password log-in. KP Scholars is offered before school, after school, on weekends, during summer and also over winter/spring breaks covering the entire calendar year (the team is generally off for the majority of June). The program is implemented at community centers, local libraries, non-public schools, place of religious worship, public school/district, community college or at a student's home (online). Academic sessions with a maximum of 10 students are led by a single live tutor via the internet. In this dynamic virtual environment, students can fully “drop into” to engage in a discovery-based learning environment where one learns not simply by following but by actually doing. Access to a computer or laptop and the internet are required (tablet technology not sufficient for tutoring). Our students can access tutoring from any site that has the required technology. On a case-by-case basis, KP Scholars may provide students with loaner equipment. While our materials are in English, many of our tutors are bilingual in Spanish and Korean and are able to speak to students in their native languages. Additionally, the delivery of content can be slowed to accommodate the language proficiency of our students. For our special education students, KP Scholars will make reasonable accommodations: increased time to complete assignment and/or assignment broken into smaller units; adjusted student grouping and possible adjustment to equipment / technology and; students may utilize written or oral communication.

The program recruits its tutors from graduate programs in education from across the country, including Harvard, University of Pennsylvania, and Columbia. At a minimum, each of our tutors will have a bachelor's degree and 1 year of successful teaching experience. In general, KP Scholars does not provide transportation; however, in rare circumstances KP Scholars provides transportation to some sites through a third party. Arrangements are made with the parents of those students that require transportation.

Laureate Learning CenterLaureate Learning Center, Inc.’s (LLCI) research-based program has a proven record of raising theachievement of students in grades K-12 in Language Arts, Math and Science. Since 2011, we have served approximately 152 K-12 students in reading, math and science with an average change from pre-test to post-test of 20 percentage points, where a change of 18 percentage points or greater is significant. Students begin by taking a diagnostic test, and the results of that test are used to create an individual learning plan, which then guides the instruction.

Each student will receive a minimum of 30 hours per subject in either math, language arts or science.The maximum student-teacher ratio is 10:1. We offer our services year around Monday-Friday, 4pm-8pm (small group in-person or online), and on Saturday's 10:00am-2:00pm (small group online or one-to-one online). Services are available before and after school Monday

through Saturday; and during the summer. Online students will receive a tablet to use for their tutoring: LLCI may provide internet services as needed. Laureate does not provide transportation for students, but hopes that the large number of options for service delivery will help parents with their transportation needs.

LLCI requires our tutor's to have an Associate's degree or its equivalent; however, we have a preference for certified teachers.LLCI has a Spanish speaker to translator parent and student materials and will hire additional translators for languages as needed. Spanish-speaking students will work with bilingual teachers and materials in Spanish. Laureate Learning Center has teachers on staff specifically certified and trained to work with students with special needs and to ensure effective development of learning plans for SWDs. Online students will have access to tutors via telephone before and after sessions when question(s) arise. Modifications will be provided to IDEA students, and will include such things as abbreviated work, frequent breaks, and a certified special teacher. LLCI will take into consideration to information contained in the IEP's.Learn It Online, LLCLearn It Online Learn It Online provides a blend of computer-assisted and live instruction. Students in grades K-12 will have 2-4 computer-assisted lessons per week, as well as work individually with an instructor, but tutoring may also be delivered of no more than 10 students per instructor. Learn It Online will provide 30 or more hours of instruction in reading or math. The computer-assisted lessons can occur either in a computer lab at the student's school or from home and the live instruction sessions can occur either at the student's school, an alternate suitable location or from the student's home. Our program is proven effective; students that have completed our blended program demonstrate average gains between pre and post-test 54% in math and 27%, where an improvement of 1% is considered significant.

Learn It Online requires, at a minimum, an Associate’s Degree but strives to hire certified teachers as instructors. Learn It Online is able to accommodate Students with Disabilities (SWDs) and English Language Learners (ELLs). For SWDs, we can include the use of visual and graphic organizers, provide students with extended time and, through the use of the computer from home, reduce distractions. For ELLs, we strive to hire bilingual instructors to facilitate communication with the students and their parents/guardians and provides reporting to parents in Spanish or other languages, as necessary.

Learn It Online provides all the necessary equipment and instructional materials for our program, including a computing device and Internet connection. Families of students in our program are not required to provide any items. Our program is tailored to the needs of working parents. We do not provide transportation, but since our program generally occurs from the student’s home, transportation is usually not required.

Learn It Systems Learn It Systems provides instruction in reading or math to students in grades K-8 in small groups at the student's school or online (distance learning). Our programs are

offered in small groups that never exceeds 10 students per instructor. Since 2007, Learn It Systems has provided services to more than 100,000 K-12 students nationwide. Students who have completed our programs demonstrate an average increase of 53% in the math and 43% in the reading; SWDs averaged gains of 52% in reading and 61% in math; and ELLs averaged gains of 63% in reading and gains of 77% in math. Each student will be offered at least 30 hours of instruction in reading or math. Learn It Systems offers our programs before school, after school, on weekends, and over the summer. The majority of our programs take place after school.

For our in-person program, called Learn It Advantage, we provide our services at a child’s school; however, if school facilities are not available, we will offer services at alternative locations such as a local place of religious worship, non-public school site, or local library. Learn It Advantage looks for a location that is within walking distance of the school. To accommodate the schedules of working parents, Learn It Advantage tries to coordinate with the school’s busing system to ensure that students are still able to catch a later bus after tutoring. Learn It Advantage provides its program in English; however, Learn It provides accomodations for English language learners, including supplementing our program with our English Language Learner curriculum called Adventure ELL and can provide progress reporting in both English and Spanish. Learn It strives to hire instructors that are bilingual to communicate with students and parents. Learn It also provides accommodations to students with disabilities, including additional time and reduction of distractions. For the online program, each student is provided with a netbook or tablet computer and an Internet connection to access services online. Learn It Systems supplies all necessary materials for instruction, and students and parents are not required to provide any supplies for instruction. Learn It Systems strives to hire Illinois certified teachers for our programs. If certified teachers are not available, Learn It Systems requires that all tutors have at least an Associate's degree.

Learning Solutions Inc. d/b/a Sylvan Learning CenterSylvan tutoring offers in-person, small-group instruction to meet individual student needs. The Sylvan tutoring program assesses each student and uses the results to create a personalized learning plan for each student’s reading or math instruction. All Sylvan tutoring programs are tied to Common Core standards. We require all instructors to have a minimum of a Bachelor's degree. Sylvan reading teachers are required to have a state teaching certificate. Sylvan math teachers are required to have a minimum of a Bachelor's degree in math or have a Bachelor's degree in a math-related field. In addition to classes during the school year, we also can provide instruction during the summer. We do not provide transportation to or from classes, for either in-school or in-center instruction. Students typically attend two to three days per week for one to two hour sessions. Students will receive a minimum of 30 hours of instruction, and up to 40 hours of instruction, per subject in reading or math. Sylvan provides all of the materials needed for instruction. While our materials are in English, Sylvan can provide bilingual teachers to provide instruction in Spanish for students with limited English proficiency. We will also make accommodations for students with disabilities, such as providing extended time to complete assignments, shortening assignments, accommodating special seating needs, making equipment and technology accommodations, and by allowing non-written presentation of the lessons and responses from the students. Sylvan will communicate with parents by providing progress reports every step of the

way. Over the last two years, 83% of students completed their program and met the goals of their Individualized Learning Plan.

Our in-school instruction, known as the "Ace it!" program, will be conveniently located in the public or non-public schools. Instruction will take place immediately after school, or on Saturday mornings to minimize the need for transportation. We also may provide instruction at libraries or places of worship. We begin the program with an assessment to determine the student’s specific learning needs. Students are then placed in small groups based on their learning needs and ability levels. Instruction is given in no greater than 8:1 student-teacher ratios. Since 2010, we have served 318 K-12 students in our reading program, with an average growth of 0.5 grade equivalence.

We also offer instruction at our learning centers in the evenings and on Saturdays to allow for more flexibility in scheduling. Our in-center instruction uses SylvanSync™,in which Sylvan certified instructors teach each Sylvan lesson on an iPad. Instruction is given in a 3:1 student teacher ratio. Since 2010, we have served 261 K-12 reading students, with an average growth of 1.0 grade equivalence, and 271 K-12 math students with an average growth of 1.3 grade equivalence.

Light House Youth CenterFounded in 2006, the Light House Youth Center (LHYC) has provided tutoring and mentoring services to over 1020 youth, serving grades K through 12th in Reading and Math. Data indicates that of approximately 300 K-8 students who have received tutoring at LHYC since 2006, they made an average 10% gain after 40 hours per subject, where a 7% gain is statistically significant.

LHYC offers academic services, after school, weekends, and summers. A standardized assessment test will be administered to all students prior to beginning tutoring. The results of the pre-assessment test are utilized in the formation of an Individualized Learning Plan, and instruction will be based on the results of the pretest. Tutoring is provided in small groups with a maximum ratio of ten students to one tutor. LHYC provides a minimum of 40 hours of tutoring in Reading and/or Math. Students will have the option of attending tutoring three times per week or 1.5 hours or twice a week for 2 hours after school.

The minimum education requirement for tutors is 60 semester hours or 90 quarter hours of college credit. The LHYC will provide all instructional materials necessary for the program. While our materials are in English, bilingual tutors will provide service to Spanish-speaking students. For our students with disabilities, we offer extra time to do work, a division of the lessons into smaller units, adjustments to the physical environment, and the option for non-written materials and student responses. Tutoring will occur at the students’ home schools and/or our youth center site. When this is not possible, an acceptable tutoring site that is convenient for the student and their family will be chosen from a public library, community center or church. Transportation if needed, will be the responsibility of the parents.

EdisonLearning provides an on-site math and/or reading program filled with high-interest, hands-on Learning Force lessons that "Gets Kids Thinking"! Students in grades K-8 receive a minimum of 30 hours of instruction per subject from highly qualified certified teachers. Since 2006 we have served over 21,000 students nationwide in our K-8 SES program and have seen average gains of 14% in reading and 18% in math from our pre- to post-testing scores. A difference of greater than 5 percentage points is statistically significant. Our small group setting of no more than 10 students per class provides teachers with the opportunity to meet the learning needs of all students including English Language Learners and students with special needs. For English Language Learners in grades K-2 we provide both Spanish materials and a teacher who can provide support in Spanish. For grades 3-8 each lesson are embedded in each English Language lesson for English Language immersion support and teachers may be either Spanish or English speaking depending on the composition of each class. For students with disabilities we modify curriculum by extending time to complete work, dividing assignments into smaller tasks, and adjusting student grouping as necessary to meet the needs to all students. Students are given the Learning Force pre-and post-assessments that allow teachers to meet the learning needs of each child in their classroom. All materials are provided to students and teachers including a variety of hands-on learning manipulatives, a classroom library, individual workbooks, and all materials teachers need to successfully support each student in their classroom. All programs are held on-site at your child's school so the transition from the regular school day to their afterschool classroom does not require extra support from working parents. The EdisonLearning program, can be offered before and after school, on Saturdays, during both the regular school year and during the summer. The number of hours per week varies based on input from the school community.Orion's Mind Orian’s Mind Orion’s Mind believes that students learn more when they are having fun! Our research-based curriculum teaches math and/or reading in a small group format using lessons, educational games, problem solving activities, and creative exercises. The lessons are team-based, hands-on, active, and utilize creative thinking skills. At the beginning of the program, students are pre-tested. The results of the pre-assessment are utilized in the formation of an Individual Learning Plan (ILP). The program sessions follow the ILP. Orion’s Mind Program has the following features:• Serving grades K-8 with no more than 12 students per instructor• Most programs are held at the child’s school to minimize the need for transportation; Orion's Mind also has off-site locations. Under certain circumstances, Orion's Mind may be able to provide transportation• Typically 3-4 hrs a week (schedule varies by site; most programs take place after-school for 1-2 hr sessions) for a minimum of 25 hours per subject in reading and/or math (due to funding issues, parents may have to select one subject)• All materials are provided by Orion’s Mind; the students are given books and other materials to take home to continue practicing their reading skills• Orion’s Mind instructors have a minimum of a high school diploma and the equivalent of 60 semester hours of college credit and complete the Orion’s Mind training program; over 80% of our instructors have teaching certificates; 50% of our instructors have masters degrees• Orion’s Mind provides English immersion services to limited English proficient students.

Prime Time Computer Services, Inc.Our “Passport to Learning” Tutoring Program reflects the company’s commitment to promote differentiated instruction, critical thinking skills and 21st Century computer literacy skills through enriching opportunities that compliment and expand the school day. Students begin the program by taking a diagnostic and prescriptive assessment via a computer housed in a public school computer lab. The assessment will identify each student's skill gaps in mathematics or reading and provide each student with an individualized learning plan. Students will use the latest technology, interactive lessons, real world applications and individual tutoring in Reading OR Mathematics (Grades 1-12) from on-site certified teachers in a self-paced learning environment with 1:1 computer ratio. (Students are taught in a computer classroom with adult monitors at an 8:1 ratio. The actual instruction comes from the computer at a 1:1 ratio.) Due to the use of computer based learning programs, students are assessed on a continuous basis. Students will receive a minimum of either 30 hours of computer based mathematics instructions OR 30 hours of computer based reading instruction. The subject of instruction is selected by the parent. Our general program structure includes a 1.5 hour session, 2 times per week. Our potential delivery times are before school, after school, weekends and summers. Prime Time Computer Services will provide all materials necessary for the program. Prime Time Computer Services will offer computer-based learning options, using the Spanish version of the A+nywhere Learning System, to Spanish-speaking LEP students. Bilingual mentors will provide services to accomodate these students. For our students with disabilities, we offer extra time to do work, a division of the lessons into smaller units, adjusments to the physical environment, adaptive technology, and the option for non-written materials and student responses. Because our services our provided in public school classrooms, the only parent transportation required is transporting students from the program at the programs dismissal. PTCSI has assembled data showing the effectiveness of our program. For example, we assisted Dolton School District 148 (Dolton, Illinois) delivering Supplemental Educational Services since 2006. In that period, 84% of Roosevelt Elementary SES students (K-6), who completed the after-school program, improved their reading achievement an average of 1.6 grade equivalent years as reported by Scantron’s EdVision Performance Assessment.rogressive Learning Progressive Learning Progressive Learning is an online/on-site blended learning program with an integrated mentoring and motivational component. Our program provides 1-to-1 online academic coaching in addition to on-site support. Students are assigned an online Academic Coach, an experienced, credentialed teacher with a record of improving the academic

performance of youth. Students also receive instruction from an On-Site Instructor, a credentialed teacher from the students’ school.

Our self-paced online instruction targets each student’s specific goals by assessing skill sets in reading and/or math and identifying areas of remediation. Results from the assessments generate an Individualized Learning Plan. A post test evaluation is performed to measure student gains. Illinois K-12 students working with Progressive Learning from 2007-2012 on average realized a 30% gain in reading and math from pretest to post test, which is statistically significant. Parents are encouraged to be involved throughout the course instruction. Instructors will be available to answer parents' questions, ensuring they play an important role in the student's performance.Rocket LearningRocket Learning Rocket Learning provides reading or math tutoring to students in one of three models of instruction: Rocket learning begins by testing students to see what their needs are, working with parents and districts to develop a learning plan, and tutoring the students based on the learning plans. MODEL #1: In-person tutoring using workbook-based curriculum for reading students in grades K-12 and for math students in grades 1-12, which is delivered in a group setting of 8-12 students per tutor. We may supplement instruction with tablets that are pre-loaded with our Lift-off curriculum.MODEL #2: In-person tutoring using online curriculum for reading students in grades K-8 and math students in grades K-12, which is delivered in a group setting of 8-12 students per tutor.MODEL #3: Distance tutoring using online curriculum for reading students in grades K-8 and math students in grades K-12, which is delivered in an individual setting (1 student per tutor) with the tutor interacting from a remote location with the student via audio and chat.

Since 2005, Rocket Learning has successfully tutored more than 195,000 K-12 students. During the 2011-12 school year, the average growth of our students was: 7.39% in reading and 8.52% in math. We shall provide each student enrolled in our program with a minimum of 30 hours of instruction in reading or math. We shall deliver services to students before school, after school, on weekends, or during the summer (January-December). Weekday tutoring sessions shall be 1-3 hours in length and occur up to 5 days per week. Weekend and

summer tutoring sessions shall be 3-6 hours in length and occur up to 6 days per week. We shall tutor students face-to-face at public school/district sites, community centers, libraries, places of religious worship, or business sites. We also tutor students online.

Our tutors have: 1) a college degree with a relevant concentration, 2) experience teaching academically challenged Title I students, 3) completed training on the practical implementation of our programs. Our lead tutors have 4+ years of teaching experience and have held at least one prior instructional leadership position. Lead tutors provide daily instructional monitoring and coaching that further enhance the effectiveness of our services.

For students participating in our workbook-based program, we provide the workbooks, manipulatives, and handheld devices preloaded with the workbook’s lessons. For students participating in our online program (both the in-person program and the distance program), we provide Internet-enabled laptops, an internet connection (as needed), and a headset with a microphone. ALL INSTRUCTIONAL MATERIALS ARE OFFERED NO COST TO PARENTS/GUARDIANS. Typically, we do not provide transportation for students. However, in certain rare circumstances, we may offer transportation.

We adjust the program’s pacing for LEP students and use multiple audio-visual media to enhance student understanding (including pictures, audio content, hands-on manipulatives, paper-and-pencil curriculum, and a digital tablet). We often pair LEP students with bilingual tutors. We typically place Student With Disabilities(SWDs) in small, skill-based groups with other SWDs of similar academic levels. Tutors provide SWDs with extended time-on-task.

SCHOLARS SCHOLARS FOR THE 21ST CENTURY(SCHOLARS), provides after-school tutoring in mathematics and/or reading. SCHOLARS will provide all materials free. Since 2010, SCHOLARS has served more than 200 students in reading and/or mathematics. The “Summary of Chicago Public Schools Supplemental Educational Services (SES) Providers Impacts” (covering 2008-2011) indicated that SCHOLARS has statistically significant positive impacts, as measured by improvement in test scores. SCHOLARS' diagnostic assessment tool is an online activity. The diagnostic results provide a detailed analysis of the student’s strengths and weaknesses. This analysis leads to an Individual Learning Plan. We focus our tutoring on the Learning Plan with face-to-face tutoring, both individually and in small groups. During each session a groups may have 8 students per tutor (8:1), but typical groups are between 3-5 students per tutor. Students will receive a minimum of 30 hours per subject. Tutoring is offered after-school, weekends and summers. A typical schedule is 2 or 3 days per week, and 1.5 to 2 hours per day. We utilize our students’ schools for tutoring sites to minimize transportation. However, other service locations are community centers, colleges, libraries, non-public schools or churches. Our state certified educators have various advance degrees in education, and some are retired educators.SES of IllinoisSES of Illinois The instructional method and curriculum employed by SES of Illinois are both nationally-recognized and research-based. After the student takes a diagnostic test an individual intervention plan will be designed, which will also reflect accommodations for students

with disabilities, students with 504 plans, and/or English language learners. We offer Spanish Curriculum for grades 1-5 in both Reading and Math. We hire Bilingual tutors for all grades. Moreover, certified special education teachers are assigned to this cohort of students to more effectively provide an intervention program that will address their specific and unique academic needs. We offer them extra time on assignments, we change the physical setting, we offer a smaller ratio, we provide them with assistive technology, and also allow them to work on non-written materials. We provide the students with all the educational material they need including pencils and pens. We will provide a program of a minimum of 22 hours per subject in reading and/or math for K through 12th graders.

Since 2007, SES of Illinois, Inc. has served more than 13,000 students in Illinois with a gain of greater than 10% in reading and math for all grades K-12 where a gain of 5% or more is considered signifcant.

SES is provided at the school-site and at non-public schools using a small group model with a maximum teacher student ratio of 1:10. Typically, we offer four sessions, 1 hour daily; however, circumstances in a given district or at a given location may require a different schedule. We are able to provide our services before school, after school, on the weekends, and in the summer. SES of Illinois tutors are state-certified teachers and/or certified paraprofessionals who meet Illinois’ qualifications for providing SES. While we do not provide transportation services we provide our services in neighborhood schools in an effort to minimize inconvenience to parents and maximize participation by all. SLC Reading & Math SLC Reading & Math SLC Program consists of research-based curricula to improve reading and math primarily for students who do not speak English as their native tongue. Since 2008, SLC has served approximately 900 students in grades K-8 in reading and math, with an average improvement of 175 points between the pre-test and the post-test, where a change of 100 or more points is considered significant.

Students undergo a diagnostic assessment to develop an individualized learning plan stating their needs and learning objectives. SLC's reading program helps these students make the transition into English in an efficient manner while helping students generally improve their reading skills. Students receive computer-based instruction as well as assistance from a tutor. For example, students participate in oral reading exercises as well as expressive and artistic activities, such as readers' theater to demonstrate their understanding of concepts introduced during the lesson. SLC's math program similarly uses a web-based solution program and a tutor in the classroom.

Students receive a minimum of 30 hours instruction per subject in reading and/or math in small group setting of up to 10 students per tutor. Students are not required to bring any materials. Program services are available Monday through Friday before and after school 1 to 2 hours per day for a maximum of 6 hours per week, as well as on Saturdays – onsite at the students’ school. We also offer our services according to the same general schedule at offsite locations (community centers, our own business location, libraries, and non-public schools) and during the summer onsite and offsite. Since most

of our services are offered at the students' school, transportation will not be offered.

Key SLC staff includes experienced education professionals in the areas of Reading and Mathematics, ESL, and Special Education. Most educational staff members possess, at a minimum, a bachelor’s degree in education and over 2 years of experience in serving low-income, struggling readers–particularly those categorized as English Language Learners. Most staff members are bilingual–speaking English and Spanish.SPC Educational Solutions SPC Educational SolutionsSPC EDUCATIONAL SOLUTIONS is offering a tutoring program founded on individualized instruction.Since 2010, we have served approximately 650 students in K-12 in reading and math with an averageimprovement of one grade-level equivalent. Every student will be given an individualized tutoring plan (ITP), based on an assessment of their performance in reading or math. Instruction will be managed by using an online tutoring curriculum. The students will complete their lessons on a tablet pc, provided at no additional cost to the student or parent, during the tutoring sessions with the help of the tutor. Students will move through the program at their own pace, mastering a skill before moving to the next one.

A typical schedule may consist of 3 or more hours of after-school tutoring per week in the child's school, resulting in a minimum of 30 hours of reading or 30 hours of math. There will be one tutor for up to 12 students. At least one tutor in each school will be a certified teacher or formerly certified teacher who will oversee the work of other tutors in the school. Services will be offered 12 months a year. The summer program will be tailored to meet the specific needs of children, parents and school districts. Tutors will have a minimum of a bachelor's degree and will receive special training in the content and in individualized instruction. Services will be available in Spanish for LEP students, including all lessons translated into Spanish and tutors who are Spanish speakers. Students with disabilities may receive extended time to complete their work, smaller units of work, or audio/hearing devices, written and non written presentation of material and student responses to the material. All instruction materials will be provided at no cost to the student or parent. Since students will be tutored in their home schools, there will be no need to transport them to a tutoring site.

Sylvan Learning CentersWe offer two types of programs: The Sylvan program, which takes place in our centers, and the Ace It program which takes place in our schools. In every district the parent will have the right to select one or the other of these programs. In both programs we begin with the initial assessment that identifies a child's needs and strengths. We then develop a personalized program designed to help the child achieve their goals. Each student is placed in small group settings according to academic needs. Since 2008 we have served approximately 300 K-12 students per year in both reading and math. These students have had an average increase from their pre-test to post-test scores of 21.5 points, which increase is considered significant.

The minimum number of hours per student is 30 per subject in reading or math. In the Sylvan program, the student/teacher ratio is 3:1. In the Ace It program the

student/teacher ratio is 6:1. In the Sylvan program tutoring occurs after school, as well as weekends and during the summer. We have centers located in Champaign, Decatur, Mattoon, Springfield and Carterville. Transportation would need to be supplied by the parent both to and from our center. Our Ace It program occurs after school and in the summer in two hour blocks. Students would typically attend five blocks per week. The Ace it program is offered at public schools and community centers. Parents would only be responsible for transporting their students home after the program ends.

In both programs, all teachers are state certified. All instructional materials, equipment, and manipulatives are supplied for the students. Students are not required to bring anything to either program, everything needed will be supplied. At this time we are not able to provide LEP services to students in either of our programs. Accommodations and modifications will be provided for students with disabilities who need them in both programs. These include such items as extended time for assignments and tests, dividing assignments in half or in smaller sections, written copies of orally given assignments, special seating, shortened assignments, simplified curriculum, modified grading, prioritizing key concepts, and alternative assignments will be given when needed. Sylvan Learning/KCISylvan Learning offers three opportunities for students to participate in our programs. Students may attend the Sylvan Center, where they receive the Sylvan Center program, or they may attend their school or another location using the Ace it! Tutoring Program. Students also have the choice to participate via the internet with Sylvan Online.

There are some common elements in these three programs;• Individual or small-group instruction – maximum 8:1 student teacher ratio• Use of a diagnostic assessment to identify each student’s specific learning• Use of the Diagnostic assessment results create a learning plan • Minimum of 30 hours of instruction in math or reading• All textbook and classroom materials

While all of our materials are in English, Sylvan hires teachers who speak fluent Spanish to assist students' learning needs. For students with disabilities we offer shorter assignments, more time to finish assignments and changes to the physical setting in which the tutoring takes place (an online option for homebound). Each child's placement is reviewed by management to provide appropriate accomodations or modifications. KCI Enterprises may provide transportation based on number of students participating in each school; when such transportation is not available, parents will be responsible for transporting the students.

Sylvan Center Program (and Online)In-center instruction uses a digital technology/blended learning with iPad®. Instruction is given in small groups (1-4 students). Tutors teaching the Academic Reading programs are certified teachers, and tutors teaching the Sylvan Math must hold a Bachelor's degree and pass a rigorous area competency test administered by Sylvan. It is preferred that instructors have previous teaching experience, and/or specialized educational certifications and membership in professional educational organizations. Sylvan Center tutoring hours may vary from 1-3 hours. Tutoring hours for students attending a Sylvan

Learning Center are available year-round M-F, 9 am - 8 pm, Saturdays, 9 - 5 pm, Sundays, 1 - 5 pm.; in general parents can schedule tutoring time within those hours. Tutoring sessions for Sylvan Online are built on one-hour increments. Schedules vary based on each child's location of their computer and internet system. KCI Enterprises may be able to assist in providing computers and internet availability for delivery of program. Since 1999 Sylvan Learning - KCI has served over 10,000 students in grades K-12 in Reading and Math with an average improvement of 1.2 grade equivalence after 30 hours of instruction.

Ace it! Tutoring ProgramInstructors must possess a Bachelor’s degree. It is preferred that instructors have previous teaching experience, and/or specialized educational certifications and membership in professional educational organizations. Students in schools or other locations using Ace it! Tutoring typically receive tutoring three days a week in 1.5 hour sessions per day before or after school or on Saturdays, or during the summer. The Ace it! program establishes effectiveness goals that are different for each grade level making it hard to average all students together, but since 2006 as an approved SES provider in Missouri and Illinois we have served approximately over 10,000 students in grade K-12 in Reading and Math, and 82% of students have met or exceeded their grade level goal in their tutoring plan.

The Achievement Academy The Achievement Academy has provided Supplemental Educational Services to over 5,000 students in grades K-12 since 2006. Our tutors communicate regularly with parents/families and send monthly, written progress reports to them. For example, in the 2012 school year, of the 150 students in 8th grade who received Math and Reading tutoring from us, this group demonstrated an average 25.75 point gain in Math and 31.42 points in Reading, where a gain of 10 or more points. The 11th grade students showed an average 36.00 point gain in Math and a 38.20 point gain in Reading.

The Achievement Academy model provides for a pre-assessment which informs an Individual Learning Plan; the tutoring follows the plan. After 30 hours a Post Test Assessment is given to assess the students progress. The Achievement Academy will provide each of our students with at least 30 hours of tutoring per subject in Reading and/or Math. Our program is delivered as a web based online learning model and/or a traditional small group instruction model. Typically students are tutored with no more than 10 per instructor. The Achievement Academy offers tutoring after school, on Saturdays, and during the summer. For weekday sessions (Monday through Friday), students typically receive between two and three sessions per week, and each session lasts between one-and-a-half to two hours.The Achievement Academy provides all necessary equipment and materials for our tutoring sessions. Typically, we provide tutoring at locations that have computers with Internet access. For sites that do not provide computers with Internet connections, we provide laptops/tablets and a projector. We work closely with working parents to arrange tutoring sessions that work with their schedules. We offer tutoring at students' schools, homes, libraries, places of religious worship, or community centers. No transportation is provided by the Achievement Academy. The Achievement Academy's tutors are state certified teachers. We tutor students with limited English proficiency (specifically students who speak Spanish), and have Spanish-speaking staff available to speak with parents.

The Black Star ProjectThe Black Star Project Our program integrates traditional tutoring with family and community involvement to create a comprehensive plan for student success and support in all areas of student life. Between 2007 and 2012, The Black Star Scholars Tutoring Program has increased students’ Reading grade equivalency scores .55 points, based on serving 49 students in grades 3-12. Students’ math scores have increased by more than a grade point, based on serving 29 students in grades 1-8.

A standardized assessment will be administered to all students prior to beginning tutoring, and based off this assessment and a meeting with teachers and parents, a comprehensive and individualized tutoring plan (ITP) will be composed for each student. The individual student curricula will be tailored to meet and exceed the goals and objectives dictated by the ITP as well as any additional goals that individual teachers may request for student success. An assessment is re-administered at the conclusion of the program to determine progress.

We will tutor students for a minimum of 30 hours per subject throughout the school year in reading and/or math. Students will have the option of attending tutoring three times per week for 1.5 hours, or twice a week for 2 hours after school. We tutor students in a ratio of 1 instructor to every 6 students. The minimum educational requirement for all tutors is a high school diploma and an associate's degree or its equivalent. For Spanish-speaking students, we have Spanish-speaking teachers and will provide all of our materials in Spanish. We provide added time and adjusted physical space for students with disabilities. We offer books on tape to be read aloud to vision-impaired students. We provide non-written presentation of information and will accept non-written responses. Whenever possible, tutoring will occur at the students’ home schools. When this is not possible, an acceptable tutoring site that is convenient for the student and their family will be chosen, and could include our home tutoring offices, a public library, community center or church. Transportation, if needed, will be the responsibility of the student and parents.r Owl Tutor OwlTutor Owl tutor students in 2nd through 12th grade in Math with a minimum of 30 hours of instruction. Tutor Owl has been providing online tutoring in mathematics since 2007. During this time, we have provided services to over 11,000 students (K-12), most of whom were low-income students performing below grade level. We have worked with students of varying ethnicity, students with a disability, and English Language Learners. For these 11,000 students; these students had an average gain of 30 points from the pre and post test, where a gain of five or most points is considered significant.

Our curriculum is researched-based and aligned to the Common Core Standards. Our program starts with an online assessment to determine students' math skill gaps. From the pre-test, we work with parents and school to create an individual learning plan, which tutors use to provide targeted instruction. At program's end, students take a post-test that identifies the precise skills and subject areas of student growth attained during our program. Our lessons and tests are available in both English and Spanish. In addition we modify our instruction to accommodate students with special needs and/or disabilities; Shorter lesson plans, verbal and written communication, fewer problems,

visual aides, larger print, and additional time to complete the work series.

The student can log into the computer program from anywhere at the prearranged time (before or after school, weekends, or summer). Although, typically, tutoring takes place in students' homes. Thus, eliminates the need for parents to transport students to and from a tutoring location. In addition, parents are encouraged to observe the tutoring session, and a tutoring session. During tutoring, students interact with tutors on a 1:1 ratio via computer communications in “real time” – they can hear each other and see what each one is doing on the computer screen. Students communicate with tutors through a headphone and microphone and on the cyber whiteboard, provided without charge by our company. Tutor Owl will attempt to make arrangements with the student’s school or a local community site, such as a library, place of religious worship, and non-public schools to get the student access to a computer. Tutor Owl provides all materials needed for our services to include if needed, a computer and internet service. Tutor Owl hires only tutors who have a minimum of a college degree. Many of our tutors have a Master’s degree. Most of our tutors have experience teaching Title I, low achieving, English Language Learners, and/or disabled

Tutoria on IpadsWe provide individualized, one-on-one tutoring in a safe online environment, with tutors who all have atleast a Bachelor's degree. Students who have taken tutoring in Math have recorded a minimum increase of 40% from pre to post assessment. Students who have taken tutoring in English have recorded a minimum increase of 35% from pre to post assessment. Students who have taken tutoring in Science have recorded a minimum increase of 49% from pre to post assessment.

There is no transportation cost involved as the classes are conducted online at the student’s home. Before the start of the tutoring program, the students take an assessment test, which may be taken either offline or online. On the basis of the results of the test, we formulate an Individualized Learning Plan, which will be being followed by the tutor. Our tutoring program includes a minimum of 30 hours of tutoring in Reading, Math or Science. We cater to students in grade K to 12. A standard tutoring session will be for 90 minutes, 2 times to 3 times per week. The tutoring program is available between 8 am to 10 pm, all 7 days in a week, 365 days a year. The virtual classroom has a whiteboard where the tutor and students can share their problems and work on the lessons. The student and tutor communicate through chat. To enable a student to participate, we provide an Internet-enabled iPad to the students as needed. All the instructional material is provided. The student is not required to provide anything. Our iPads have a program available in Spanish, and our tutors are bilingual in Spanish. For students with disabilities having problem traveling can easily avail our services from their homes, under their parents care. For students with disabilities, we offer extra time to complete the lesson plan. The whole lesson plan can be divided into smaller units. The software allows for the information to be presented to the students orally. Tutorial ServicesTutorial Services Tutorial Services combines one to one on-line instruction with phone-based tutoring support, both of which are provided to students in their home. Tutorial Services has served over 5000 K-12 students in Reading and Math since we were founded in 2004. In school years 2009-2012, 3778 K-12 students in reading and math gained an average of

5% points and 7% points respectively where a 2% increase is considered significant. We provide each family with one computer if needed. Prior to beginning the initial tutoring session, students complete an on-line diagnostic assessment.Our certified teacher-tutors who combine the diagnostic results with standardized test score data, information provided by parents and classroom teachers, and other academic data provided by the school to create an Individualized Learning Plan.

The students' assigned tutors calls them bi-weekly to provide phone-based tutoring. During these sessions, tutors review the concepts and skill sets covered each week, provide instructional support for topics students may be struggling with, and adjust the student's learning path (including curriculum and activities) when needed. Students have access to the on-line program 24 hours a day, seven days a week. Students are scheduled to complete a minimum 30 hours per subject of reading or math. The computer-based instruction of our program is available in most languages; however, phone-based tutoring will be provided in English and Spanish. All forms sent home will be in English and Spanish. For our students with disabilities, we offer extended time, assignments in smaller units, options in presenting material. We also have options in student responses to the material.Y.E.S. Center Y.E.S. CenterThe model of instruction provided by the Y.E.S. Center (Y.E.S.)includes both individual and smallgroup tutorial instruction. We have served a total of 36 students in grades 2-8 who had an average increase of one grade-level in reading and in math. The participants will complete pre-test assessment instruments. Following the tutoring, students will take annual post-tests to determine their progress. The maximum ratio of instructors to students will be 1:9. Each participant will receive up to four hours of supplemental instruction per week in reading and/or math. The total minimum hours of service in reading and math will be 30 hours. If sufficient funding allows, parents may choose both subjects. If not, parents will decide if their child gets reading or math.

The schedule for the delivery of services during the fall and spring semesters is Monday – Friday, either before or after school. We can also offer tutoring during the summer. Typically, our schedule is Monday- Friday from 7am- 8am, 3:30p -5:30p during the academic year and from 9am-12pm and 1pm- 4pm during the summer. Parents may choose either the before school or the afternoon session, or both sessions for their child to receive tutoring. During the academic year, weekends are also available and during the summer. Services may either be provided at the Y.E.S. Center office located at 1201 S. Giant City Road in Carbondale, Illinois or onsite at each school district.

All instructional staff will have at least a high school diploma and have successfully completed at least 60 semester hours or 90 quarter hours of coursework at an accredited institution of higher education. Instructional materials include multimedia educational aids, supplies, textbooks and computers. All materials will be provided for the students. For services for students with disabilities, we offer adjustments to the physical environment of the classroom and extended time to get their work done. The Y.E.S. Center does not provide transportation. Transportation directly from the schools to

the program may be provided daily to the students by district buses. If no transportation is available by the school districts, parents will be responsible for providing transportation to and from the program.

Overall results indicate that since 2007, the Family Advocacy Services YES program for SES has served 36 students in reading and math with an average improvement of 18.5 points in reading and 15.67 points in math and an overall average gain of 17.085 points, between pre-tests and post-tests. A difference greater than 8 points is considered the threshold for being statistically significant.
