9.1.1 Trustee Liaison Report – Trustee Graf-Stone informed the Board that Staff has been very busy. Their biggest task was the budget.

Village Administrator Rudy Espiritu gave a presentation on the Budget Workshop for the FY2020-2021 Budget. He informed the Board that for the first time since he has been in Berkeley, all the funds in the budget have a positive fund balance. The General Fund was a challenging budget to do because of unknowns (the Tollway). The revenues from the Billboard and Cell Towers will be stopped due to having to bring them down during the tollway construction by
December 1st. 2020. Because of the unknowns a Status Quo Budget has been submitted (assuming everything for the most part will stay the same). Cost continue to go up, with revenues staying flat (new revenues are needed) Previous fiscal year, required departments to make cuts Current fiscal year was helped by the red-light camera revenues, otherwise would have had a deficit in preparation of the budget. New General Fund Budget is balanced, but required ($122,000) in cuts across all departments The Village cannot continue to make these cuts annually without staffing cuts that directly impacting the community Possible revenues options – Cell Phone carriers on the Water Tower, from the Tollway Project (the revenues the Village asked for), Red-light camera revenues, New billboard on the East side of the Tollway. Possibly moving the Utility tax revenue to the General Fund The Village expects the current fiscal year to be balanced. Village Administrator Espiritu informed the Board that the only reason why the Village will balance is because of each department being stringent on purchases. They were asked to hold off making certain purchases. The new Fiscal Year 20-21 No tax increases or fee charges (except for pension costs) No water or sewer rate increases Reductions to get balanced – includes reorganization in personnel ($122,405) Initiatives – 6 Board laptops (5 yrs. old) – Board members agreed they do not need their laptops replaced at this time. Events Coordination & Marketing Rezoning/Zoning Code Updates St. Charles Rd. Corridor Plan (TIF) Marketing of Village Properties (TIF) Façade Program (TIF) Capital Improvements – Proviso Drive Alley Improvements (carry over to next fiscal year) Electronic Message Board Server Replacement

the Music in the Park - some of the contracts are coming in. The Lions Club co-sponsors the Music in the Park, however Trustee Perez spoke with the Lions Club about sponsoring the Polar Plunge event. The Lions Club will get back to Trustee Perez on which event they will sponsor.

President Lee thanks all the Department Heads and Board members for meeting with him.
President Lee would like to look at Robo-Call system. Would like to look at what updates on communication can be done.
President Lee thanks and appreciates all the work done by Staff.
President Lee thanked the Board for volunteering to be Trustee Liaisons. It will be beneficial to all.
Community Round Table will be held on April 23rd.
11 Village Administrator – Robo-Communication may be discussed at their next meeting. On behalf of the Staff, Village Administrator Espiritu thanked the Board and they are looking forward for a good fiscal year.
12 Village Attorney – Attorney Smith reported on some bills that are pending in the General Assembly in Springfield. The first one is that each year every municipality in
Illinois has to publish in the newspaper what’s called a treasury annual report, which is a list of all of your revenues and expenses broken down to by source and then employee salaries and benefits and then vendor’s payment. The newspapers love this publication because they earn several hundreds of dollars from each municipalities to publish it. Now everyone has websites, no one really looks at the newspapers, this bill will allow you to put it on the website, instead of the newspaper. Another bill is to restore the 10% state income tax local government distributive fund share. It is now down to 6%. If this bill is passed, you would get 4% more net revenue from the state income tax which is split per capita. That will be a great benefit for all municipalities. The third bill is interesting. Because counties may allow adult use cannabis sales within unincorporated areas, the issues come up is what if you have pockets of unincorporated areas next to municipalities that have opted out of such sales. You can end up in a situation where the village/city/town says no, but across the street in the unincorporated (whichever county) one of the shops opens up. This bill enacted into law will create a 1 ½ mile radius from municipalities which opted out extending it to unincorporated areas prohibiting the counties from overriding your local zoning decisions. Another bill that has been introduced “Right to Garden Act” which would prohibit municipalities from precluding gardens from being present on residential properties. The last bill that is moving quickly through the house to ban red-light in non-home rule municipalities. This would not affect the Village of Berkeley because it is home rule.

RE: Increasing the Number of Video Gaming Terminals Permitted at Fireside Eats and
Elsie’s Place Cafe
Background
Effective January 1, 2020, the State of Illinois allowed an increase in the maximum number of video gaming terminals to six from five terminals per licensed establishment. The Village currently has six establishments that offer video gaming. We have received applications from both Fireside Eats (5725 St. Charles Road) and Elsie’s Place Café (5555 St. Charles Road) to increase their number of terminals from 5 to 6.
The Village Board may allow or not allow up to six video gaming terminals at video gaming locations, in a case by case determination at the request of each video gaming terminal license holder. For a location to increase the number of terminals, the Village Board needs to consider and act on requests on a location by location basis, per Sections 4-2-8(C)(2) and 4-2-13 of the Village Code.
It should be noted that both establishments were found to already be operating with a 6th video gaming terminal without Village approval. Village staff fined each establishment $500 and required them to unplug the machine until the Village approves the 6th video gaming terminal.
Previous Action
Fireside Eats received their video gaming license in 2015 and Elsie Place Café received their video gaming license in 2014.
Recommendation
It is the recommendation of Village staff that the Village Board of Trustees approve:

AN ORDINANCE OF THE VILLAGE OF BERKELEY, COOK COUNTY, ILLINOIS INCREASING THE NUMBER OF VIDEO GAMING TERMINALS PERMITTED AT BERKELEY’S BEST WINGS, INC. D/B/A FIRESIDE EATS – 5725 ST. CHARLES
ROAD
WHEREAS, the Village Berkeley (“Village”) is a home rule municipality pursuant to Article VII, Section 6(a) of the Constitution for the State of Illinois of 1970 with the power and authority to perform any function pertaining to its government and affairs; and
WHEREAS, pursuant to Section 4-1 of the Illinois Liquor Control, 235 ILCS 5/4-1,
the Village President and Board of Trustees have the authority to determine the number, kind and classification of liquor licenses and their fees; and
WHEREAS, pursuant to Sections 25(e) and 55 of the Illinois Video Gaming Act,
230 ILCS 40/25(e) and 40/55, the Village President and Board of Trustees have the authority to determine the number and placement of video gaming terminals in the Village; and
WHEREAS, Berkeley’s Best Wings, Inc. D/B/A Fireside Eats (“Licensee”), located
at 5725 St. Charles Road, Berkeley, Illinois (“Property”), holds a Class G liquor license issued by the Village, and the Licensee was issued a supplemental video gaming permit by the Village in 2015 pursuant to Section 4-2-13 of the Village of Berkeley Village Code, which allowed for five (5) video gaming terminals to be located on the Property; and
WHEREAS, the Licensee has requested a modification to the supplemental video
gaming permit issued by the Village to allow for one (1) additional video gaming terminal at the Property in addition to the five (5) video gaming terminals already present at the Property, to allow for up to six (6) video gaming terminals in total at the Property; and
WHEREAS, the Village President as the Local Liquor Control Commissioner has
approved the application of the Licensee to allow for one (1) additional video gaming terminal at the Property in addition to the five (5) video gaming terminals already present at the Property, to allow for up to six (6) video gaming terminals in total at the Property, subject to the approval of this Ordinance by the Village President and Board of Trustees;
NOW, THEREFORE, BE IT ORDAINED BY THE PRESIDENT AND BOARD OF
TRUSTEES OF THE VILLAGE OF BERKELEY, COOK COUNTY, ILLINOIS: SECTION 1: Incorporation. That the recitals above shall be and are incorporated in this Section 1 as if fully restated herein.
SECTION 2: Approval. That the supplemental video gaming permit issued by the Village to Licensee is hereby amended to allow for up to six (6) video gaming terminals in total at the Property.

SECTION 3: Severability. That if any Section, paragraph or provision of this Ordinance shall be held to be invalid or unenforceable for any reason, the invalidity or unenforceability of such Section, paragraph or provision shall not affect any of the remaining provisions of this Ordinance.
SECTION 4: Repeal. That all ordinances, resolutions, motions or parts thereof in conflict with this Ordinance shall be and the same are hereby repealed. SECTION 5: Effectiveness. That this Ordinance shall be in full force and effect upon its passage and approval according to law.

RE: Blackboard Utilization and Functionality
Background The Village currently utilizes Blackboard Connect to communicate with Village residents and businesses. Currently, the service is utilized to communicate important information during emergencies, inform residents about important upcoming meetings, or forthcoming Village events (Music in the Park). Data is imported and updated into the system through the Village’s Blackboard Connects portal by Blackboard. On a monthly basis, Blackboard updates residential and business listings for the Village. This service is included with the Village’s current Blackboard subscription and users are allowed to opt-out of receiving communication at any time. Generally, past practice, has been to use pre-recorded robocalls to communicate with residents about upcoming meetings or events. The Mayor asked staff to provide an overview of the Blackboard Connect System and explain current communication practices, utilizing the system, with residents and businesses. The Blackboard Connect system allows the Village to communicate with residents and businesses in a variety of ways. More specifically, through the following communication channels:
• Text Message • Phone Call • Email • Survey
For phone calls, messages can either be pre-recorded, like current practice, or the platform has the ability to convert text into speech. If the Village communicates with a text message (SMS), only those with a cellular phone receive the message; those with a landline do not. The platform also allows the Village to target specific sections of the community with communication, based upon their address on file. Currently, the Village is split into eight (8) sub-sections in Blackboard. Additionally, Blackboard Connect allows the Village to communicate in a wide array of different languages including Spanish, among others. Staff also wanted to highlight the reporting functionality of the Blackboard platform. For example, the robocall that was recorded for the Annual Tollway Townhall Meeting in January, reached 39% of the phone numbers in the system. It allows the Village to track how many of the

calls went to voicemail, received no answer, or were hang-ups, among other categories. Staff has included data on the three most recent robocalls as an attachment for reference. These metrics are also provided when other communication methods are utilized (text message, email, etc.) through the Blackboard platform. The reporting platform does not allow for reporting on the number of users who edited, deleted, or changed their information. Previous Action None. Recommendation Staff is seeking direction from the Village Board regarding future Blackboard communication policy. Budgeted Amount The Village annually spends approximately $5,500 on the Blackboard Connect System. Action Discussion on the future utilization of the Blackboard Connect System to communicate with residents and businesses. Attachments

RE: Invest in Cook and CDBG Grant Applications
Background The Invest in Cook and Community Development Block Grant (CDBG) Programs are now accepting applications for their 2020 programming year. Both programs are administered by Cook County. Staff has identified additional grant funding opportunities for recreational upgrades at the Village’s Flood Control Project. The Village would apply for $73,000 for land acquisition through the Invest in Cook program and $400,000 for infrastructure upgrades through the CDBG program. The grant requires the Village Board to submit a resolution supporting the submission of a grant application for both the Invest in Cook and CDBG programs. Previous Action The Village has previously submitted and been awarded Invest in Cook and CDBG grant funding. Recommendation Staff recommends the following:
1. Motion to Approve a Resolution Authorizing the Submission of an Application for a Community Development Block Grant Program Grant Application for Property Improvement Purposes.
2. Motion to Approve a Resolution Authorizing the Submission of an Application for an Invest in Cook Program Grant Application for Property Acquisition Purposes.
Budgeted Amount If awarded, the Invest in Cook and CDBG grants will both require a local match. Staff is currently working with Cook County to determine the local match for each grant. The cost of the local match will be incorporated into the final Fiscal Year 2020-2021 budget. Action

Background
The Village of Berkeley owns certain equipment and vehicles (see attachment 1) utilized by the Berkeley Fire and Police Departments and the Administration Staff in the normal course and operation of Village business. Some equipment and vehicles are no longer necessary and useful to the Village and it is in the best interest of the Village that the property be disposed of. The equipment and vehicles will be disposed of in such a manner and way as determined proper and otherwise consistent with the provisions contained in Illinois Statute 65ILCS 5/11-76-4.
Previous Action
No previous action has been taken on this matter.
Recommendation
It is the recommendation of Village staff that the Village Board of Trustees approve a Resolution Authorizing for the Disposition of Certain Surplus Equipment and Vehicles Owned by the Village of Berkeley.
Budgeted Amount
The funding for the vehicles that will be sold will be put back into the Vehicle Replacement Fund to offset purchases of future vehicles.

WHEREAS, Illinois Statute (65ILCS 5/11-76-4) allows for the disposition by a Municipal
Corporation of “surplus” personal property; and

RE: An Ordinance Implementing an Indemnification Agreement with The County of Cook In Lieu of Insurance and Surety Bond Requirements for the repair of water main on Wolf Rd.
Background On the afternoon of February 18, 2020, a water leak was discovered in front of 1418 Wolf Rd. Appearing to be minor in nature, Public Works staff attempted to locate the source of the leak given its location in the road and the possibility that it would be a main break versus a service leak. Given its minor presentation, the leak was left for further investigation and potential underground leak detection on February 19th. Placed on standby behind other more major water main breaks for leak detection the morning of the 19th, staff discovered that freezing overnight temperatures and a forecasted high in the twenties created an ice build-up of several inches on the street as well as the nearby sidewalk. Given the concern for public safety, the repair was shifted to an emergency need and Vian Construction was called to assist with the repair. Wolf Rd. has a four-inch asphalt layer on top of a twelve-inch concrete layer that makes repair difficult, further complicated by traffic on the Cook County roadway. Work to be done in Cook County right of way typically requires a permit. Given the emergency nature of this water main repair, Cook County was contacted and an emergency permit was issued. The attached ordinance represents the “Hold Harmless” agreement that Cook County also requires as part of this repair. Previous Action The water main was successfully repaired on February 19, 2020. Recommendation Staff recommends approval of an Ordinance Implementing an Indemnification Agreement with the County of Cook In Lieu of Insurance and Surety Bond Requirements. Budgeted Amount There is no cost associated with this Ordinance approval. ORDINANCE IMPLEMENTING AN
INDEMNIFICATION AGREEMENT WITH
THE COUNTY OF COOK IN LIEU OF
INSURANCE AND SURETY BOND REQUIREMENTS
WHEREAS, the Village of Berkeley needs to perform construction, maintenance, repair,
or activities including, but not limited to, parades, direction of or stoppage of traffic to, or on,
Village of Berkeley property lying in, or upon, Rights of Way of the County of Cook within the
corporate limits of the Village of Berkeley, and
WHEREAS, the County of Cook with regard to the issuance of permits for disturbance of
County Right of Way requires that insurance protecting the County of Cook from any and all
claims that may arise during the course of or in consequence of such work be secured, and
further, that a surety bond be posted to guarantee restoration of the County Right of Way
following completion of any construction, maintenance, repair or activity; and
WHEREAS, the County of Cook will waive surety bond and insurance requirements in
favor of the Village of Berkeley and, will accept from the Village of Berkeley in lieu of such
insurance, a Hold Harmless Agreement.
NOW THEREFORE BE IT ORDAINED By the Village of Berkeley;
1. That for and in consideration of waiver by the County of Cook of the insurance and
surety bond requirements as foresaid for projects or activities which may be undertaken
by the Village of Berkeley involving a disturbance of any right of way of the County of
Cook within the corporate limits of the Village of Berkeley that we do hereby agree that;

a. The Village of Berkeley, or its successors, or assigns, shall indemnify and
save harmless the County of Cook, its officers, agents, employees and
servants against all loss, damage or expense that it or they may sustain as a
result of any suit, actions or claims of any character brought on account of
property damage, injury to or death of any person, or persons including
persons performing any work or activity under a permit issued by the
Cook County Transportation and Highways Department which may arise
in connection with the work or activities to be performed under all permits
arising in any manner from the negligence or wrongful acts or failure to
act by the Village of Berkeley and its employees in the performance of
the work or activity, or any contractor doing emergency work or
maintenance for the Village of Berkeley; and
b. The Village of Berkeley shall, at its own expense, appear, defend and pay
all charges of attorneys, costs and other expenses arising therefrom or
incurred in connection therewith, and, if any judgement shall be rendered
against the County in any such action, the Village of Berkeley shall, at its
own expense, satisfy and the Village of Berkeley shall discharge the
same.
2. The Village of Berkeley shall also indemnify and save harmless the County of Cook
from any claims against or liabilities incurred by the Village of Berkeley of any type or
nature, to any person, firm or corporation arising from the Village of Berkeley wrongful
or negligent performance of the work or activity.

3. No amendment or repealer of this ordinance shall become effective until thirty (30) days
next following written notice to the County of Cook of such proposed modification
repealer. Said notice shall be served upon the County of Cook at the office of the
Superintendent of Transportation and Highways, 69 W. Washington, 23rd Floor, Chicago,
Illinois 60602, Attn: Permit Office.
4. The clerk of the Village of Berkeley is hereby authorized and directed to forward to the
County of Cook, Department of Transportation and Highways, Permit Division, a
certified copy of this ordinance upon its passage and approval.
5. This ordinance shall be in full force and effect upon its passage, approval and publication
as required by law. All ordinances or parts of ordinances in conflict with the terms and
provisions hereof are hereby abolished.
6. Nothing in this Ordinance negates the necessity of obtaining a permit, where such permit
is otherwise required. This Hold Harmless Agreement does not negate bond and
insurance from private contractors doing new construction for the municipalities.

General Conditions
1. Definition of “Owner”: The “Owner” is the Name/s listed on the Cook County Transportation andHighways Department (CCDOTH) Permit as “Owner/s”. The “Owner” is the “Grantee” listed inthe Public Way Regulatory Ordinance (the “Ordinance”), Chapter 66.
2. Capitalized terms used in this Permit and not otherwise defined herein shall have the meaningsascribed to them in the Public Way Regulatory Ordinance (the “Ordinance”), Chapter 66, ArticleIII, and Sections 50 et seq. of the Cook County Code. Requirements set forth in these GeneralConditions are in addition to and not in limitation of the requirements of the Ordinance.
3. The CCDOTH Permit is only applicable for the work shown on the final approved submitted planson Cook County Right of Way (ROW). The permit does not release the Owner from fulfilling therequirements of any other laws or other required permitting relating to the permitted work. It isthe responsibility of the Owner to acquire all approvals for work not on Cook County ROW. TheOwner is responsible to acquire all approvals for any work on Cook County ROW that isconnecting to infrastructure owned by another entity.
4. Fulfillment by Owner of all requirements set forth in the permit application and its instructions,including without limitation, permit fees, insurance and bonding are a condition of this Permit.Issuance of this Permit, without the fulfillment of all requirements by Owner shall not act as awaiver of Owner’s obligation to comply with such requirements, unless approval in writing of suchchange is given by the Cook County Superintendent of Transportation and Highways.
5. The Permit can be revoked pursuant to the terms of the Ordinance or at the discretion of theCook County Superintendent of Transportation and Highways.
6. At least two (2) days advance notice prior to the start of work shall be given to the CCDOTHPermit Office, (312) 603-1670 or email [email protected] .
7. No changes, alterations, or revisions to the Permitted Work are allowed unless approved inwriting by the Cook County Superintendent of Transportation and Highways or his designee.
8. If Owner discovers during the progress of the Permitted Work that subterranean conditionsprohibit the construction of said improvement in and along the alignment as outlined in the plans,it is expressly understood that all Permitted Work shall cease until a proposed revised alignmenthas been approved by the CCDOTH and the Permit has been modified.
9. The Owner shall furnish all material to do all work required, and pay all costs which may beincurred in connection with such work, and shall prosecute the same diligently and without delayto completion. See Ordinance for additional requirements as to work in the Public Way.
10. All construction methods and construction materials shall be in accordance with the latest versionof the Illinois Department of Transportation (IDOT) Standard Specifications for Road and BridgeConstruction, IDOT Supplemental Specifications and Recurring Special Provisions, IDOTStandards, Cook County Special Provisions and Cook County Standards.
11. Upon completion of the Permitted Work, Owner shall in a timely manner, (but in no event morethan 30 days unless another time frame is directed by the CCDOTH Permits Division) restore thePublic Way substantially to the same condition in which it was before the Permitted Work started.The work includes but is not limited to removing all debris, rubbish, materials, apparatus, tools,and equipment, as well as all excess excavated materials, from the Public Way.

12. Should future construction and operation of the highways by Cook County require alteration or relocation of the Owner’s Facilities, such change shall be made by the Owner, its successor or assigns upon the written request of the Cook County Superintendent of Transportation and Highways without expense to said County or State. Requirements for any such requested alteration or relocation are further detailed in the Ordinance.
13. The Owner, its successor and assigns, assume all risk and liability for accidents and damages that may accrue to persons and property, during the prosecution of the work or any time thereafter, by reason of the location, construction, installation, operation, maintenance, repair and work referred to herein, and the Owner, by acceptance of the Permit, agrees to indemnify and save harmless Cook County from any such claims for damages and from all costs and expenses incurred on account thereof and in connection therewith.
14. In accordance with the Ordinance, and agreement by the Owner, the Owner acknowledges and agrees that the Permit is null and void if the Owner is delinquent in the payment of any tax or fee administered by the Cook County.
15. Furnish the CCDOTH Permits Division one as-built PDF in 24”x36” format.
16. Notify CCDOTH Permits office in writing for final inspection. The letter can be email to [email protected]
CCDOTH Construction Notes
Drainage
1. The drainage systems shall be kept clean and free of debris at all times.
2. The Owner shall be responsible for providing positive drainage.
3. CCDOTH reserves the right to make connections to the proposed storm sewer for the purpose of draining the highway.
4. As a condition of granting this permit, which includes the point discharge of storm water onto the Cook County Transportation and Highways Right Of Way, the Owner hereby grants permission to the Cook County Transportation and Highways Department to enter onto private property to inspect the detention control structure.
Erosion Control and Landscaping
5. The parkway shall be kept clean and free of debris at all times.
6. Any disturbed areas within Cook County ROW require erosion control blanket prior to final landscaping per current Illinois Environmental Protection Agency (IEPA) standards.
7. Cook County Right-of-Way to be restored with 4” topsoil, fertilizer and sod. This note supersedes any note in the plans.
Excavation and Backfill
8. The Owner shall manage the excavation, transport and disposal of all excavated materials (i.e. soil, debris, etc.) in accordance with local, state and federal regulations.
9. As a condition of this permit, the Owner shall request CCDOTH to identify sites in the Right-of-Way where a Highway Authority Agreement governs access to soil that exceeds the Tier 1 residential remediation objectives of 35 Ill. Adm. Code 742. The Owner shall take all measures necessary to protect human health (including worker safety) and the environment during and after any access to such soil.

11. All trenches within Cook County ROW shall be trench backfilled with FA-6 sand in accordance with Method 1 in accordance with Article 550.07 of the (IDOT) Standard Specifications for Road and Bridge Construction.
Median, and Curb and Gutter (PCC)
12. In the removal of medians and curb and gutter the use of any type of concrete breaker that will damage the underground structures will not be permitted.

15. The pavement shall be kept clean and free of debris at all times.
16. Unless specified in the Permit, no equipment other than pneumatic-tired equipment used during the installation shall be permitted to stop or operate on the pavement nor shall any excavated materials be stored temporarily or otherwise on the CCDOTH pavement.
17. Saw cut the full depth of pavement, median, sidewalk and curb and gutter at the limits of removal.
18. All pavement patch openings that are open to traffic shall be immediately surfaced with a temporary bituminous patch at least three inches in thickness. This patch then must be inspected daily and additional bituminous patch material must be placed, daily if necessary, to maintain the patched area at the same elevation as the adjacent undisturbed pavement for a period of not less than 30 days. After 30 days, permanent replacement in kind shall be made to the base course and pavement surface.
Pavement, Entrance (Driveways, Side Streets)
19. For entrance installations, the Owner shall remove earth to its full depth, starting at the edge of the pavement, for the full dimensions of the proposed entrance, and replace with materials to be used in the construction of the entrance.
20. The entrance radius meeting the edge of shoulder or the back of curb must terminate 3' from the property line extended to the edge of shoulder or the back of curb. If this criteria cannot be met, a letter from the neighboring property authorizing the encroachment must be submitted.
21. The CCDOTH reserves the right to restrict access to permitted entrances on future roadway improvements.
22. The Owner acknowledges that if or when the County of Cook improves the highway the pavement composition at the above mentioned entrance(s) may be substituted.
Pavement, Hot Mix Asphalt (HMA) pavement, patching, and resurfacing
23. HMA Full Depth Pavement thickness shall be 12-inch on a 12-inch thick sub base granular material consisting of 3 inch top lift of CA 06 and 9 inch bottom lift of Porous Granular Embankment (PGE). HMA Pavement shall be built per the IDOT Standard Specifications for Road and Bridge Construction Art 407. The PGE sub base shall be built per the IDOT Standard Specifications for Road and Bridge Construction Art 207 and 1004.05.
24. HMA Mill and Resurface Pavement thickness shall be per the approved permit plans. HMA
Resurfacing shall be built per the IDOT Standard Specifications for Road and Bridge Construction Art 406.

25. HMA Patching shall match the existing pavement thickness. The patch length shall be a minimum of 6 feet. The patch width shall be either 6 feet (from edge to center of lane or, from center of lane to centerline) or 12 feet (full lane width). The pavement opening shall be saw-cut to the full depth of the pavement at the limits of removal and must extend 1 foot beyond each side of the trench excavation. HMA Pavement Patch shall be built per the IDOT Standard Specifications for Road and Bridge Construction Art 407.
26. The top lift for all above HMA pavements shall be Hot-Mix Asphalt Surface Course, Mix “D”, N70 at a thickness of 2 inch.
27. Below the top lift for all above HMA types shall be Hot Mix Asphalt Binder Course, IL-19.0, N70 at a minimum thickness of 2-1/4 inch (Resurfacing if used) to 10 inch (Full Depth, Patching) and a max lift thickness is 4 inch. Pavement, Portland Cement Concrete (PCC) pavement and pavement patching
28. Saw cut the full depth of pavement.
29. PCC Pavement thickness shall be 10 inch on a 12-inch thick sub base granular material consisting of 3 inch top lift of CA 06 and 9 inch bottom lift of Porous Granular Embankment (PGE). 31. Pavement patches greater than or equal to 15SY shall use pavement fabric in accordance with IDOT standard 420701 and provide 3 ½ inches of clearance between the pavement surface and the top of the fabric.
32. Where the proposed pavement or median abuts the existing pavement, median or curb and gutter longitudinally, provide a tied longitudinal construction joint in accordance with IDOT standard 420001, using 30” long #6 (3/4” Dia.) deformed tie bars at 24 inch centers.
33. Where the proposed pavement or median abuts the existing PCC pavement or median transversally, provide a transverse joint in accordance with IDOT standard 442101, using 18” long #12 (1-1/2” Dia.) smooth epoxy dowel bars at 12 inch centers.
34. Provide transverse sawed contraction joints every 15 feet in accordance with IDOT standard 420001, using 18” long #12 (1-1/2” Dia.) smooth epoxy dowel bars at 12 inch centers and align proposed joints with existing joints. If a proposed joint is located less than 6 feet from an existing joint, then the existing pavement or median shall be removed and replaced up to the existing joint.
35. Construct curb and gutter in accordance with IDOT standard 606001. Provide a tied longitudinal construction joint in accordance with IDOT standard 420001, using 30” long #6 (3/4” Dia.) deformed tie bars at 24 inch centers.
36. Construct medians in accordance with IDOT standard 606301. Provide a tied longitudinal construction joint in accordance with IDOT standard 420001, using 30” long #6 (3/4” Dia.) deformed tie bars at 24 inch centers.

37. Where a median opening is provided, the pavement shall be crowned at the centerline using a
one percent cross slope. Sidewalk and Bus Shelters
38. In the removal of sidewalk the use of any type of concrete breaker that will damage the underground structures will not be permitted.
39. All proposed bus shelter and bus shelter pads must meet the current IDOT Bureau of Design and Environment (BDE) Manual and IDOT Bureau of Local Roads (BLR) Manual, Public Rights-of-Way Accessibility Guidelines (PROWAG) and Americans with Disabilities Act (ADA) requirements.
40. All proposed sidewalk (cross-walk) shall be ramped in compliance with the current IDOT BDE Manual, IDOT BLR Manual, PROWAG and ADA requirements.
41. All proposed curb ramps shall be inspected after construction. IDOT form D1 PD0031 shall be filled out for each location. If there are any deficiencies the deficiencies shall be fixed and the form refilled out for the location until the curb ramp is compliant. A copy of the final form shall be submitted to the CCDOTH Permits office at [email protected] for the permit file. CCDOTH Permits office will forward the completed forms to the Cook County ADA Coordinator for the Cook County ADA file.
42. All proposed sidewalk shall be 8" thick within driveway limits.
Traffic Control
43. Owner shall provide and maintain at its own expense, such temporary roads and approaches, as may be necessary to provide access to driveways, houses, buildings or other property abutting the site of the Permitted Work. Access shall not be blocked.
44. No lane closures or traffic detours relating to Permitted Work will be allowed between the hours of 6 a.m. to 9 a.m. and 3 p.m. to 6:30 p.m., (other than as allowed for emergency maintenance per the Ordinance). All traffic control devices must conform to IDOT Highway Standards and the latest edition of the State of Illinois Manual on Uniform Traffic Control Devices (MUTCD) for Streets and Highways. All lane closures shall be in accordance with IDOT Highway Standards.
45. The Owner shall conduct its operations in a manner so as to insure the minimum hindrance to traffic, using the pavement and at no time shall its operations obstruct more than one half (1/2) of the available pavement width.
46. When existing traffic control signs such as stop signs, stop ahead signs, and crossroad signs are removed in the progress of the Permitted Work, said signs shall be immediately reset as close as possible to their original location. After the completion of the Permitted Work has been approved, said traffic control signs shall be restored to their original position and condition or as directed by the CCDOTH Permit Engineer.
47. The use of flagmen and the number, type, color, size and placement of all traffic control devices shall conform to IDOT Highway Standards and the latest edition of the State of Illinois MUTCD for Streets and Highways.
Utilities, All
48. It shall be the responsibility of the Owner to co-ordinate with the all utility companies sharing the Cook County ROW and relocate the existing power poles, fire hydrants, guardrail and appurtenances as needed for the proposed permit work. There shall be no cost to the county.
49. As a requirement of this permit all utility owners (private and government) shall maintain a membership with J.U.L.I.E. locating service until the utility is completely removed from Cook County ROW.

Utilities, Aerial
50. All aerial lines crossings or parallel must have a minimum clearance of 18’3”.
51. Pole owner permission is required for all cable, conduit and other appurtenance connection to a pole.
52. Proposed aerial cable shall not block the existing traffic signal heads.
53. Proposed aerial cable shall not touch existing traffic signal equipment.
Utilities Underground
54. All auger pits and excavations shall be a minimum of 10 feet from the edge of pavement or back of curb, and wood or steel sheeting shall be used. Auger pits left open overnight shall be protected with concrete barrier walls. The ends of the concrete barrier walls shall be protected with crash attenuators. The barrier wall and crash attenuators design shall meet IDOT BDE Manual and IDOT BLR Manual Design requirements.
55. All external casing voids shall be pressure grouted or filled with trench backfill using pumping or jetting outside of the casing. The inside of the casing shall be sealed or filled using the external void procedures.
56. That a minimum depth of 42 inches will be maintained from the ground surface to the top of the conduit, cable, or pipe and a minimum depth of 36 inches from the true flow line of the drainage ditch to the top of the conduit, cable or pipe.
57. Proposed underground utilities running parallel to existing water main or sanitary sewer shall adjust the alignment if the utility is within 5 feet of the outer wall of the water main or sanitary sewer. The proposed utility shall maintain a distance of 5 feet or greater while running parallel to the existing water main or sanitary sewer. The distance between parallel or crossing sanitary or storm sewer with water main shall meet IEPA requirements.
