WHO CAN ENTER

Contestants Must Be:

1. At least 18 years of age or older from the end of the contest on December 11, 2009.

2. Registered UND student members of the UND Wellness Center during the entire span of

the contest from September 14, 2009 - December 11, 2009. (This includes all Wellness

Center UND student members who are not volunteers or employed at the Wellness

Center during the contest)

3. Wellness Center volunteers and employees are not eligible to participate

HOW TO ENTER

4. Participants must enter in teams of two with only one entry per person. Teams may

consist of any combination of eligible participants as defined above. All entries must be

received by 11:59 p.m. local time on Friday, September 11, 2009 and the contest begins

at 12:01 a.m. local time on Monday, September 14, 2009.

5. To enter, go to the UND Wellness Center located at 801 Princeton Street Grand Forks,

ND 58202, and fill out a registration card for your team at the VIVA WOW! kiosk located

in the atrium. (One team member can enter entire team but registering someone without

consent will result in disqualification) This information is needed for registration: First

and Last Name, Student ID Number, and Phone of each team member, as well as a Team

Name. Registration for the contest will begin at 5:00 p.m. local time on Monday, August

24, 2009 and all entries must be received by 11:59 p.m. local time on Friday, September

11, 2009. Once the registration card has been completed, the team is officially entered

and the team name will appear on our Wellness Center Website and Facebook Fan Page.

6. Team Conduct. Every team is responsible for the conduct of its participants. Any conduct

judged as detrimental to the participants, programs, or the contest may result in loss of

the contest, suspension of the team, or appropriate action as outlined in Wellness Center

participant policies and the UND Code of Student Life. Team names must be appropriate

with no foul language, innuendoes, profanity or any other name that may be offensive to

any of the other participants, teams, or the program. The Wellness Center has the right

to change team names if they are deemed to be inappropriate by Wellness Center staff.

HOW TO WIN

7. Teams will compete in a qualifying contest from September 11, 2009 – November 20,

2009. The goal of the qualifying contest is to select (5) Five teams as “Finalists”, based on

their VIVA WOW! Coins that they’ve amassed as a team over the twelve week contest.

Then, the (5) Five team “Finalists” will go on to compete in the VIVA WOW! Finalists

Video Challenge which begins on Monday, November 23, 2009. Those (5) Five “Finalists”

must submit a video by Friday, December 4, 2009 by 11:59 p.m. local time. Fan voting on

the Finalists videos will take place the week of December 7, 2009 with the winners being

announced on Friday, December 11, 2009. (The videos will be voted on based on the

“fan’s favorite” video showing how the UND Wellness Center has changed the team’s

healthy behaviors)

THE PRIZE(S)

8. There will be a total of (5) five “second place” winners; (4) four “Finalists” winners; (1) one

“Grand Finalist”; and (1) one Grand Prize winner. “Second place” winners will receive a

package which includes (2) two DVDs, (2) two VIVA WOW! T-shirts, (2) two Wellness

Center Water bottles, and more. Four (4) “Finalists” winners will receive Nike+ iPod Sport

Kit with choice of either iPod Shuffle or Nike Free 5.0 Running Shoes valued at $250 per

team.

9. The Grand Prize: One (1) “Grand Finalist” will be voted on by peers in the Viva Wow!

Video Challenge and will receive a package which includes (1) one pair of roundtrip

airline tickets to Las Vegas, Nevada and a (4) four night stay on the Las Vegas Strip

during Spring Break 2010. The trip must be used the week of March 15, 2010. The trip is

for the two participants of the winning team only and includes one hotel room for the

two participants to travel together. Upon winning, the two participants must claim the

prize together at the UND Wellness Center no later then Friday, December 18, 2009. At

this time the UND Wellness Center will book the two round trip flights and one hotel

room for the two winners. The UND Wellness Center will cover up to $300 USD per

roundtrip flight and up to $600 on the hotel room. This is based on current prices for

roundtrip airline tickets from GFK-LAS on allegiant flights and a 4 Night stay at the

Imperial Palace in Las Vegas.

10. Prizes cannot be redeemed for cash or substituted for any other items by the winner(s).

The winner(s) cannot assign or transfer prize. The UND Wellness Center reserves the right

to substitute a comparable prize of like or greater value, including cash, for any prize, for

any reason.

11. The payment of any tax liabilities incurred by the entrant as a result of this contest and

any prize won are the sole responsibility of the entrant and entrant will be required to

provide his/her Social Security number for tax reporting purposes. When required, the

University will file an IRS Form 1099 with the Internal Revenue Service for the

approximate retail value of the prize. Once claimed, a prize cannot be revoked/returned

by winner to avoid tax obligation. The entrant will indemnify and hold harmless the

University if taxing authorities attempt to collect unpaid taxes from the University.

12. By participating in this contest, entrants agree to release the University of North Dakota,

the Wellness Center, and their officers, directors, agents, representatives and employees

(collectively, the "Released Parties") from any and all actions, claims, injury, loss or

damage arising in any manner, directly or indirectly, from participation in this contest

and/or acceptance or use of the prize. By accepting any prize that they may win,

entrants also authorize the Released Parties to use their name, photo, video, and

information about their participation in Viva Wow! in promotional material or on a

winner's list (if applicable) without further compensation, unless prohibited by law.
