Mission of USA Volleyball The United States Volleyball Association (USAV) is the National Governing Body (NGB) for the sport of volleyball in the United States and is recognized by the Federation International de Volleyball (FIVB) and the United States Olympic Committee (USOC). The vision of the USVBA is to be acknowledged as the world leader in volleyball. In order to accomplish this mission, the following goals are to be achieved:

� Competitive Success: To win gold medals in every international competition; � Sport Growth: To achieve full participation in volleyball at all levels and in all

geographical areas; � Sport Enhancement: To improve support services necessary for the quality and conduct

of programs to ensure recognition as the authority and expert for volleyball; � International Representation: To have influential positions on all international boards and

commissions; � Recognition: To receive consistent recognition of volleyball coaches and athletes as top

World Qualifiers Coming to Colorado Springs BY BILL KAUFFMAN | MARCH 11, 2014, 12:46 P.M. (ET) COLORADO SPRINGS, Colo. (March 11, 2014) – USA Volleyball will host concurrent men’s and women’s 2014 FIVB World Championship NORCECA third-round qualification tournaments May 15-18 at the U.S. Olympic Training Center (OTC) in Colorado Springs. The Colorado Springs Sports Corp will serve as the local organizer. The World Championship, the pinnacle event of the FIVB, is held every four years with each of its five zones holding qualifying tournaments into the 24-country event for each gender. NORCECA includes National Federations from North America, Central America and the Caribbean. “USA Volleyball is excited to have this opportunity to showcase elite international volleyball at the U.S. Olympic Training Center and working with the Colorado Springs Sports Corp on putting together a memorable event,” USA Volleyball CEO Doug Beal said. “We are proud to call Colorado Springs our headquarters hometown where so many Olympic dreams come through in all sports.” Both the men’s and women’s champions earn a spot into their respective FIVB World Championship. The U.S. Men will face Guatemala, Haiti and St. Lucia in round robin action May 15-17. The U.S. Women host Guatemala, Honduras and Panama in round robin pool play May 15-17. Both genders will hold championship and third-place matches on May 18. All matches will be held in Sports Center I on the OTC complex. Both tournament pool schedules will be announced later this month. All four teams in each gender tournament will play once a day. USA Volleyball will video stream all matches during the tournament. Daily match tickets include four matches with both U.S. teams playing once each day. Ticket prices for the qualification tournament are:

� Daily General Admission: $10 � Daily Groups of 12 or More in Advance: $8 per ticket � Event Pass for all Four Days (16 matches): $30

Tickets will be available for purchase at the door or online (click here). Advance ticket orders will be available at Will Call on the day of the event. Group ticket rates available online only. Based on ranking within the NORCECA zone, both the U.S. Men and U.S. Women advanced directly to the third round of the NORCECA qualification system into the FIVB World Championship. Each of their third-round opponents competed in the second round. On the women’s side, Panama and Guatemala won their respective second-round pool and Honduras was the third-place team in its group. On the men’s side, Guatemala and St. Lucia won their second round pools and Haiti finished second.

Expectations of volunteers

� Volunteers are expected to read and understand the handbook.

� Always be polite and courteous, you are representing USA Volleyball and we want everyone to have a positive experience while attending the event.

� Both on-court operation volunteers and event logistic volunteers must be present at a pre-practical training held in 4-5:30pm on Monday, May 12.

� On-court operation volunteers must be also present at practical clinics held in 5:00-6:30pm on Wednesday, May 14. The volunteers are required to arrive 30 minutes before the practical clinic starts.

� Assigned volunteers will be expected to be at the venue 1 hour prior and 30 minutes after each match.

� If volunteers are going to be late they must contact the volunteer liaison. � Volunteers are expected to read and sign the attached volunteer contract and return to the

volunteer liaison. � Volunteers are expected to report any accidents, injuries or illnesses to the volunteer

liaison immediately.

Dress Code

� Volunteers are expected to wear event t-shirt and credentials given to them by USA Volleyball. This uniform will show security, USA Volleyball staff and participants that you are part of the official event.

� Shirts must be tucked into shorts. � Shoes must be clean with no black soles.

Credentials

� Credentials must be worn at all times except when you are on the competition floor. At this time give your credential to the Volunteer Liaison for safe keeping.

o Upon leaving the floor you must collect your credential from the volunteer liaison.

� Your credential will give you access to a volunteer assembly area by the main competition floor to meet the Volunteer Liaison. This credential does not allow you access to the player locker rooms.

During the Match

� Volunteers must be attentive to their jobs. Pay attention to what is happening on the court and ensure that you are completing your job to the best of your ability. The match will move quickly and you must pay attention to what USA Volleyball staff or the officials tell you while on duty.

� No cell phones are allowed to use on the court.

Security

� Always wear your official NORCECA World Championship Qualifiers Credential. � Never carry any weapons, regardless of the type of license you hold. � Do not attempt to resolve any conflict. Locate the closest security guard or staff member

immediately about the situation.

Parking Parking is free for volunteers. We ask that you try to carpool with friends to save spaces for others. Details will be given to you through your Volunteer Liaison. Volunteer and staff parking areas will be assigned and may be located outside of the Olympic Training Center parking area. Please park only in the area assigned to volunteers due to the limited parking available at the OTC.

Problems and Concerns Communication is a key to the success of the NORCECA World Championship Qualifiers. If you have questions or concerns, please contact the Volunteer Liaison immediately.

Job Descriptions Volunteer Liaison: Responsible for finding and securing volunteers for each match. They are responsible for ensuring that the assigned volunteers show up on time and are ready for the event. (On Court Operational Volunteers)

Ball Shaggers (6): This person is responsible for ensuring the players have the ball in a timely manner during the match. They will be responsible for rolling the ball to the necessary person for the continuation of play. Floor Wiper (2): The floor wiper will run onto the court when told by the down official. They must wipe any moisture from the floor after a play is complete. They are only allowed to wipe the floor when the official tells them to not when a player requests. Floor Mopper (6): The floor moppers mop the floor in an organized fashion during the technical time outs as well as the team time outs. Exact timing and fashion to mop the floor will be outlined in the practical clinics. See Appendix A for more details. Flag Bearers(8): These people can double as ball shaggers or floor wipers etc. These volunteers will hold the flags of the respective countries playing in the match, during the march in, introductions and playing of the national anthems. See Appendix B for ceremony details.

Runners (2): The runners are responsible for assisting the statistician and public relations personnel with needs they may have during the matches. They will be responsible for distributing stats to the press, teams, announcers and supervisor during the matches. (Event Logistic Volunteers) Mascot (1): This person appears with a mascot costume by the court during matches and on the court at intervals of matches and sets to generate Team USA sprits. This volunteer will only be working during prior to, during and following Team USA matches. Will Call (4): Will Call volunteers will be stationed in the foyer of Sports Center I. They will be responsible for distributing pre-purchased tickets to fans with proper ID. They will also be responsible for walk-up tickets sales. Ticket Takers (2): The ticket takers validate the admission tickets/stamps and credentials of teams officials and staff at the east entrance doors to the venue. Ticket Takers will be responsible for stamping the hands of fans as they enter for re-entry. They must verify that tickets being used for entry are appropriate for the session. VIP Hosts (2): The VIP Hosts will check guests into a hospitality area by monitoring credentials or authorizations of event staff. The VIP Hosts provide the guests with needed assistance to create fun spectating experiences. Visitor Services (18): The visitor services will guide attendees to right direction for their fun and safe spectating experiences. Some visitor services will be assigned around a match court to lead attendees to correct area, some will be assigned in hallway area to keep attendees from restricted area, and some will be assigned in parking lots to direct the right parking spots. The visitor services assigned around the match court will be responsible for assuring that seating areas are filled to capacity when possible. They may need to ask spectators to slide towards the center of the bleachers to allow for space for additional spectators. This should be done in a respectful and courteous manner at all times. In addition, they will hand out programs and handbills. The visitor services assigned in hallway area will be responsible for keeping team locker rooms area closed from attendees and other public people. Only if attendees go to bathrooms next to team locker rooms, they are allowed to get in the closed area. The visitor services assigned in parking lots will direct match attendees to park their cars at the assigned spots in the Olympic Training Center. They will monitor the parking areas, and walkways leading to Sports Center I. Spanish Translators (4): The translators will provide interpretation to the participating teams which come from Spanish speaking countries during stay at the Olympic Training Center. The translators will also interpret comments made by those team coaches during press conferences. French Translator (1): The translators will provide interpretation to the participating teams which come from French speaking countries during stay at the Olympic Training Center. The translators will also interpret comments made by those team coaches during press conferences. Sales Attendants (2): The sales attendants will be engaged in onsite event goods sales . The merchandise table will be located near the entrance of the Sport Center 1. The volunteers operate

cash register and receive payment from attendees in cash or credit card, and accurately counts and provides change to attendees when applicable. Concession Attendants (2): The concession attendants will be responsible for carrying and selling foods and beverages throughout the matches. The merchandise table will be located around the entrance of the Sport Center 1. The volunteers operate cash register and receive payment from attendees in cash or credit card, and accurately counts and provides change to attendees when applicable. Training Site Assistants (2): The training site assistants will provide each team with assistance regarding their training at Sports Center 2 if needed. The volunteers will ensure that each training court is cleaned up before and after each training session and that provided equipment, such as a net, balls, towels, beverages, etc., is returned without any missing after training. The time shifts training s of training site assistants will be in 7:30am-1:30pm and 6:30pm-9:30pm.

VOLUNTEER WAIVER AND RELEASE AGREEMENT In consideration of being allowed to participate in the Colorado Springs Sports Corporation athlete/sports

program, related events and activities, the undersigned appreciates, acknowledges and agrees that:

1. There are risks of injury from participating in the activities involved in this program or events, including the potential for paralysis, disability or even death. (2) (a) I have consulted with and obtained the authorization of my primary care physician (“Physician”), in order to allow me to participate in the physical and emotional aspects of the Activity; (b) I will share any results of the Activities with my Physician; and (c) I will follow the advice of said Physician in connection therewith;

(3) I FULLY UNDERSTAND that: (a) THE ACTIVITY MAY INVOLVE INHERENT AND OTHER RISKS AND DANGERS OF SERIOUS BODILY INJURY, INCLUDING PERMANENT DISABILITY, PARALYSIS AND DEATH ("Risks"); (b) these Risks and dangers may be caused by my own actions or inactions, the actions or inactions of others participating in the Activity, the c onditions in which the Activity takes place, the equipment used for the Activity, the facilities in which the Activity takes place, and/or THE NEGLIGENCE OF THE "RELEASEES" NAMED BELOW; (c) there may be OTHER RISKS AND SOCIAL AND ECONOMIC LOSSES either known or not known to me or not readily foreseeable at this time but that may arise in connection with my participation in the Activity; and I FULLY ACCEPT AND ASSUME ALL SUCH RISKS AND ALL RESPONSIBILITY, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERS; FOR ANY LIABILITIES, LOSSES, COSTS AND/OR DAMAGES I incur as a result of my participation in the Activity;

(4) I willingly agree to comply with the stated and customary terms and conditions for participation, if, however, I observe any unusual or unnecessary hazard during my presence or participation, I will take reasonable steps to avoid any hazard and immediately bring such to the attention of the nearest official.

(5) I consent to abide by the rules of the Activity and conduct for guests of the OTC as applicable to the Activity and understand that violations will result in full or partial forfeitures of the Activity.

(6) I, FOR MYSELF, AND ON BEHALF OF MY HEIRS, ASSIGNS, PERSONAL REPRESENTATIVES, and NEXT OF KIN, HEREBY RELEASE, HOLD HARMLESS,PROMISE NOT TO SUE, AND WAIVE ALL CLAIMS RELATING TO THE UNITED STATES OLYMPIC COMMITTEE, OR OTHER SPONSORING ORGANIZATION (IF ANY), THEIR OFFICERS, COACHES, VOLUNTEERS, STAFF, SPONSORS, AND/OR AGENTS, ("RELEASEES") WITH RESPECT TO ANY AND ALL LIAIBLITY, CLAIMS, DEMANDS, LOSSES, OR DAMAGES CAUSED IN WHOLE OR IN PART BY MY PARTICIPATION IN THE ACTIVITY FROM WHATEVER CAUSE, INCLUDING, WITHOUT LIMITATION, THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, EXCEPT AS THE RESULT OF GROSS NEGLIGENCE OR INTENTIONAL MISCONDUCT.

(7) This Waiver and Release of Liability shall remain valid for the entire Activity and for the full calendar year in which it is signed or until it is expressly revoked by written notice from me to the USOC, whichever occurs first; provided however, that any such revocation shall not in any manner affect the waiver and release of liability given hereunder for any acts or occurrences prior to receipt of said written notice by the USOC or prior to termination of my participation in the Activity.

(8) I FURTHER AGREE that if, despite this RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT I, or anyone on my behalf, makes a claim against any of the RELEASEES, I WILL INDEMNIFY, SAVE, AND HOLD HARMLESS EACH OF THE RELEASEES from any litigation expenses, attorney fees, loss, liability, damage, or cost which may incur as the result of such claim.

(9) This Release and Waiver of Liability shall be governed by the laws of the State of Colorado and shall be binding on the parties hereto and their heirs, personal representatives, successors and assigns. Venue for any action shall be Colorado Springs, Colorado.

(10) This Release and Waiver of Liability constitutes the entire agreement between the parties hereto with regard to the subject matter herein and supersedes and replaces any prior agreements, oral or otherwise, between the parties hereto. No change or modification of this Release and Waiver of Liability shall be valid unless the same is in writing and signed by the both parties.

OTC PARTICIPANT CODE OF CONDUCT

All Participants agree to abide by the below described code of conduct for athletes, coaches, and guests of the USOTC, as app licable and understand that violations may result in forfeiture of privileges or other disciplinary proceedings.

1. The transportation, possession or unauthorized use of alcoholic beverages, illegal drugs, drug paraphernalia, or IOC-banned

substances on the premises is prohibited.

2. Tobacco in any form is not permitted at the OTC.

3. Participants must keep their ID card on them at all time, and may not allo w ano ther to use P ar ticip ant’s I D ca r d at any

time. This includes passing ID cards to allow another access the dining hall.

4. The OTC is not responsible for loss or damage of personal property owned by athletes, coaches, guests or residents on site.

USOC Risk Management recommends the purchase of tenant insurance by individuals residing on site.

5. Animals are strictly prohibited at the OTC, including the dorms, with the exception of service animals. Service animals must

check in with the front desk staff of the OTC for proper registration of the animal.

6. Tactical sports equipment (e.g. firearms, ammunition, archery arrows, and bows) must be reported to front desk staff upon check-

in so that it may be stored in the appropriate location as determined by the OTC. Oversized sports equipment should not be stored in the dormitory.

7. For those Participants visiting the OTC, visitors are prohibited in the dormitory areas or on the premises between 12:00 AM and

7:00 AM daily (Colorado Springs), 11:00 pm – 7:00 am (Chula Vista and Lake Placid).

8. For those Participants visiting the OTC, OTC quiet hours are from 10:00 PM – 7:00 AM daily.

9. For those Participants staying and/or living at the OTC, any physical damage to a facility or loss of items from a dormitory room

(e.g., blankets, lamps, etc.) will be paid for by those individuals assigned to the room in which the damage or loss occurs.

10. For those Participants staying and/or living at the OTC, candles, incense, or any other type of open flame items and appliances are prohibited in the dormitories.

11. For those Participants staying and/or living at the OTC, space heaters, electric frying pans, toaster ovens, microwaves, hot plates,

coffee pots, or any appliance or equipment rated above 6 amps and/or 750 watts are strictly prohibited in the dormitories. I f Participant needs additional outlets, a multiple outlet strip is required. Extension cords and plug-in adapters are not acceptable.

12. For those Participants staying and/or living at the OTC, the dormitory hallways must remain clear of debris at all times. No one

may store bicycles, furniture or other items in the hallways. If Participant requires bicycle storage, or need an item of furniture removed, please contact the front desk staff at x4444 (Colorado Springs), x#9009 (Chula Vista), or x2600 (Lake Placid).

13. For those Participants staying and/or living at the OTC, checkout from the dorms on day of departure is 9:00am. All belongings

must be removed from Participant’s room prior to 9:00am, but Participant is allowed to keep Participant’s access card until after breakfast/lunch, as applicable. If Participant has an afternoon flight, Participant may store Participant’s belongings in the luggage room, across from the registration desk in the athlete center in Colorado Springs or in the ACI in Chula Vista. Check with the front desk staff for access. Additional charges will be assessed if Participant does not vacate Participant’s room at the time set forth above.

14. If Participant would like to keep Participant’s ID as a souvenir upon departure, Participant may do so. However, Participant must

scan Participant’s ID at the front desk before leaving the OTC in order to properly checkout of Participant’s Activity.

15. Participant should direct any questions regarding this Code of Conduct to the front desk staff.
