Editor’s commentWhilst reading through Phil Mullin’s column for this issue, I idlypondered the potential benefits of having a cocktail bar inpractice. Perhaps that’s taking the customer service concept atad far – but there’s plenty to be said for raising your waitingarea game with interactive touch-points for both children andadults; a small TV playing cartoons in a kids’ zone, say, alongside

current fashion magazines on a coffee table (with an actual coffee too), ortablets with patient education apps installed. These are all relatively simplesteps that can lead to a more pleasurable patient journey in your practice; turnto page 19 for more thoughts on the subject.

Thanks to upgraded and redesignedsoftware, the new ImpressionIST 4 videocentring system from Rodenstock is said tooffer a more intuitive 3D measurementmodule and faster analysis of the patient’sdata. The heart of the unit is the 3D stereocamera technology for fast, accurate andautomatic determination of individualparameters, and it does everything withjust one photograph. The General Optical Council (GOC) has launched a public consultation on a draft policy forconsensual disposal of fitness to practise (FTP) cases. The policy has been designed to helpthe GOC carry out its function of investigating and acting on FTP concerns proportionatelyand fairly. According to the council, consensual disposal will be appropriate in a case wherethe registrant admits the facts of the allegation, the grounds of impairment, that their FTPis currently impaired and the registrant and the registrar are in agreement about theappropriate level of sanction. All consensual disposal agreements will be approved by theGOC FTP Committee. The consultation will run until October, and the outcome will beconsidered by the council at its November meeting.

CooperVision has acquired ProcorneaNederland for an undisclosed sum, and with it an in-road to the ortho-k sector viaProcornea’s DreamLite technology. “Byworking together on the significantchallenges of myopia and other visionissues, CooperVision and Procornea willundoubtedly have an even greater impactin changing lives around the world,” saidAndrew Sedgwick, CooperVision executivevice president, strategy and businessdevelopment. Procornea will continue tooperate as a complementary business for thetime being, retaining its offices and brands.

SynergEyes UK is holding a series of CETroadshows for practitioners in Septemberand October to celebrate the distribution ofSynergEyes hybrid contact lenses in the UKand to launch the new SynergEyes Duetterange. The roadshows will feature a one-hour CET lecture by Nick Atkins, and a 30-minute live fitting demonstration from theSynergEyes professional services team, ledby Phil Thompson. Essilor has begun manufacturing its flagship Varilux X series at its UK Centre of Excellence inThornbury. The site provides a local manufacturing platform for all digitally surfacedpremium lenses and, after a rigorous and successful testing phase, local Varilux X seriesproduction is now fully operational.

Essilor MD, Mike Kirkley, said: “I was delighted to witness the first official order being madein Thornbury. This event generated a lot of excitement for the business because productdemand has grown at an unprecedented rate. A new super-primary eyecare company (PEC),encompassing five local optical committees(LOCs) and covering 10 million people, couldbe a template for the sector to match thesustainable transformation partnerships (STPs)being rolled out by the NHS as part of itsreforms, claims the Local Optical CommitteeSupport Unit (LOCSU). However, LOCSU COO,Richard Whittington, insisted that the opticalsector would need to match the geography ofSTPs if it was to achieve its strategic objectiveto see eye health services delivered at scale.He also said that the merger was a blueprintfor the consolidation of PECs across LOCs andwithin optics.

His comments were made following mergertalks hosted at LOCSU at the end of May.Around the table were representatives fromEast Sussex, West Sussex, Dorset, Wiltshireand Hampshire LOCs. Talks are ongoingabout widening this to cover other areas inthe south of England. All five LOCs andLOCSU agreed a proposed structure for themega-PEC. Each LOC would have boardrepresentation with relationships andresponsibilities. Discussions also set out thedistribution of responsibilities between theLOC and the PEC.

The next priority for the sector would be thepublication of a detailed, national model forstep-down care services. “By doing this, wewill have a unified approach in terms of ourcontracting model, our clinical model andour cost mode,” Richard said. “This year’sNational Optical Conference will also focuson a series of practical workshops that willensure that those appointed to a PEC todeliver governance, financial, clinical andperformance elements of the contract havethe specific skills and support to succeed.”Visit www.locsu.co.uk for links to NOC feedson social media and a video update.

The countdown to World Sight Day on 12 October has begun and Vision Aid Overseas (VAO)is inviting practitioners to register online to support this year’s campaign. Sarah Namusobya,VAO fundraising officer, said: “World Sight Day is the only international day of awarenessand fundraising to focus global attention on blindness and vision impairment that affects astaggering 640 million people worldwide…and you can actively make a difference by joiningthousands of people across the world to raise vital income to improve global health for all.”

Individuals, groups and companies can support the campaign by donating on the VAOwebsite, donating for each pair of glasses sold or eye test carried out during October, andby selling VAO World Sight Day raffle tickets. The charity is hoping to raise at least £15,000to provide practical training and skills sharing sessions to local eyecare workers in Ghana,Ethiopia and Zambia. Register at www.visionaidoverseas.org

A new evidence-based myopia ‘calculator’developed by the Brien Holden VisionInstitute (BHVI) has been created to supportclinicians in educating patients andmanaging myopia appropriately. The freeweb-based tool runs on a range of electronicdevices and merges patient informationwith different optical and pharmacologicaltreatment options to illustrate the impacton their future level of myopia. Thecalculator has been developed as part ofthe myopia management programme beingimplemented by the BHVI.

“We have a series of myopia projects toaddress a major public health problem ofour times,” said Professor PadmajaSankaridurg, head of the myopiaprogramme at the BHVI. “We’re not onlydeveloping novel optical treatments formyopia management and conductingepidemiological research, but throughprofessional development courses and aidslike the calculator, providing practitionerswith the knowledge and tools to offer their patients the best evidence-basedtreatment options.”

College of Optometrists14

New research funded by the College of Optometrists has found that introducing anintermediate-tier service (ITS) for eyecare services could reduce the volume of patientsreferred to hospitals by GPs and provide replacement services at lower costs. The research,published by BMJ Open, although not applicable across all parts of the UK, investigated thechanges in volume of hospital ophthalmology patients and the related costs, before andafter minor eye conditions services (MECS) were introduced in two London boroughs.

Michael Bowen, director of research for the College of Optometrists, said: “This is importantresearch, both for the optical sector and beyond; there is very little previous research onthe wider effects of introducing schemes such as MECS and the existing pool of data hadrecommended further investigation into the cost and benefit of these services. The resultsare encouraging, and suggest that MECS can reduce the volume of patients referred tohospital, and potentially reduce costs.”

Aston University’s Optometry School has been accredited as a new providerof the College of Optometrists’ Professional Higher Certificate in ContactLens Practice. The resulting Higher Certificate will provide optometrists withknowledge and clinical skills in the field of contact lens practice. The courseis already available at City, University of London, and Moorfields Eye Hospital/University College London.

The Professional Higher Certificate in Contact Lens Practice is part of a growingcollection of higher qualifications developed by the College in order to enableoptometrists to provide extended services in key areas such as glaucoma,low vision and medical retina. Courses are undertaken on a part-time basisand are designed to fit with life in a busy practice or hospital department.

Jackie Martin, the College’s director of education, said: “We’re delighted towelcome Aston University as a new provider for one of our accreditedhigher qualifications, enabling more optometrists to develop their skills andunderstanding in contact lens practice. We know that our higherqualifications are in demand and do enable optometrists to developprofessionally and progress in their careers.”

AN INDEPENDENT VIEW

Independents – a community

The focus of our last column, published in July,was on Specsavers’ announcement that it wasplanning to install OCT in all of its stores in thenext two years or so. As stated at the time, ourintention was to concentrate on how to get the message out to the general public thatindependents routinely offer OCT today in morethan 1,000 practices across the UK, and thatthe independent sector is the place to go forlong-term eye health care.

The challenge for AIO has been to devise a wayto bring the independent sector together as acommunity so that a common message can bewidely broadcast that optimises the resourcesof the sector in the most productive way. TheAIO council has now decided on the approach itwill take to do this, which involves streamliningthe Independents Code introduced last year andmaking it into a very public facing brand thatextols the benefits of going to an independent.

The newly streamlined code, and the brand,will focus on concise messaging about long-term eye health care, as much time as patientsneed in the consulting room chair, seeing thesame practitioner on repeat appointments,offering OCT and not trying to sell patientseyewear that they don’t need.

The detail of the new AIO initiative, what itoffers the sector, and how practices can getinvolved will be a feature at the AIO Conferencein Cambridge on 1 and 2 October. With thelaunch of three new insurance products forindependents, including an all risks practice-based PI policy, a great lecture programme andmore than 30 CET points available at very lowcost, the conference will play a part in helpingto bring the independent community together –something that is an imperative for the futurehealth of the sector.

Below are some of this year’s major innovations that we are bringing to our customers.

• Mineral free-form lensesWe at Norville have made a large investment in producing mineral free-form in the UK; the free-form designs are ideally suited to mineral materials to produce such a complex design. The mineral market in the USA and several European countries has shown to be growing. Norville will be the only producer of mineral free-form currently in the UK.

• On-the-block Automatic Spin Coating for lenses with a proprietary front surface hard coating. Allowing us to block, generate polished, hard coated, and engraved with full automation, untouched by human hand, to give a faster throughput time. This again is a fi rst for the UK market.

• Front and Back surface photochromic coatingAt Norville, we are delighted to announce that we are to be the fi rst UK laboratory to apply a photochromic application coating to the front AND back surface. We will be able to take base white lenses of all indices/polymers and turn them into brown and grey photochromic lenses. This will offer a wider range of lenses that were previously not available. You will be able to have 1.6, 1.67 and 1.74 bifocals, gradient photochromic, and specifi c darkening levels to order; just name a few examples.

• Colour-fast Coating process where problematic materials such as Trivex, 1.74 and Polycarbonate can be coloured. This front and back surface process will give a complete uniformed tint with little or no colour degradation over time; your tint will stay the same natural colour hue and density over time.

• New-state-of-the-art vacuum coating chamber installed with high end AR coating processes, including improved Bayer ratings. In the near future we will be launching Barbour Plus - an AR coating with a soft blue hue.

• Our new Mirror Coating Range has seen improved sales of mirror, up by 400% since 2015, with our new ranges, also a brand new purple mirror colour has been introduced due to customer demand.

• New range of glass mineral colour coatings in light, medium and dark grey - again, the only RX laboratory in the UK producing this.

PRACTICE MATTERS

The one thing that is similar for doctors,dentists and opticians is that in the UK, thewaiting area is almost universally thesame. Basically, it’ll be a few chairs with acoffee table with out-of-date magazines.The walls are usually plain; they mighthave a poster and if you’re lucky some toysfor small children to keep them occupied.

It is rare to find a waiting area that isstimulating and adds something to thepatient experience, be it multiple, regionalor independent. Some say that the waitingarea shouldn’t be too comfortable as itstops patients looking at the products ondisplay. But, in reality, how many peopleactually spend time looking and trying onframes while they wait?

It might be that you feel that patients don’tspend much time in your waiting area, barthe couple of minutes before theirappointment, if they are waiting for afamily member, or if you’re repairing theirframes. This time all mounts up and makesthe waiting area an integral part of theexperience of being in your practice. Buthow many of us have actually given anyreal thought to the look and feel.

With the rise in social media, every aspectof your practice can be shared instantly, somaking people feel comfortable andrelaxed is a big plus to your ratings. We allknow that the actual waiting time isimportant, but the perception of the waitalso matters, so help your patients beproactive. Offer them free Wi-Fi so they canstay connected; you might even offercharging points for their devices,particularly for patients coming in at theend of the day who may be short onbattery. Direct them to your website orhigh-quality education content, either via

tablets or screens in your waiting area. Thiscan not only improve the waitingperception, but can stimulate conversationabout your products and services.

FANCY A CUPPA? A great way to make people feel at home isto offer them a cuppa; not only does it fillwaiting time, but it can relax patients andenhance their feeling of receiving value formoney. Remember: it’s always a good ideato offer cold drinking water. You can evenuse the water cooler to make a point thatit’s important for eye health to remainhydrated. If you do offer drinks, there aretwo very important points. Firstly, maketoilets available and, secondly, alwaysmake sure the area is kept clean and tidy.There’s nothing worse than seeing dirtycups and coffee stains everywhere.

You may have space restrictions, but wherepossible don’t just put chairs in a straightline, or around the walls. Where you can,mix it up. So possibly a block of four chairs,then a set of two. It makes it less formaland patients won’t end up staring at thewalls trying to avoid eye contact. Doremember that wherever you place chairs,leave room for wheelchairs and buggies.There is nothing more stressful than hittingchairs with a wheelchair and having toapologise numerous times. Create a childfriendly zone with bean bags, and if moneypermits offer tablets with kids’ apps.Keeping children happy and occupiedrelaxes parents, children and your staff.

Moving chairs and introducing drinks arefairly cheap ideas to implement. Otherchanges may be costlier and would need tobe considered at the next shop fit. But ifyou are looking to improve the waitingarea, you need to consider colour, lighting

and furniture. To make people feel relaxed,opt for natural colours, use daylight whereyou can, and mirrors to make areas looklarger. Don’t just opt for one chair type,people are all different. Personally, a comfysofa works best, but my father-in-law, whostruggles to get out of chairs, needs a highback armchair, which can be used to leverhim up. So, think about your patients andtheir requirements. Whatever you put inyour waiting area must be easy to cleanand robust enough to last.

Technology is just part of life now, soconsider adding this to your waiting area. Itmight be having screens showingeducation information, or tablets forchildren to play games. If you offer hearingcare, have a screening device in the waitingarea. But don’t just put a TV on a wall andtune into the news, any technology addedto your practice must be considered andhave a goal in mind. Think about yourwaiting area and make it work for you. Bybeing different to everyone else, you’llstand out from the crowd, have morerelaxed patients and get the social mediacomments that you can be proud of.

Beth Ralph, a contact lens optician (CLO)from Sheffield, says: “The priority for allmy patients when it comes to lenses iscomfort first, vision next and price last.”

Beth is a busy working mum, CLO, and part-business owner at her practice Elliott &Heath Optometrists, which she shares withher husband. Like most working mums, lifefor Beth is busy and her hours are long, soshe needed a lens that could complementher busy lifestyle.

“The lenses I originally tried were souncomfortable and after four hours of wear Ihad to remove them. It was at this point thatmy husband brought home DAILIES TOTAL1 – Ihave never looked back since. In another case, I saw a patientwho is a keen sportsman and enjoys tennis.Wearing glasses compromises hisperipheral vision and gets in the way. Iimmediately suggested DAILIES TOTAL1knowing these lenses will provide him withexcellent vision and long wear. I trulybelieve anyone that wears glasses shouldhave the option to use contact lenses.

11.30AM – 2PM: FITTING WITH CONFIDENCEWhen I fit DAILIES TOTAL1, I no longer askmy patients ‘are your lenses comfortable?’ Inow say, ‘can you feel your lenses?’confident in the knowledge that they feellike nothing. Other times, I’ll put a highquality lens such as DAILIES TOTAL1 in oneeye and an alternative lens in the other andask which they prefer. Almost every time,they will choose DAILIES TOTAL1.

When I fit an existing contact lens wearerwith DAILIES TOTAL1, the response I get is‘why haven’t I been given these before?’. Inthe past, they have typically been used as asecond option or ‘the problem solver’ if theydon’t get on with their current lenses. Thisshouldn’t be happening as it’s our duty aseye care professionals to recommend thebest product for our patients.

2PM – 5PM: A RANGE OF PATIENTSYesterday an eight-year-old and her mothercame to me for advice on contact lenses forwear after school. Many parents are unawarethat contact lenses are an option for theirchildren. My only criteria for offering contactlenses to children is that they can fit them

and remove them themselves. My 11-year-old and six-year-old daughters are bothextremely active. They take part in lots ofactivities after school and I believe no childshould miss out on any activity just becausethey wear glasses. Neither need a visioncorrection yet, but should either needanything in the future, I wouldn’t hesitatewith fitting DAILIES TOTAL1.

6PM: REFLECTIONAfter a busy day tending to between 12 and14 patients, my drive home is spentreflecting on the patients I’ve seen andfeeling extremely fortunate to have the jobI have. My ultimate priority at the start ofevery working day is to improve the eyesight of each of my patients, and in turn,enhance their lives. I sometimes see a childwearing glasses with no confidence,looking at the floor, the second thoselenses are in, they are a different person.

11PM: 16 HOURS OF AMAZINGBy the time I am preparing to go to bed, Irealise I have lasted a full 16 hours withouteven thinking about my lenses or noticingthem there. No need for solutions beforebed as these get binned, just a new pair forthe next day.

A square shape is exaggerated, or a desired cat-eyesilhouette is overpowering. Eyewear designers and internationalbrands understand the challenges. They are resolving theseframe obstacles for the independent optician and their patients,with intriguing designs that are trendy, stylish and chic, and aperfect fit for smaller face types.
