Dear Service Providers,Attached is your registration form for The Gold Coast Wellness Festival. Registrations are open to not-for-profit, non-commercial enterprises and other services that promote wellness. We encourage you to take part and provide activities and information to the Gold Coast Community.

The aim of the Wellness Festival is to promote mental and physical wellness within the Gold Coast Community during Mental Health Week; “Be Active, Get Connected, Stay Involved.”

Invitations are open to agencies, groups and organisations who cater for the well-being of the families and residents of the Gold Coast. There are internal and external sites available. Agencies and groups wanting to participate need to be able to provide staff for the sites.

To truly capture the theme of the event all stallholders are asked to decorate their tables/stalls in a bright & colourful manner to reflect the theme of the festival - health, positivity & community well-being. This can be in the form of flowers/greenery on the tables, a table top water feature, aromatherapy, bright tablecloths, balloons, pot plants, a bowl of sweets, a basket of giveaways, stall holders in multi coloured hats, decorations and any other wonderful ideas you may have.

The Gold Coast Wellness Festival aims to promote well-being and highlight services and groups available to the Gold Coast Community. There will be entertainment, food, and activities. The Wellness Festival will be celebrated during Mental Health Week. The public are invited to join in and celebrate from 9am to 2pm on Saturday 11th October.

Terms and ConditionsApplicants: Please ensure you have read and signed the Terms and Conditions prior to completing and returning this form.DefinitionsStall Holder: means the holder of a permit granted by FSG AustraliaSite Allocation: means the site allocated to the Stall holder by the FSG AustraliaFestival: means the Gold Coast Wellness Festival 2014Festival Venue: Old Burleigh Road, Broadbeach QLD 4218

FSG Australia Obligations1. FSG Australia takes no responsibility for damage, loss or theft of

property or motor vehicles. 2. The Festival operating times are Saturday 11th October 2014

from 9:00am to 2:00pm. 3. FSG Australia reserves the right to change the date and operating

times.4. FSG Australia is not responsible for any loss or damage in the

event that it exercises its right to change the date and operating times of the Festival.

5. FSG Australia reserves the right to determine and direct the site allocation. The stall holders accept the decision of FSG Australia, in its absolute discretion, is final and binding.

6. Employees of FSG Australia may request the adherence to or may enforce any of the terms and conditions of this Registration Agreement.

7. Not every Registration Application will necessarily receive a stall at the Festival. The ultimate decision in relation to whom stalls are allocated to is at the sole discretion of FSG Australia and no cor-respondence will be entered into relating to the decision process or the outcome.

8. FSG Australia reserves the right, at any time, to verify the validity of registration applications and to disqualify any registration appli-cation that is not in accordance with these Terms and Conditions.

9. By submitting a registration application, applicants including suc-cessful applicants consent to FSG Australia using and disclosing the information provided for the purposes of conducting the Fes-tival, reviewing, processing and awarding the stalls to applicants and any other matter connected to or incidental to the Festival.

10. FSG Australia may request to use photos and quotes from Stall Holders which will become the property of FSG Australia, who may use them for marketing and promotional purposes, as agreed.

11. The Festival can be suspended or terminated at any time with-out notice. These Terms and Conditions may be amended or withdrawn at the discretion of FSG Australia. FSG Australia will publish any revised conditions at www.fsg.org.au. Applicants and Stall Holders should visit the website from time to time to review the current and effective Terms and Conditions.

Stall Holder’s Obligations12. Stall Holders are responsible for ensuring that they comply with

all State and Federal Occupational, Workplace, Health and Safety Regulations, Australian Standards and other relevant acts, rules and regulations applicable to their occupation, display and exhi-bition as a stall holder including but not limited to all appropriate testing, tagging and use of gas & electricity.

13. The Stall Holders understand and accept that FSG Australia do not provide security outside operating hours and therefore stall holders must provide their own. Furthermore Stall Holders agree that FSG Australia are not responsible for any losses that occur as a result of the occupation of their stall during or outside busi-ness hours.

14. Stall Holders are responsible for any damage they may cause directly or indirectly to the Festival Venue.

15. Stall Holder applicants must have Broadform (public and products) liability insurance for an amount no less than $10 million for any one event, covering their activities.

A Certificate of Currency noting FSG Australia as an interested party and noting the Gold Coast Wellness Festival is covered under the Stall Holder applicant’s insurance policy must accompany this registration application and FSG Australia will keep a copy on file.

16. Sites must be left in a clean and tidy condition on departure. Any

cost incurred by FSG Australia for the removal of any rubbish left on site will result in the charge being passed on to the Stall Holder.

17. Stall Holders are not permitted to share, sub lease or assign a site with other operators, or any other party without the prior written approval of FSG Australia.

18. The Applicant or Stall Holder shall not do or say anything or cause anyone to do or say anything that may prejudice, or be detrimental to or cause damage to the name and reputation of the Festival or FSG Australia.

19. The Stall Holder will notify FSG Australia in writing immediately upon the need to cancel their participation in the Festival.

20. Stall Holders are restricted to offering for sale only what is permit-ted by the prior written approval from FSG Australia.

Indemnity21. The Stall Holder hereby indemnifies and will keep indemnified,

FSG Australia against any liability, action, claim, demand, costs, charges or expenses, arising from any accident, loss, damage or injury to person or property in connection with or arising from any act, omission, negligence or thing done or omitted by such Stall Holder or any servant, representative, agent, invitee, volunteer, licensee of their or any other person or persons under their direc-tion or control.

Hours of Operation22. The Festival Venue will be available to Stall Holders for set-up

from 7:00am - 8:30am Saturday 11th October. 23. All Stall Holders must be set up and prepared for operation no

later than 30 minutes before commencement of the Festival oper-ating times unless prior arrangements are made.

24. Stall Holders must cease operating at 2.00pm.25. Stall Holders must have a staff member in attendance and

keep operating for the duration of the Festival operating times. Intention to depart prior to these times must be approved by FSG Australia prior to the Festival commencing.

26. The Festival Venue will be available to Stall Holders for pack-up from 2:00pm to 3:30pm Saturday 11th October.

Vehicles and Road Safety27. Setting Up - Vehicles will be allowed onto the site from 6:30am

to 8:00am on Saturday 11th October, however must be removed from within the Festival area by 8:00am on the day of the Festival. All vehicles are to be removed.

28. Packing Up - Vehicles will be allowed back into the area at 2.00pm or when defined safe by FSG Australia, and must be removed from within the Festival site within 1.5 hours.

Termination of Participation in the Festival29. FSG Australia may terminate the Stall Holder’s participation

forthwith where the Stall Holder breaches any of the terms and conditions of this agreement and may require the Stall Holder to remove its products and services immediately. The applicant agrees that FSG Australia will be entitled to remove the Stall Holder’s products from the allocated site at the operators cost if the Stall Holder does not remove such products when required by the FSG Australia.
