Walking Field Trip - Permission Slip

My son/daughter,________________________________, has permission

to walk from Cuba Schools to the History Museum located at 308 N.

Smith Street in Cuba, Missouri on May 18, 2015 and to the Ozarks

Regional Library in Cuba, Missouri on May 20, 2015. I understand that

my child will be stopping at various points to observe the local murals

along the way and that my child will be walking approximately 2 miles

round trip on both days. My child has permission to participate in all

activities planned for these two days.

Please note that students will be back before lunch and that the activity

may be canceled in the event of rain. Students not attending the field

trip will be assigned to a teacher who is not participating in the event

and all classes will resume normal schedule upon return to school. In

addition, as this is a curriculum field trip and an assignment will be

given. Any student not participating will be given an alternate

assignment covering similar material. If you have any questions, please

signature on this form as noted above. This form must be returned by

May 1, 2015.

Organization and Cuba’s Historical Society. We sincerely appreciate

their willingness to share our town’s history with the students.
