　　　手術間に手洗い施行している麻酔科医➡ 36%

医療従事者の内、　感染症陽性患者接触時➡ 95% が手洗い施行　感染症不明患者接触時➡ 58% が手洗い施行

(Tait and Tuttle, 1995)　　　 PACU 新規患者入室時➡ 20% が手洗い施行

　➡手洗いの重要性は医療者に理解されているが、　　　実践されていない。

手指衛生②

手袋の使用　　使い捨て手袋もサージカルグラブも指先の感覚　を低下させる。　　　　 (Tiefenthaler et al, 2006)　　　 Extra thin サージカルグラブは他の手袋と比較し　指先の感覚を上昇させる。　 (Kopka et al, 2005)

➡ 手袋使用で確かに指先の感覚低下は認められる。

手指衛生②

➡CDC ・ ASA ・ OSHA は感染予防の為にも手袋使用を　スタンダードプリコーションに組み入れている。

手指衛生は手洗いと手袋の使用からなる

手洗いの重要性は医療者に理解されているが、残念ながら確実に実践されているとは言えない

手袋の使用はデメリットもあるが、メリットの方が大きい

CDC recommendations for activities that require handwashing

1. Decontaminate hands before having direct contact with patients2. Decontaminate hands before donning sterile gloves when inserting a central

intravascular catheter.3. Decontaminate hands before inserting indwelling urinary catheters, peripheral

vascular catheters, or other invasive devices that do not require a surgical procedure. 4. Decontaminate hands after contact with a patient’s skin. (e.g. when taking a pulse or

blood pressure, of lifting patient)5. Decontaminate hands after contact with body fluids or excretions, mucous

membranes, nonintact skin, and wound dressings even if hands are not visibly soiled.6. Decontaminate hands if moving from a contaminated body site to a clean body sire

during patient care.7. Decontaminate hands after contact with inanimate objects (including medical

equipment) in the immediate vicinity of the patient.8. Decontaminate hands after removing gloves.9. Before eating and after using a restroom, wash hands with a nonantimicrobial soap

and water or with an antimicrobial soap and water.

Summary of recommendations of the ASA for hand hygiene

1. Wash hands before performing mvasive procedures.2. Wash hands after touching blood, body fluids, secretions, excretions, and

contaminated items, whether or not gloves are worn.3. Wash hands immediately after gloves are removed, between patient contacts, and

when otherwise indicated to avoid transfer of micro oraganisms to other patients or environments.

4. It may be necessary to wash hands between tasks and procedures on the same patient to prevent cross contamination of different body sites.

5. Waterless handwashing solutions may be considered when the anesthesiologist cannot leave the room.

6. Appropriate barrier precautions such as gloves, fluid resistant masks, face shields, and gowns must be used routinely with all patients.

7. Remove gloves and gowns promptly after use, before touching noncontaminated items and environmental surfaces, and wash hands before seeing another patient.
