1988-present, Attending Physician, San Francisco, CA, UCSF Medical Center: I attend 16 weeks out of the year on the transplant services. I have been doing this for the past 23 years. I have performed more than 500 deceased donor transplants (1988-present) and more than 150 living donor liver transplants (1997-present).

SUMMARY OF CLINICAL ACTIVITIES

My clinical activities have been related to the development of the transplant service. The program that I have taken direct responsibility for is the living donor liver transplant program. This high visibility program presents significant risks and benefits to the donor, recipient, and the institution. With a cautious approach, our program now has done a large number of the operations. Our results in this program are arguably the best in the world. Currently we are a large contributor of patients to the NIH funded Adult to Adult Living Donor Liver Transplant Study group.

SUMMARY OF SERVICE ACTIVITIES

My service activities are mainly focused on the care of the transplant patient. At the hospital and interdisciplinary levels, I have taken a leadership role in the complex issues related to administration of the largest abdominal transplant service in the United States, if not in the world. I have concentrated my expertise on selection of the ideal patients for transplantation, quality assurance, perioperative efficiency, service line integration, billing and contracting, appropriate transfer and discharge of our organ transplant patients, and working with our hepatology and nephrology physicians in an integrated fashion to provide optimal patient care.

At the Department/division level my major goals have been to provide mentorship for the transplant faculty, both in clinical and non-clinical areas. I have provided direction and support for research focus for each of the faculty. Currently, the Division has the most NIH funding of any Division in the Department of Surgery. The clinical activity of the Division has been outstanding, both in terms of quality and quantity. I also head the Merit and Promotions Committee and have participated in key search committees for Department chairs.

On a more global basis, I've worked with the California Transplant Donor Network as a Board member to improve organ donation, UNOS/OPTN as a board member, and then President to improve organ donation and allocation within the United States, and I've worked with advising state legislators and national legislative bodies regarding concern for recipient insurance

TEACHING NARRATIVE

My teaching responsibilities are directed toward medical students, residents, fellows and junior faculty.

For the medical students I have taught a class for 26 years on the clinical management of fluid and electrolytes. This small session class has received praise from the students. In addition, the medical students rotate on the transplant services where I am involved in their ward teaching. I have also mentored senior students in research projects.

For the residents and fellows, I provide teaching in the operating room and on the ward.

As a transplantation surgeon in a complex field, I have been assisting my faculty in both their clinical and research efforts. This has included helping them further new operative techniques and providing advice regarding clinical issues. I have been assisting the junior faculty in obtaining NIH funding, placing them in key situations on the national transplantation scene, and at the same time giving them time to take advantage of the opportunities.

MENTORING NARRATIVE

I have mentored medical students, residents, and fellows. Most recently, I mentored Mariya Samoylov, a University of California San Francisco medical student. We collaborated on studies of hepatocellular carcinoma in patients awaiting liver transplantation. Mariya was the first author on a paper recently published in the journal Liver Transplantation and we are waiting editorial decision on our recently submitted revisions to a second paper. In addition providing guidance and her research, I have also provided guidance and her career choices.

I have mentored many of the transplant fellows here at University of California San Francisco. Our senior fellow, Garrett Roll, collaborated with me on several papers that have either been published or submitted. I also helped Garrett with his surgery post fellowship position and help him find the position in Birmingham England.

RESEARCH PROGRAM

Public Policy

Organ allocation policies for patients awaiting transplantation have been a source of great controversy as they affect patients' lives, transplant centers and the government. Currently, a government contractor, the United Network for Organ Sharing (UNOS) manages the contract for the OPTN, the entity that determines organ allocation policies. The OPTN has been essentially responsible for organ allocation since 1988 and has develops allocation and distribution policies based upon input from its committees. I served on several different UNOS committees and as Chair of the Liver Committee from 1993 to 1995.

Prior to 1995, decisions about creation and modification of policy were based upon trial and error methods, where proposed policies were implemented without any method of predicting in advance the effect of the policy on organ allocation. Because of the uncertainty of predicting the effect of policy changes, it became paramount to develop a computer model for organ allocation.

Predictive models can be created if the events in question can be described from past experience. Because organ allocation is the matching the organs to the patients, the variables that describe the patients and the organ allow the creation of a model. Patients who need transplants can be described in terms of their degree of illness, age, the transplant center they are listed with, and many other variables. Organ donors can be described by similar information.

By using the descriptions of patients and organs, a computer model can be created to predict the effect of the change in organ allocation policy upon the national use of the organs. UNOS has now implemented this system for organ allocation.

I have continued my interest in the creation of public policy regarding organ transplantation. I served as the chairman of the Scientific Advisory committee for the Scientific Registry of Transplant Recipients (SRTR). This committee directs the formation of policy regarding the study of outcomes of patients on the waiting list and after transplantation

In collaboration with the principals at the SRTR, I wrote the paper regarding correcting racial disparities in organ allocation that was the lead article in the NEJM. The findings of this study resulted in a significant change in the manner that kidneys are allocated for transplantation in the US.

My research continues to focus on improving organ allocation.

Hepatocellular Carcinoma

Liver transplantation has become a mainstay in the management of many patients with hepatocellular carcinoma. In collaboration with Dr. Francis Yao, I have published on the selection of patients for transplantation, the management of patients on the waiting list, and the post-transplant outcomes. This work has been recognized internationally in the debate about which patients should be selected for transplantation. The "UCSF Criteria" have been debated as a replacement for the current criteria, "Milan Criteria." Because of the practice of naming of criteria for the center where they were developed, "UCSF" has been discussed extensively in this literature and in public debate

This paper was the lead article in the New England Journal of Medicine. It caused a change in organ allocation policy that has proven to be beneficial. This was a collaborative paper between the University of Nebraska and UCSF. This was a demonstration of a protective value of portacaval shunt allowing the use of smaller grafts for living donor liver transplantation. The smaller graft sizes can be obtained from the donor at lower donor mortality and morbidity risk. I conceived of the goal to combine the centers' data, and I helped with the data analysis and manuscript preparation.
