Instructions for returning the questionnaire:

There are two ways you can send this back to me.

First way: 1. Complete the questionnaire and save it with a new name, noting the

location (folder) it is saved in. Then,2. Open a new email addressed [email protected], find where it

says something like “attach,” and click it. Then,3. Remembering the location where the questionnaire was saved, go

there and attach the file. That’s it!

Second way:After completing the questionnaire, use the copy and paste commands to copy and paste the entire contents of the questionnaire into a new email addressed to [email protected].

Please let me know if you have any questions about returning the questionnaire, and please remember to also send a very recent photograph of the animal you are wanting me to work with, making sure that she or he is facing the camera.
