The Children’s Developmental Services Agency is the lead agency for the North Carolina Infant Toddler Program serving children birth to 3, who have developmental delays or at risk for delays due to an established condition. Services include Early Intervention Service Coordination, specialized therapies including Speech, Physical and Occupational therapy and transition support for children exiting the Infant Toddler Program. Chatham Trades uses referral information and real work (both in its facility and in the community) to determine each individual’s vocational interests and readiness for work. Financial Assistance

Cardinal Innovations Healthcare Solutions is the local management entity / managed care organization for the state-funded mental health, substance abuse, and developmental disabilities services for Chatham County & 9 other counties in north central NC. In April 2005, the N.C. Department of Health and Human Services (DHHS) began changing how Medicaid-funded services for mental health, substance abuse, and development disabilities were provided.

Individual Support -Mental Health for Adults (18 & Older): Medicaid billable; Effective 04-01-07: Individual Support Services are “hands-on” individualized assistance with everyday activities that are required by an individual with severe and persistent mental illness in order to live independently in the community. The services are provided for adults ages eighteen (18) and older and are intended to support a person in a private home or licensed group home that have a documented transition plan and want to move to independent housing. Individual Support can be billed while the person is living in the licensed facility or private home up to six (6) months prior to moving. Specifically, this service provides assistance with Instrumental Activities of Daily Living (IADL) including preparing meals, managing money, shopping for household necessities, using the telephone, housecleaning, laundry, transporting the individual to access the community, medication management, supervision and cuing. The goal is to provide coaching to the individual in areas of need and fade this support over time.

Service Type/Setting: Individual Support is a periodic service. It is intended to support an individual who is living in an independent residence, or who is transitioning to independent housing from a licensed residential facility within the next two (2) months. This service is available to those residing in non-licensed supported housing where there is not twenty-four (24) hour staffing. This service is not available to those living in licensed adult care homes or other licensed community based residential settings, or who are maintained within a family residence except for two (2) months prior to moving to independent housing.

Program Requirements: Individual Support is a one on one service provided directly to the individual. The service must be reflected in a Service plan based on an Individual Support Planning process that reflects the strengths, needs and preferences of the person served. The goals incorporated into the Service Plan must justify the hours requested, and must include a step-down plan which identifies and utilizes natural supports.

Entrance CriteriaAdults eligible for this service must be age eighteen (18) or older, must have a diagnosis ofSevere and Persistent Mental Illness (SPMI), and must demonstrate a deficit in at least oneInstrumental Activity of Daily Living (IADL).

NC Innovations is a means of funding services and supports for people with various developmental disabilities who are at risk for institutional care in an Intermediate Care Facility for Individuals with an Intellectual Disability / Developmental Disability. Innovations will also provide funding for people to return to their homes and communities from ICF-MRs.”

Process for NC Innovations Waiver Services (formerly CAP-I/DD): For Orange-Person-Chatham Counties (OPC), call 1-800-233-6834, the Cardinal Innovations Healthcare Solutions Access line and ask to speak to someone about getting on the Registry of Unmet Needs. The screening may be done over the phone, taking 30 minutes to an hour, and arrangements may be made for someone to return your call. If the individual meets the criteria, their name is placed on the registry (formerly referred to as waiting list.) People are tracked on the registry according to time waiting or the date they are placed on the registry. It will be helpful in the process to have as much documentation of your family members' Developmental Disability as possible, including the most recent psychological evaluation that has been done. If your family member has Medicaid, there may be other services that they are eligible to receive. Please ask the DD Specialist you talk to about available B-3 services such as respite.

CAP-DA: (Community Alternatives Program for Disabled Adults) Begins at Age 18+The Chatham County Public Health Department is the contact agency: 919-742-5641 Ext 8267 (Rebecca Blalock). They provide a package of services to allow adults age 18 & older who qualify for nursing facility care to remain in their private residences. CAP/DA is an extension of Medicaid, and is intended for individuals who have very limited financial resources. Eligibility for CAP/DA differs from regular Medicaid in several important ways, however. Most significantly, CAP/DA eligibility is determined by the income and assets of the client only (not the spouse or other family members). For those who qualify, CAP/DA provides extensive home and community-based services, which may include some services not covered by regular Medicaid, such as: 1) adult day programs, 2) more extensive home care (but not 24-hour care), and 3) supplies such as incontinence products and nutritional supplements. Information required to complete the application includes the client’s Medicaid number (if already on Medicaid), Social Security number, monthly income, primary physician’s name and phone number, and client’s diagnosis, as well as information about their needs for assistance.Once this information has been provided, the client is placed on the CAP/DA waiting list. The wait may be as long as 18-24 months – call as soon as possible. The waiting list is first-come, first-served. No exceptions are made based on the severity of need. The only possible exception is when the client has already been residing in a nursing home for at least 3 continuous months and wants to return to a community setting; such clients may be given higher priority based on a different funding source. Initially, the application is reviewed for financial eligibility. A physician must also complete paperwork indicating that the client is at a nursing home level of care. Then, if the client qualifies financially, care needs are evaluated through a home visit. A physician must also complete paperwork indicating that the client is at a nursing home level of care. for CAP/DA services, the client enters the program. A case manager is assigned to assist the client in getting the services they need and monitoring the care that is provided as needs change.

Provides case management with limited financial aid, emergency assistance, referrals and other necessary support; and 3) Young Parents Demonstration Project which exposes young parents, parents-to-be- to training that helps to discover & develop abilities, skills, and interests. Will ensure that parents are more equipped to meet the demands of the workplace and of being a parent.

Medicaid: A joint state-federal program that pays for a wide range of health care needs for those who are over 65, or disabled, or blind; children; caretaker relatives of children; and pregnant women. Applications are taken at the Department of Social Services, (919) 542-2759. Decisions on applications should be made within 45 to 90 days.

Medicaid for the Aged, Blind, and Disabled: Services generally provided for a nominal co-payment include 1) hospital care, physicians care, & prescription drugs. There are no co-pays for recipients under the age of 21, for recipients who are in intermediate care or skilled nursing facilities, recipients with developmental disabilities who are in an assisted living facility or group homes. Services covered may include home health services, personal care services, hearing aid services, clinic services, dental care, medical transportation, mental health care, speech or physical therapy, or other needed therapy, durable medical equipment, and hospice care, etc.

Medicare: A federal health insurance program for individuals who are age 65 and over, those under the age of 65 with certain disabilities or ALS (amyotrophic lateral sclerosis, or Lou Gehrig’s disease), and a person of any age with End-Stage Renal Disease (permanent kidney failure requiring dialysis or a kidney transplant). The State Department of Insurance operates The Seniors’ Health Insurance Information Program (SHIIP) to provide counseling and guidance to senior citizens about Medigap and other Medicare products and benefits. Supplemental Security Income (SSI): A federal program that provides monthly cash payment to supplement the income of those who are aged, visually impaired, or disabled. Applications are taken at the Social Security Administration offices & it is recommended that an applicant call 1-800-772-1231 or 1-877-319-5774 first to make an appointment. The date of the initial phone call is the application date & is important because benefits are

prorated back to the date of application. Processing can take months or may be delayed due to missing information. Frequent checking can help. Make written notes of your phone calls.

Program Description: READD Center stands for Relaxing Environment for Adults with Developmental Disabilities. The facility is located 7 miles outside of Siler City in Mt. Vernon Springs. This day program was created to give DD adults a structured program which the clients have specific goals on the Plan of Care. Crafts, games, outings, parties, and part-time compensatory education classes are taught by a qualified CCCC instructor, are available. The program provides daily snacks as well as a full lunch. This program is free but the client must have CAP services to attend the program.
