How Will You Be Successful? Read and understand the 2018 DPRP Standards.

Have the capacity and commitment to deliver the program for at least 1 year (at least 16 weekly sessions in the first 6 months and at least 6 monthly sessions in the last 6 months).

▪ Appendix A of the 2018 DPRP Standards: Organizational Capacity AssessmentNationalDPPCSC.cdc.gov – enter “Organizational Capacity Assessment”

Begin offering the lifestyle program within 6 months of your effective date.

Enroll enough eligible participants to ensure you meet minimum participant requirements each 6 months.

Delivery Modes-In-person. Yearlong lifestyle change program delivered 100% in-person for all participants by trained Lifestyle Coaches, meaning participants are physically present in a classroom or classroom-like setting. Lifestyle Coaches may supplement in-person sessions with handouts, emails, or reminder texts, although none of these may be the sole method of participant communication.

Online. Yearlong lifestyle change program delivered 100% online for all participants, meaning participants log into course sessions via a computer, laptop, tablet, or smart phone. Participants must interact with trained Lifestyle Coaches at various times and can interact using various communication methods, including online classes, emails, phone calls, or texts.

Distance learning. Yearlong lifestyle change program delivered 100% by trained Lifestyle Coaches via remote classroom or telehealth (i.e., conference call or Skype) where the Lifestyle Coach is present in one location and participants are calling in or video-conferencing from another location.

Combination. Yearlong lifestyle change program delivered as a combination of any of the above defined delivery modes for all participants by trained Lifestyle Coaches.

Participant Eligibility Criteria Must be 18 years of age or older

Must not be pregnant at time of enrollment

Must have a body mass index (BMI) of ≥25 kg/m2 (≥23 kg/m2 if Asian American)

Must not have a previous diagnosis of type 1 or type 2 diabetes prior to enrollment

Must be considered eligible based on either –

▪ A recent (within the past year) blood glucose test (may be self-reported for CDC recognition purposes)

▪ A positive screening for prediabetes based on the American Diabetes Association/CDC Prediabetes Risk Test

▪ Clinically diagnosed gestational diabetes mellitus (GDM) during a previous pregnancy

▪ Reminder: For Medicare Diabetes Prevention Program (MDPP) suppliers, a self-reported screening, blood test, or GDM are not permitted.

Important things to note once approved: Your approval date: the date the application is approved

▪ Classes and data collection may begin on or after the approval date and must begin within 6 months following the effective date.

Your effective date: the first day of the month following the approval date (This date begins Sequence 1.)

Your individualized data submission and evaluation timeline

▪ Each organization will submit data once every 6 months starting from the effective date.

▪ You may submit data at any time during your submission month.

The DPRP awards three categories of recognition:

Pending Preliminary Full

Pending RecognitionMust meet Standards requirements 1-4:

▪ Requirement 1: Submit an application for recognition.

▪ Requirement 2: Use a CDC-approved curriculum.

▪ Requirement 3: Agree to the intervention duration (12 months).

▪ Requirement 4: Agree to the intervention intensity (Minimum of 16 sessions delivered approximately once per week during months 1-6, followed by a minimum of 6 sessions delivered approximately once per month during months 7-12).

May remain in Pending status for up to 36 months

▪ Can achieve Preliminary during this period

▪ Can achieve Full during this period without going through Preliminary first

▪ Failure to achieve Preliminary or Full recognition during this period will result in a loss of recognition and a mandatory 6 month waiting period before reapplying.

Preliminary RecognitionRequired to become a new MDPP supplier, if not already in Full recognition

Must have been in Pending recognition status for at least 12 months

Must have submitted data on at least one completed class

Must meet Requirement 5 in the DPRP Standards:

▪ Based on eligible participants who attended at least 3 sessions in the first six months and whose time from first session attended to last session attended was at least 9 months; must have a minimum of 5 participants who meet these criteria

▪ Of those participants who meet the above criteria, at least 60% must have attended at least 9 sessions in months 1-6, and at least 60% must have attended at least 3 sessions in months 7-12

Preliminary RecognitionMay remain in Preliminary status for up to 24 months, provided requirements for Preliminary recognition are again met at 12 months after initially achieving Preliminary, or be withdrawn and wait 6 months before reapplying.

Must submit the required data every 6 months

Must achieve Full recognition within 24 months or be withdrawn and wait 6 months before reapplying

Full RecognitionRequired to become an MDPP supplier after the 24 months of Preliminary expires

Must have been in Pending status for at least 12 months

Must have submitted data on at least one completed class

Must meet requirements 5-9 in the DPRP Standards:

▪ Based on eligible participants who attended at least 3 sessions in the first six months and whose time from first session attended to last session attended was at least 9 months; must have a minimum of 5 participants who meet these criteria

Full RecognitionRequirement 6: The evaluated, yearlong cohort of participants must have body weight documented during at least 80% of the sessions.

Requirement 7: A evaluated, yearlong cohort of participants must have physical activity minutes documented during at least 60% of the sessions. Note: Zero recorded minutes does not count as documented minutes.

Requirement 8: The average weight loss across all participants in the evaluated, yearlong cohort must be a minimum of 5% of “starting” body weight.

Requirement 9: A minimum of 35% of all participants in a cohort must be eligible for the lifestyle change program based on either a blood test indicating prediabetes or a history of GDM.

Reminder: For MDPP suppliers, a self-reported screening, blood test, or GDM are not permitted.

Full RecognitionMay remain in Full recognition status for up to 24 months without meeting each requirement for Full

Must submit the required data every 6 months

Must re-achieve Full recognition within 24 months

Can remain in Full on a Corrective Action Plan for an additional 12 months (beyond the 24) if requirements for Preliminary are met

Must be withdrawn and will need to wait 6 months before reapplying if requirements for Preliminary are not met 24 months after initially achieving Full

Reporting of Data to the DPRPAll organizations must make a data submission every 6 months according to their data submission schedule.

Organizations will receive a report after each data submission.

▪ Progress report if a full cohort(s) of data is not available

▪ Evaluation report if a full cohort(s) of data is available

▸ Full evaluation on complete cohort(s)

▸ Progress on ongoing participants (those who began 7-12 months before evaluation)

▸ Progress on new participants (those who began 1-6 months before evaluation)

All reports evaluate program results under the current Standards.

Data: Enables us to monitor progress individually by program and across the nation

Sustainability/Reimbursement: Private and public payers reimbursing for the program are requiring CDC recognition.

Support: Recognized programs have access to technical assistance, training, and resources.
