Members of Campus Recreation.
Recreation Facilities are Age Restricted
To use the Recreation Center, patrons must be at least age 18+. To use the Boyden Gymnasium, patrons must be at least age 15+. To use Boyden and Hicks pools, patrons must be at least age 6+. To use Boyden Handball/Squash Courts patrons must be at least age 6+. To use the Mullins Tennis Courts patrons must be at least age 6+.
Please Note: Patrons ages 6 – 17 MUST be accompanied and supervised by an adult sponsor with recreation membership or guest pass.

MEMBERSHIP APPLICATION
In applying for membership I represent and certify that I am 18 years of age or older and all facts and information set forth on the Membership Application are true, correct and complete.
By becoming a member of Campus Recreation, I agree to these Terms & Conditions.
MEMBERSHIP PAYMENT
I agree to pay the total due upon signing the Membership Application. I understand that Membership purchase is non-refundable.
MEMBERSHIP TERM
All memberships shall be on a semester to semester basis commencing on the date of membership application and full payment.
Memberships may be purchased by individual semester, two semesters at a time or an entire year consisting of three semesters. (Fall, Spring & Summer)
Membership begins at the beginning of each academic year and ends on the last day of the University’s summer vacation period.
Membership purchase for future academic years is not allowed.
MEMBERSHIP PRIVILEGES
Membership is non-transferable and I will use my University issued UCard or UCard Badge. Any attempt to transfer membership shall make the membership null and void.
MEMBER’S RESPONSIBILITY
All use of the Recreational Facilities and its programs shall be undertaken at the sole risk of the member.
It is specifically agreed that Campus Recreation or its employees shall not be responsible or liable for articles lost, damages or stolen in or about or in connection with the Department of Recreation nor for loss or damage to any member property, including automobiles and their contents.
It is also agreed that any damages to the Campus Recreation Facilities, its staff, or the facilities and its contents or property, or the property of any member by another member is the sole responsibility of the offending member.
I acknowledge that the Department of Campus Recreation operates under rules and regulations established for the safety, comfort and protection of members or other patrons and I will abide by and be bound by all posted rules and regulations as well as rules and regulations subsequently approved and posted or published by Campus Recreation.
GENERAL PROVISIONS
I acknowledge that no promises, representations or warranties have been made to me by any employee, agent or otherwise about the Recreation facilities and its programs.

I acknowledge that it has been explained to me that the Membership application is a legally binding and enforceable contract and that I have agreed to maintain my membership for the period of time specified.
The Department of Campus Recreation may for any reason, in its sole and absolute discretion, cancel my membership at any time.
2019 / 2020 MEMBERSHIP TERMS & CONDITIONS GENERAL POLICIES & PROCEDURES
ACCESS POLICIES Member must present their valid UCard for entry to all recreation facilities. NO UCARD = NO ENTRY. Member’s UCard is non-transferable and for the exclusive use of the person named on the card. Member must possess and display upon request appropriate identification. Campus Recreation reserves the right to request identification at any time. Non-Members, suspended members and expired members will NOT be permitted access to recreation facilities. ID Misuse and/or improper entry is subject to disciplinary action, including
immediate removal and suspension from Campus Recreation facilities and further University discipline.
Associate members and guests must be at least age 18 to use the Recreation Center. Guests entering recreation facilities must present valid guest pass.
ALCHOL/DRUGS/SMOKING
Campus Recreation facilities are substance-free facilities. ANIMALS
Only service animals, defined as any guide dog, service dog or other animal individually trained to provide assistance to a person with a disability, are permitted in recreation facilities.
The owner should properly display the animal’s credentials. ATTIRE Exercise attire and footwear is required. Footwear: closed-toed and closed-heeled shoes, non-marking, rubber-soled athletic-type shoes. Shirts and upper body clothing should cover the entire torso and sides. Cut off t-shirts, cropped tanks, halter tops and uncovered sports bras are not permitted.

Athletic-type bottoms are required. Shorts must be long enough to cover the buttocks and groin when exercising or moving and must be worn properly on the hips.
Jeans, khakis and pants with buttons, metal zippers, metal rivets, belts and belt loops are not permitted.
Campus Recreation strongly recommends that no jewelry be worn during participation.
Clothing with offensive or profane language, designs or pictures is not acceptable.
Campus Recreation decisions regarding appropriate attire is final.
Gloves and/or hand wraps are to be worn for speed bags and heavy bags in the boxing area.
CELL PHONES
Cell phone usage is not permitted beyond recreation facility access turnstiles or entrances.
CLOSING TIME
Activity areas close 15 minutes prior to the facility closing time.
All areas, including locker rooms, should be vacated by closing time.
CLOSURES / INCLEMENT WEATHER Campus Recreation typically follows the University schedule with regard to closures due to inclement weather. For information on Campus Recreation facility hours and alerts go to our website at www.umass.edu/campusrec or call 413-545-0022. Campus Recreation facilities/facility areas may be closed or restricted without prior notice due to special events, scheduled cleaning, maintenance and/or unforeseen circumstances. CONDUCT Use of Campus Recreation facilities is a privilege; participants are expected to respect the rights of others. Members and guests are expected to display appropriate behavior while participating. Campus Recreation reserves the right to refuse access to or remove any individual whose behavior is inappropriate or in violation of University rules and regulations.

EQUIPMENT CHECKOUT Equipment available for checkout at no additional cost
includes balls, combination locks and shower towels. UCard is required to check out equipment. Participant is responsible for all issued items.
FACILITY USE
Campus Recreation facilities may not be used for private or commercial purposes unless approved by Campus Recreation (i.e. personal training, private instruction, sales and/or solicitation).
FOOD & BEVERAGE
Food and gum are NOT permitted in recreation facilities. Water, in a sealable and unbreakable plastic container, is the only beverage permitted Glass containers are prohibited. Containers are subject to substance check by Campus Recreation
GYMNASIUM Gymnasium courts are for basketball, volleyball and badminton. All other activities are subject to approval by Campus Recreation. GUESTS Members may host a non-member by purchasing a guest pass. Guest passes are available at a daily rate of $10.00 per guest/visit. Members may host a maximum of 2 guests per day. Age restrictions apply. The sponsoring member MUST accompany the guest at the time of purchase and WHEN USING RECREATION FACILITIES.
Guest must show valid photo ID prior to purchasing a guest pass.
PERSONAL BELONGINGS Campus Recreation strongly encourages participants to secure all personal items or leave
personal items at home. Campus Recreation is not responsible for lost, stolen, damaged or unattended personal
belongings. Locks are available for day use at equipment/towel counters.
PHOTOGRAPHY / VIDEOGRAPHY
Photography and/or videography is not permitted without prior campus recreation approval. VIDEO SURVELIANCE
Activity areas are subject to 24-hour surveillance. Live video is randomly monitored by Campus Recreation and UMass Police.

GENERAL Members are responsible for reading, understanding and abiding by all facility rules and
regulations. Not all rules are posted in immediate areas. If a staff member requires cooperation, please
adjust behavior accordingly. Areas and equipment within Campus Recreation facilities shall be used for intended purposes
only. Participants using recreation facilities and equipment assume the liability of and agree to
compensate campus recreation for any damage other than normal wear and tear. Campus Recreation reserves the right to render decisions on situations not specifically
covered in the rules. Failure to comply with staff instructions and/or improper facility use may result in loss of facility
and/or membership privileges. Proper ID (for both member and guest) is required for entry to ALL recreation facilities.
Unauthorized use of facilities may subject person to trespassing notice from the UMass Police Department.
