The Classification of Periodontal Disease: An Update for 2019~ Sponsored by Dentsply Sirona

Bill GaudieBill graduated BDS(Otago) with Distinction and, following a period in both hospital and private practice, returned to Dunedin graduating MDS(Otago) in Periodontology with Distinction and then gaining FRACDS. Bill has been in specialist periodontal practice in Wellington since 1988. He has lectured widely to dentist, hygienist and specialist groups; has been a longstanding member of NZDA with involvement in both Branch and national committees; and is a longstanding member of local and international periodontal groups.

Katy McLaughlinKaty completed her BDS (Otago) in 2008 and worked at Dunedin hospital and in private general practice in Auckland before completing her DClinDent (Otago) in Periodontics in 2015. She has previously worked as a periodontist in both Nelson and Wellington and is now permanently based at Capital Periodontics and Dental Implants with Dr William Gaudie. Katy is passionate about periodontal health as the foundation to her practice, and also has a keen interest in the art and science of soft tissue grafting and dental implant placement and maintenance.

Frances RuddimanFrances graduated from the University of Otago with a BDS in 2011. She spent two years working as a Dental and Maxillofacial House Surgeon in Christchurch and Hawke’s Bay Hospital, which developed her interest in surgery and the management of medically complex patients. Following a year in private practice in Christchurch, Frances returned to Otago to complete her three years of specialist training in the field of Periodontology and Dental Implants. She has been working at Wellington Periodontics with DJ and at Hutt Dental Specialists with Morris since early 2018 and also travels to Palmerston North to provide periodontal care to patients in the Manawatu. Frances is an active member of dental and periodontal organisations including the International Team for Implantology (ITI), The New Zealand Dental Association, The New Zealand Society of Periodontology, and The Australasian and New Zealand Academy of Periodontists.

Morris WongMorris was born and raised in Palmerston North, and graduated with a BSc from Massey University in 1985. Following a post-graduate Diploma, he entered the School of Dentistry at Otago University, where he completed a BDS in 1990. His first year working was as Dental House Surgeon at Napier Hospital followed by locum dental positions in Private dental practice. In 1992 Morris travelled to Hong Kong, primarily to study language and to experience the Chinese culture. What started as a 6-month visit eventuated into a 7 year stay, where Morris worked in a private medical clinic as well as completing his Masters in Periodontology. Towards the end of this period Morris headed a dental team as part of a 10-week medical project in the North West of China.

Morris returned to New Zealand in 2000 and gained his specialist registration as a Periodontist. The next 15 years were spent working in a group practice in Wellington specialising in Periodontics and Dental Implants, and building up the Lower Hutt practice. Currently Morris is working full time in Lower Hutt, and serves as visiting Periodontist to Hutt Hospital, where he works primarily with the combined Cleft Lip and Palate team. The practice is also developing a partnership with Marine Reach to provide dental care in rural areas of Fiji.

Morris practices all aspects of periodontics and dental implants, and is a member of the New Zealand Dental Association, New Zealand Society of Periodontology, Australia New Zealand Academy of Periodontology, Royal Australasian College of Dental Surgeons, American Academy of Periodontology, Australasian Osseointergration Society, and the International Team for Implantology. Spare time is usually spent with family, involvement in the Anglican Chinese Church and writing periodontal reports. Current interests are skiing, snow boarding, tramping, running, basketball, and music. Morris is married, and has 3 children aged 17, 15 and 12.

DJ ZanicottiDJ graduated with a BDS from Tuiuti University (Brazil) in 2004, and has been a specialist in Periodontics since 2006 (Faculdade de Odontologia de Piracicaba

- Unicamp, Brazil). He also has a specialist degree in Dental Implantology and Masters Degree in Clinical Dentistry (Dental Implantology) from Universidade Positivo, Brazil (2010), and a PhD from the University of Otago, New Zealand (2015). He is an active member of numerous national and international dental organisations including the International Team for Implantology (ITI), American Academy of Periodontology, The New Zealand Dental Association, The New Zealand Society of Periodontology, and The Australasian and New Zealand Academy of Periodontists. DJ has presented lectures and surgical training programmes and his literature contributions include conference posters and presentations, scientific articles, and a textbook chapter. DJ is the leading Periodontist at the Wellington Periodontics and Dental Implants.

President’s Report

Dr Gavin Cho President

After attending a thoroughly enjoyable NZDA Conference in Auckland last year, I am really looking forward to another great NZDA Conference but this time on our home turf. While the conference is held early in the week (Monday 29 - Wednesday 31 July) compared to typically later in the week (due to Wellington’s shortage of conference venues and available dates), there’s the benefit of not having our valuable weekends shortened by conference lectures!

We have a significant number of local presenters contributing to the academic content, and Wellington’s craft beer and local music talent will feature on the two social evenings.

There will be a number of Wellington submissions for the NZDA awards so make sure that that you and your better halves come along for a great night out at Te Papa in support of our nominees.

It would be great to be able to demonstrate the unity within the Wellington Branch by our attendance and support by all dentists and dental practice staff to this years conference! Go onto the NZDA website and make sure you register before the Early Bird cut off date of 3rd June.

Our line up of excellent monthly branch meeting presentations continues following last months delivery by Dr Teekayu Jorns who provided us with an insight as Oro-facial Lead Consultant of a multi-disciplinary centre for the management of Oro-facial Pain in Northern Thailand. He has offered his slides to the branch which will be added to our website. Thanks to Jo Ferguson for making this happen.

This month, we are having our branch meeting in Lower Hutt so we expect to see more Hutt Valley colleagues at the pre-dinner drinks (which is kindly sponsored by Dentsply) and also for dinner. I’m told that we will see a full line up of our five local Periodontists so we are in for a treat!

We invite all hygienists and oral health therapists to this meeting.

NZDA Wellington Committee Meeting Minutes

MINUTES OF LAST MEETING:Moved: Dr Hitchings Seconded: Dr Ruddiman

MATTERS ARISING:Unfortunately Drs McKelvey and Hitchings have been unable to meet to review and suggest changes to the ‘Rules of the Branch’ however Dr Hitchings has reviewed them and will speak about her suggestions in General Business.

NZDA Awards: It was agreed that Dr Ichim will submit an application under the category of ‘Branch Initiative’ for our successful Mural project where we engaged with the local Community.

Dr Excell will nominate Dr McKelvey under the “Outstanding Young Dentists” category

Dr Excell will confirm the eligibility for the “Public Service” and the “Outstanding Colleague” awards and advise the Committee if any suggested members would be able to be nominated.

Dr Ichim updated the meeting on the Water Kits as there are only 6 kits left.

FINANCIAL REPORT:Dr Hitchings advised the Committee that the invoicing software she uses is not compatible with Windows 10 and her email. The invoicing software package is an old programmes and is not compatible with Xero. Dr Hitchings is looking at options to make it workable and will keep the Committee updated.

GENERAL BUSINESS:

Branch meetingsThere was discussion around the offer from GSK to present at the July meeting however it was agreed that this was not a topic that the Committee felt was relevant however they may wish to hold a separate evening for Dentists in the Wellington area who may be interested. Dr McKelvey will inform GSK.

Discussion then moved to the need for a presenter for the July Branch meeting and the preference of NZDA that Branch meetings do not impact members around the time of Conference it was confirmed and agreed with Sarah Ting that she would present at the July meeting we would have no Branch meeting in August and the new Grads would move to October.

Rules of the Wellington BranchDr Hitchings as reported earlier had reviewed the “Rules of the Branch” and had initially identified some specific areas that should be updated and following discussion it was agreed that these and any other areas that either Dr McKelvey or Dr Hitchings identify as being in need of change should be sent to Dr Excell so he can take to NZDA to confirm there was no objection it should be circulated again to the Committee then to the Branch for ratification.

• Needs to include the minutes being recorded electronically and a paper copy but not in the Minute Book.

• The Young Dentist Rep should be a full voting member of the Committee (Dr Smith will confirm with her colleagues that this is what happens in other Branches)

• With the above change the number of Committee members should reduce to 7.

• Members are able to give a proxy vote to a member of their choice for voting at the AGM if they are unable to attend in person.

• The Annual Audit of the accounts is to be re worded to be ‘review of the accounts’

NZDA SUBMISSION REMOVAL AGE LIMIT RESTORATIVE TREATMENT OHTIt was agreed that Dr Excell will write a Submission on behalf of the Committee and members were also encouraged (as per the email that Dr Ichim sent out earlier this week) to write submissions.

SPEAKERS FOR 2020Dr M Wong informed the meeting that planning was underway for next year’s Branch meeting speakers and he asked if members had any suggestions to please contact him.

Diagnosis & Management of Non-Odontogenic Pain

A/Prof Jorns came and talked to the branch about the diagnosis and management of non-odontogenic pain. A/Prof Jorns began his presentation talking about the classification of orofacial pain and temporomandibular disorder (TMD). He discussed the aetiology and epidemiology of TMD.

The RDC-TMD is a diagnostic criteria for research and clinical purposes. It uses two axis for TMD – Axis I (patients with masticatory muscle pain with or without limitation of mouth opening; patients with internal derangement of the TMJ; patients with arthralgia, ostearthritis, and osteoarthrosis) and Axis II (TMD related – pain, parafunctional behaviours, psychological distress and psychosocial dysfunction).

Occlusion is no longer considered a primary factor in the multi-factorial nature of TMDs. Malocclusion may be a consequence of TMD rather than a cause. Management strategies of TMD follows a biopsychosocial model. A/Prof Jorns then went through the evidence behind various management strategies.

A/Prof Jorns then discussed the diagnostic criteria and mangagement for trigeminal neuralgia. The 2004 diagnostic criteria for trigeminal neuralgia by the International Headache Society (IHS) as:

• Paroxysmal attacks of pain (fraction of a second to 2 minutes)

• Pain has at least one of the following characteristics » Intense, sharp, superficial or stabbing » Precipitated from trigger areas by trigger factors

• Attacks are stereotyped

• No clinically evident neurological deficit

• Not attributed to another disorder

A/Prof Jorns has kindly provided the branch with a copy of his presentation, this will be on the branch website.

His take home messages were:

• Chronic orofacial pain – mostly can’t be cured.

• Chronic pain is managed all the same – no matter where the pain is

• Rule out the cause of pain – correct diagnosis!

• Use evidence-based management

• Use multidisciplinary non-invasive and reversible therapy

• Always looks at the psychosocial side of patient.
