Congratulations! You have been selected as an official contestant in the 2012 MISS RHODE ISLAND USA® pageant. We have received hundreds of applicants throughout the State of Rhode Island and we feel that you have the necessary qualifications based on your photograph, telephone interview and the recommendations of our selection committee to become the new Miss Rhode Island USA 2012! We look forward to working together with you for an unforgettable life-enhancing experience. The MISS RHODE ISLAND USA® competition offers area young women a once-in-a-lifetime opportunity to advance their personal, professional and humanitarian goals.

We have an incredible weekend planned for you. Plenty of fun ... making new and lasting friendships ... OVER $1 MILLION IN COLLEGE SCHOLARSHIPS, PRIZES AND AWARDS ... MODELING ... THE GLAMOUR … THE EXCITEMENT ... and the crowning of the new Miss Rhode Island USA. If you are crowned Miss Rhode Island USA, the most thrilling and exciting part of the reign is a two-week expense paid trip to compete for the prestigious title of Miss USA. The MISS UNIVERSE® and MISS USA® pageant is an NBC Universal Inc. and Donald J. Trump joint venture and consistently ranks among the most-watched television programming in the world. The MISS USA® pageant is sponsored by some of the most renowned companies in the world, including, Sherri Hill, New York Film Academy, Farouk Systems Haircare, Diamond Nexus Labs, Kandy Wrappers Swimwear and Chinese Laundry Footwear, to name a few.

This is truly an exciting and wonderful opportunity for you, but a lot of work must be done before you walk out on stage and meet the spotlights. Each contestant is responsible for raising a sponsorship. This sponsorship will help cover all your expenses during the Pageant weekend, as well as contributing to the cash and prize awards. The only other requirement is a $99.00 non-refundable registration fee. We understand the time and effort involved in raising sponsors, so we urge you to send in your $99.00 registration fee today.

Once we receive your $99.00 registration fee, along with the Registration Application and signed Preliminary Agreement, we will then mail you your Official State Finalist packet, and you can get started. Your State Finalist packet will include all the materials you will need to prepare for the 2012 Miss Rhode Island USA pageant. You will not be considered a state finalist until you have registered. The sooner your registration fee is in, the sooner you can concentrate on the competition, so start now!

Each State Finalist will be judged in three areas: Swimsuit, Evening Gown and Interview. During the competition, you will be asked to model an evening gown and swimsuit of your choice. You will also be individually interviewed by a panel of judges. These areas of competition are equally weighed and designed to help showcase your ability to serve as a role model to your peers. You will have the opportunity to demonstrate your stage presence, poise, physical fitness, personality, and communication skills. You will receive more information on wardrobe guidelines and be given detailed information regarding your preparations in your Official State Finalist packet.

Our program is an excellent vehicle for young women who are recognized as the elite of today and tomorrow. These young women are savvy, goal-oriented and aware. The contestants who become part of the Miss Universe Organization display those characteristics in their everyday lives, both as individuals who participate in the competitions to advance their careers and personal goals, and as women who seek to improve the lives of others. Many contestants and titleholders have had many new doors open and opportunities come their way just by participating in the pageant. Internships with major news organizations, modeling contracts, humanitarian aides, actresses and spokes models are just a few examples of their accomplishments after competing.

Participating in this pageant will be fun and exciting as well as provide an opportunity to develop your interests, aspirations, interpersonal skills and increase self-esteem. We hope that each young woman will leave with

Utilizing its nationwide grass roots infrastructure, the Miss Universe Organization is committed to increasing awareness of breast and ovarian cancers. The historic 60th annual MISS USA® pageant was televised LIVE on NBC from the Planet Hollywood Resort and Casino in Las Vegas on Sunday, June 19, 2011 and was the #1 ranked broadcast television program of the evening. In front of over 7.2 million viewers, Alyssa Campanella, the beautiful 21 year-old representing California and our former Miss New Jersey Teen USA 2007, was crowned Miss USA 2011! Alyssa will go on to represent the USA at the 2011 MISS UNIVERSE® pageant, LIVE from São Paolo, Brazil on September 12, 2011. “Bravo’s” own Andy Cohen and Giuliana Rancic of “E! Entertainment” hosted the event joined by celebrity judges Caroline Manzo, from “The Real Housewives of New Jersey”, Hip-hop artist and “Celebrity Apprentice” star, Lil Jon, and one of the elite defensive centers for the NBA champion Dallas Mavericks, Tyson Chandler, to name a few. Color commentator Kelly Osbourne was on hand with her thoughts and British rapper, Tinie Tempah and singer Eric Turner performed their smash hit “Written in the Stars.” Our own Miss Maryland USA, Allyn Rose was a Top 8 Finalist!

We are extremely proud of Kate McCaughey, Miss Rhode Island USA 2011, who represented the state of Rhode Island so beautifully! The 51 contestants all received the star treatment while in Las Vegas. Appearing throughout the city and filming for the telecast was a thrilling opportunity for all of the contestants. The excitement and memories of their time during the MISS USA® pageant will last a lifetime. Lindsey Bucci, Miss Rhode Island Teen USA 2011, competed at the 2011 MISS TEEN USA® pageant, which once again took place at the luxurious Atlantis Resort on Paradise Island in the Bahamas on July 16, 2011, viewed by a worldwide webcast audience!! Our own Miss Maryland Teen USA, Kirsten Nicholson and Miss New Jersey Teen USA, Savannah Schechter were Top 15 Finalists and Savannah also received the prestigious Miss Congeniality award! The star-studded panel of judges chose Danielle Doty from Texas as Miss Teen USA 2011! We are so excited about the many opportunities that lie ahead for all our titleholders, and the memories from their national pageants will stay in their hearts always. Joining in the excitement of a major State Pageant can be very rewarding for a young lady. It will enable her to meet new friends, enjoy an atmosphere of glamour and fashion, and develop the poise, confidence, interpersonal and career-enhancing skills which in later life will be respected and even admired!

We review hundreds of applicants throughout the State of Rhode Island and we feel that your daughter

has the necessary qualifications based on her photograph, telephone interview and the recommendations of our selection committee to become the new Miss Rhode Island USA 2012!

Our 2012 MISS RHODE ISLAND USA® pageant is the Official Preliminary to the 2012 MISS USA®

pageant. The MISS TEEN USA® and the MISS USA® pageants are a Donald J. Trump and NBC Universal joint venture. Since MISS USA® began in 1952 and MISS TEEN USA® in 1983, tens of thousands of women in the fifty states and the District of Columbia have contributed to making the competition a national tradition. The winner of the 2012 MISS USA® pageant will go on to compete for the prestigious title of MISS UNIVERSE®.

Unlike many pageants, the 2012 MISS RHODE ISLAND USA® pageant does not require a performing talent, a platform, an essay, or proof of community service. Rather, our program is designed to discover the poise, personality and beauty within your daughter. All our participants will be given instruction to enhance their pageant interview skills, and will work in groups and, where requested, one on one with our producers and directors on runway and stage presentation techniques. For many thousands of young women who have participated in the Official Preliminaries of the MISS TEEN USA® and MISS USA® pageants, the experience will be remembered for a lifetime.

The 2012 MISS RHODE ISLAND USA® pageant will be held at the Rhode Island Center for

Performing Arts in Cranston, RI before a large audience. The contestants will be chaperoned and their meals will be provided at the center by the MISS RHODE ISLAND USA® pageant.

TO QUALIFY:

You must be a naturally born female and a U.S. citizen. You must have lived in the state of Rhode Island for at least 6 months or be a full-time

student within the state. You must be at least 18 years of age but still under 27 on February 1, 2012. You must never have been married, had a marriage annulled, be a parent, or have given

birth to a child, nor be pregnant. WARDROBE REQUIREMENTS FOR STATE PAGEANT:

One (1) floor-length evening gown One (1) one or two-piece conservative swimsuit, preferably a solid color One (1) interview outfit – a nice suit, dress, or other stylish outfit One (1) opening number outfit – black cocktail dress with black, silver, or gold heels

As a state finalist, you will be our guest and the guest of your sponsors for two big days

beginning September 17, 2011. We promise fun…excitement…something of value. You will make new and close friends…receive direction and guidance in runway presentation, speech and stage techniques…work with a show producer, director and choreographer. You will take part in both stage shows – Saturday, September 17th is the Presentation Show and Sunday, September 18th is the actual pageant when we will crown our new Miss Rhode Island USA 2012!

REGISTRATION

Now that you have been selected as a contestant, your next step is to register. There is a $99.00 non-refundable registration fee. Once we receive your registration fee, we will mail you your MISS RHODE ISLAND USA® State Finalist Packet. This packet is filled with lots of important information including your sponsor forms so that you can get started. Attached you will find a Registration Application form and Preliminary Agreement included in these materials. You will need to complete and sign these forms and mail them back to us along with your $99.00 registration fee. For your convenience, you may also submit the registration fee online using PayPal. You will find that feature in the Contestant Corner section of our website. Please remember that in order to complete the registration process, you must also mail the Registration Application and Preliminary Agreement to the pageant office. Your deadline to return the required paperwork and fee is within 7 days upon the receipt of your e-mail acceptance notification. In producing a show of this caliber, it is essential that our Official State Finalist roster be completed as soon as possible. Please make every effort to meet your deadline. However, if you run into a problem, do not hesitate to contact us at 401.228.7019 or [email protected] so that we may hold your spot. SPONSORSHIP FEE:

Once you have registered as an Official State Finalist, there is a sponsorship. The sponsorship covers your activities during the entire course of the pageant. This includes a pageant orientation seminar (information to follow), an incredible pageant experience at the Rhode Island Center for Performing Arts, meals, production costs, advertising costs, printing costs, prizes and awards, and of course sending our new Miss Rhode Island USA 2012 to the national 2012 MISS USA® pageant! The sponsorship program is wonderful because you should be able to enter this pageant at little or no cost to yourself or your family, and still be part of an opportunity of a lifetime. The sponsorship is $795.00 for our State Finalists, and it is not due now. You will receive sponsorship guidelines and your specific due dates in the next packet you will receive from the pageant offices once you’ve finished registering. As an added incentive for our new contestants, if you submit the total amount of your sponsorship on or before your first installment deadline, you will receive a $100 discount off the entire fee, bringing your total amount due down to $695.

Your sponsorship may come from businesses, civic clubs, family, friends and neighbors. You may sponsor yourself, or have as many sponsors as you wish. As a State Finalist, we will send you a Sponsorship Packet that will include specific details and forms that will help you obtain your sponsorship. Ample time will be provided for you to secure your sponsorship. Sponsorship has never been a problem to a dedicated State Finalist, but you have to work at it. If you want to…you can! It could pay off in ways you’ve never dreamed! GOOD LUCK!

I hereby apply as an entrant in the 2012 MISS RHODE ISLAND USA® or 2012 MISS RHODE ISLAND TEEN USA® pageant (together, the “State Pageants”) and the opportunity to compete in the national finals of the MISS USA® or MISS TEEN USA® pageant, as the case may be, and I represent to and agree with the State Pageants as follows: REPRESENTATIONS: 1. MISS: I will be at least eighteen (18) and under twenty-seven (27) years of age on February 1, 2012. 2. TEEN: I will be at least fourteen (14) and under nineteen (19) years of age on February 1, 2012.

3. I am not married, have never been married, and have never had a marriage annulled. I have never given birth to a child and am not now pregnant nor am I a parent of an adopted child. If any of these conditions change, I will notify the State Directors immediately, and I understand that this may affect my eligibility.

4. I have never participated in a previous national MISS USA® or MISS TEEN USA® pageant.

5. I have not participated and will not participate in any other state’s preliminary pageant to the 2012 MISS USA® pageant or the 2012 MISS TEEN USA® pageant, including District of Columbia.

6. Prior to the date of this Application and Agreement, I have not been engaged in any activities which, if disclosed to the public, would bring or tend to bring myself or the MISS RHODE ISLAND TEEN USA® or the MISS RHODE ISLAND USA® pageant into disrepute, ridicule, or contempt. I am in good health and of good moral character.

7. I am a naturally born female and a citizen of the United States and at least one of the following is true: (a) I now reside and have resided in the State of Rhode Island as my permanent and primary residence for a period of at least six (6) months immediately prior to the beginning of the 2012 MISS RHODE ISLAND TEEN USA® or the MISS RHODE ISLAND USA® pageant which I have entered (although I may have attended college or university elsewhere); or (b) I have resided in the State of Rhode Island as a full-time student at a college or university for one entire semester, trimester or summer session immediately prior to the beginning of the 2012 MISS RHODE ISLAND TEEN USA® or the MISS RHODE ISLAND USA® pageant, and I am still enrolled as a full-time student at my school, college or university and will continue to reside in the State of Rhode Island (although I may have spent or be spending the summer in another state).

8. Neither I nor any member of my immediate family or anyone living in my household is currently employed by or has been employed within the last two (2) years by, or is a close acquaintance of anyone employed within the last two (2) years by MISS UNIVERSE L.P., LLLP, any State Director in the MISS USA® and/or MISS TEEN USA® pageant system or any company or enterprise that the Director may own or control, the National Broadcasting Company, Inc., Ernst & Young, Mr. Donald J. Trump, Trump Pageants, Inc., The Trump Organization, or any of the subsidiaries, affiliates or related companies of the foregoing, including without limitation, NBC Studios, Telemundo Network Group, CNBC and MSNBC. (IF YOU OR ANY MEMBER OF YOUR IMMEDIATE FAMILY IS EMPLOYED BY ANY OF THE FOREGOING OR ANY FIRM OR CORPORATION CONTROLLED OR AFFILIATED WITH ANY OF THEM, THEN YOU MUST NOTIFY YOUR STATE DIRECTORS IMMEDIATELY IN WRITING, SO THAT YOUR ELIGIBILITY MAY BE DETERMINED.)

AGREEMENTS APPLICABLE TO ALL CONTESTANTS:

9. I agree that if I win or succeed to the title of MISS RHODE ISLAND USA® or the MISS RHODE ISLAND TEEN USA®, I will participate in the MISS USA® and/or MISS TEEN USA® pageant, as the case may be, and abide by all the rules and regulations governing that pageant. If I win the MISS USA® pageant, then I agree to participate in the MISS UNIVERSE® pageant and abide by the rules and regulations governing that pageant.

10. I understand that the sponsorship requirement is due in full prior to the first day of the state pageant and agree that I am free to withdraw from the MISS RHODE ISLAND USA® or MISS RHODE ISLAND TEEN USA® competition at any time prior to my being crowned as MISS RHODE ISLAND USA® or MISS RHODE ISLAND TEEN USA®, but under no circumstances will I be entitled to a refund or to transfer to another contestant any fee or sponsorship money, either in full or in part, that I or my sponsors have paid. Likewise, if for any reason I shall be disqualified from the MISS RHODE ISLAND USA® or MISS RHODE ISLAND TEEN USA® pageant, under no circumstances will I be entitled to a refund or to transfer to another contestant any fee or sponsorship money, either in full or in part, that I or my sponsors have paid.

11. I agree that if I win or succeed to the title of MISS RHODE ISLAND USA® or MISS RHODE ISLAND TEEN USA®, MISS USA® or MISS TEEN USA®, I will not participate in any pageants during the time that I hold either of those titles unless authorized in writing by the State Pageants and MISS UNIVERSE L.P., LLLP.

12. I HEREBY RELEASE, UNCONDITIONALLY AND FOREVER, ANY CLAIM AGAINST THE STATE PAGEANTS, MISS UNIVERSE L.P., LLLP, AND THEIR RESPECTIVE PARENTS AND AFFILIATED OR RELATED COMPANIES AND OTHER RESPECTIVE OFFICERS, DIRECTORS, PARTNERS, EMPLOYEES, AGENTS AND ASSIGNS, WHICH I MAY HAVE BY VIRTUE OF MY PARTICIPATION IN MY STATE PAGEANT OR BY ANY USE OF MY NAME, LIKENESS, VOICE, AND/OR BIOGRAPHY IN CONNECTION WITH MY STATE PAGEANT, INCLUDING USE IN PROMOTIONAL AND ADVERTISING MATERIAL.

13. I understand and agree that all issues as to my eligibility or that of other contestants shall be determined by the State Pageants and MISS UNIVERSE L.P., LLLP, and I agree to abide by that determination both as to myself and other applicants. Any decision to disqualify any contestant or titleholder shall likewise be made by the State Pageants and MISS UNIVERSE L.P., LLLP, and I agree to abide by that determination both as to myself and other applicants.

14. I agree to abide by all the rules of the MISS RHODE ISLAND TEEN USA® or the MISS RHODE ISLAND USA® pageant and the MISS USA® or MISS TEEN USA® pageant now in effect or as announced from time to time hereafter.

15. I understand and agree that my acceptance as a contestant for the title of MISS RHODE ISLAND TEEN USA® or MISS RHODE ISLAND USA® and my continued consideration for the title of MISS RHODE ISLAND TEEN USA® or MISS RHODE ISLAND USA® is under the sole and complete discretion of the State Directors of the MISS RHODE ISLAND TEEN USA® or MISS RHODE ISLAND USA® pageant.

16. I agree to permit my photograph, voice recording, and name to be used without charge for all publicity and commercial purposes and any telecasts of the state and national pageants.

17. I agree that, if I win or succeed to the title of MISS RHODE ISLAND TEEN USA® or MISS RHODE ISLAND USA®, the terms of this Application and Agreement, including, without limitation, Sections 17 through 22, inclusive, shall continue to be binding upon me, and that any breach thereof shall, in addition to any other right or remedy of the State Pageants (including without limitation my disqualification, loss of title and return of prizes), entitle the State Pageants to equitable relief and the costs and expenses of obtaining the same, including reasonable attorneys’ fees.

18. I understand and agree that if I win or succeed to the title of MISS RHODE ISLAND TEEN USA® or MISS RHODE ISLAND USA®, as one of my obligations, I will go to and stay in the host city during the time required for me to appear in the MISS USA® or MISS TEEN USA® pageant.

19. I understand and agree that if I win or succeed to the title of MISS RHODE ISLAND USA® or MISS RHODE ISLAND TEEN USA®, all decisions concerning my preparation for the MISS USA® or MISS TEEN USA® pageant shall be made by my State Directors. To this end, and by way of example and not limitation, I agree (i) to utilize exclusively the services of the staff of the MISS RHODE ISLAND USA® or MISS RHODE ISLAND TEEN USA® pageant, and each pageant sponsor in connection with my preparation for the MISS USA® or MISS TEEN USA® pageant; (ii) to wear, during the evening gown competition of the MISS USA® or MISS TEEN USA® pageant, a gown provided by the pageant sponsor; (iii) not to engage or continue the services of or otherwise consult with (for monetary consideration or otherwise) any person, firm or other entity providing services in competition with a pageant sponsor; or (iv) not to engage or continue to use or consult with any interview, stage presentation, or other pageant coaches. I understand and agree that any waiver of the provisions hereof shall require, in each instance, the prior written consent of my State Directors. Any consent given shall be for the specific instance requested and shall not be deemed consent to any other request or instance.

20. I represent and agree that, between the date of this Application and Agreement and the MISS RHODE ISLAND TEEN USA® or MISS RHODE ISLAND USA® competition, I have not and will not engage in any activities which, if disclosed to the public, would bring or tend to bring, myself or the MISS RHODE ISLAND TEEN USA® or MISS RHODE ISLAND USA®

pageant into disrepute, ridicule or contempt.

21. I understand and agree that if I win or succeed to the title of MISS RHODE ISLAND TEEN USA® or MISS RHODE ISLAND USA®, my failure to comply with any of the terms, provisions, restrictions or obligations hereof shall, at the option of the State Directors of the MISS RHODE ISLAND USA® or MISS RHODE ISLAND TEEN USA® pageant, result in all or any of my disqualification, loss of title, and my obligation to return immediately any and all prizes awarded.

22. I understand and agree that if I win or succeed to the title of MISS RHODE ISLAND USA® or MISS RHODE ISLAND TEEN USA®, I will not permit the use of my name, photograph, image, or title as MISS RHODE ISLAND USA® or MISS RHODE ISLAND TEEN USA® by anyone without the prior written consent of my State Directors.

RELIANCE BY PAGEANT; EFFECT OF SIGNATURE

23. I understand and agree that the State Pageants are relying upon my representations and agreements set forth in this Application and Agreement as a material inducement to accepting my Application, and allowing me to compete for the title of MISS RHODE ISLAND USA® or MISS RHODE ISLAND TEEN USA®. Any representations which are untrue or false in any respect, both when made and as of the date of the MISS RHODE ISLAND USA® or MISS RHODE ISLAND TEEN USA® pageant, or failure to comply with any agreement hereunder or any rule or regulation of the MISS RHODE ISLAND USA® or MISS RHODE ISLAND TEEN USA® pageant of which I have had notice, may result in my disqualification from the MISS RHODE ISLAND USA® or MISS RHODE ISLAND TEEN USA® pageant.
