life insurance

• For employees who wish to supplement or replace their life insurance with a

Foresters Life Insurance Certificate and pay life insurance premiums right out of

their paycheck, Foresters has entered into an agreement with Capital

Administrative Services (CAS)

• CAS will collect Foresters premiums via payroll deductions and remit payments

to be applied to individual Employee’s life insurance contracts

•  STEP 1: Print off and complete the LIST BILL PRE-APPROVAL Form (note: to be

considered for Foresters List Bill Services, a prospective business must have

at least 3 issued, active Foresters Life Insurance Certificates)

• STEP 2: Email completed Foresters List Bill Pre-Approval Form to Foresters;

• STEP 3: Once approved, you will receive a confirmation of approval decision

via email within 3 business days

•  Step 4 IMPORTANT: Once Foresters approves your case, please contact CAS

at 1-800-780-3100. A CAS representative will work with you to prepare you for

the Employer meeting

• STEP 6: Complete Foresters Application for Individual Life Insurance (Federal

Employee or List Bill Only) and required forms. PLEASE NOTE: this is a

different application than Foresters General Application for Individual Life

•  STEP 6: Complete the Foresters Application form

– CAS and you will determine the Foresters Contract ISSUE MONTH. The

ISSUE MONTH will be the 15th of a future month depending on the

Employees pay schedule.

– CAS will collect at a minimum one month’s premium before remitting

payment to Foresters, hence all Proposed Issue Dates must be at least a

month (probably more) from the date of the application.

– Foresters Application for Individual Life insurance (Federal Employee or

List Bill Only) (and any other required forms);

•  List Bill Cases – Advanced Commissions

•  As part of our support, Foresters has developed an advanced commission

process for Producers submitting List Bill Cases.  

• For approved List Bill Cases, 40% of a Producer’s First Year Commission will

be advanced upon approval of the individual applications. The balance of the

advance paid upon receipt and processing by Foresters of the first premium

(which may take four weeks).

•  NMO principals must request Producers to be considered for the Advanced

Commission process. Requests can be made by emailing ForestersListBill.com

or by the Request Form posted on the Foresters List Bill website.
