    Writing Assessment Procedures

    Around 8:55 AM I (Andrew Ackley) went down to the library to pick up the packets for

    the classes I was administering the test to. In the library I picked up the packets for each class,

    Jennifer Johns English class and Vol Lindseys English class. Before leaving the library I counted

    each answer sheet for each class, each direction sheet, and each prompt. I apparently did not

    count the prompt sheets correctly at this point. Believing I had the correct count I signed out

    each packet and headed to the classroom.

    Upon entering the classroom students were instructed to put all books, book bags, and

    drinks at the back of the classroom and to have a pencil and blue or black pen out for the test.

    Those that did not have a pen or pencil were given one. Students were then placed into their

    seats alphabetically by class and those students absent were circled on the roster. We had

    three students absent from Mr. Lindseys class.

    The test answer sheets were then given out by class to each student making sure each

    answer sheet went to the correct student. Students were then instructed to fill in certain boxes

    on the answer sheet. After students had filled in the correct boxes the survey was read aloud

    so each student could complete the survey. The direction sheets were then passed out by Mrs.

    Prince by row and each student received a direction sheet and we had three direction sheets

    leftover as expected. I then read aloud the directions as the students read along silently. At this

    point I was reading directly from my administrators direction sheet. As the directions were

    being completed, Mrs. Prince then passed out the prompt sheets by row counting how many

    she needed per row.

    The prompt sheets were then read aloud by me (Andrew Ackley) while students

    followed along silently. Students were then instructed that they had 35 minutes to write on the

    topic. I asked if there were any questions which there were none and students began writing. It

    was about 9:30 AM at this point. The remaining prompt sheets were returned to me but we

    only had two prompt sheets instead of the expected three prompt sheets.

    At the conclusion of the test the students were instructed to place their direction sheet

    and prompt sheet inside of their answer sheet. Then I went around alphabetically picking up

    each answer sheet, direction sheet, and prompt sheet from Mr. Lindseys class as Mrs. Prince

    did the same procedure for Mrs. Johns class. Knowing we were one prompt sheet short we

    asked students if anyone had an extra copy but no one did. So Mrs. Prince and I went through

    each answer sheet to look for an extra prompt and no extras were found. I informed Mrs.

    Smith of this issue and again we went through each answer sheet, asked students and looked

    around the classroom but did not find the third prompt sheet.

    At a later time the missing prompt sheet was found at the alternative school for a

    student that was in one the classes I was giving the test to. That particular student had two

    prompt sheets. This accounts for the missing prompt sheet from my classroom. The error was

    made in the initial counting of the prompt sheets in the library.
