Halstad Living Center is currently looking for a full-time RN Care/MDS Coordinator. Every 3rd weekend, every 3rd holiday. Credit given for experience. Duties include: Resident Assessment Instrument (RAI)/MDS process for assigned residents, including: monitoring the assessment schedule in collaboration with Health Information staff; timely and accurate completion of Minimum Data Set (MDS) nursing assessments; and coordination of completion of other MDS sections with Interdisciplinary team (IDT) members. Updates care plans regularly in collaboration with the Interdisciplinary team (IDT). Also utilizing nursing skills, observing residents’ health, and communicating with doctors, residents and their families. Works closely with team members to develop and implement a nursing discharge plan.Qualifications: Current Registered Nurse (RN) licensure in Minnesota; Must be able to pass a background check; CPR Certification; Resident Assessment Coordinator (RAC) training must be completed (if not already obtained) within six months of hire; 3 years’ experience as a registered nurse and 1 year experience with MDS is preferred.HLC will pay for employee’s monthly tuition payment for LPNs/RNs. Also, HLC will provide funding with schooling to further your education for BSN, RN, and LPN.
