1. Please provide the SunLife IF Booklet   .

2. Please provide Sun Life’s experience (36 to 60 months) reports including details and claims showing waiver information (date of birth, date of disability, and face amount), month to month lives, volumes and premiums. Please see attachments below and Appendix A.

3. What was the inception date with SunLife?

Inception dates: 9/1/09-9/1/10, 9/1/10-9/1/11, 9/1/11-9/1/12

4. Have there been any rate changes in Life/ ADD premiums?

Yes, during the policy period of 09/01/2009 – 08/31/2010, the rate went from 1.60 per month to 1.30 per month. The change was effective on 01/01/2010.

5. Please provide the Sun Life Certificate for Laredo ISD RFP and Rates for Employer paid Life and AD&D?

6. Please provide a Premium of Waiver Claims Report.

7. Please provide the rate history for all life lines, plan change history by enrollment.

9. What are the eligibility requirements for Life insurance and AD&D?

As RFP #12-008 indicates eligibility for coverage mirrors the requirements for the District’s medical plan. Therefore, any employees that work 20+ hours are identified as full-time employees and are eligible for coverage.

10. Should commissions be included in the proposals?

The premium pricing for coverage that the District receives should include all costs needed to provide specified coverage. The District does not set any type of commission rate.

11. Will the plan be self administered?

Yes, the District maintains all records and determines remittance amount on a monthly basis.

12. Please provide the RFP in Word format for ease of completing forms and questions.
