Yes, I want to enroll in identity protection coverage provided by Legal Resources. I agree to pay the monthly fee through an automatic withdrawal from my bank account or credit card and understand that these fees are due in advance of coverage. This annual membership shall renew automatically on the 12 month anniversary, unless Legal Resources is notified thirty (30) days prior to the expiration date. Legal Resources must receive this completed form by the 15th of the month prior toeffective month of coverage.
