Your First Page

Your first page is going to be your roster that will be used to weigh-in and certify your players. Players will need to be listed youngest to oldest and alphabetically A-Z within their age group.

This will be placed in the very first sleeve.

Content of Certification Book• Team Roster• Coaches Information (ID’s, USA Football certs, signed coaches

contract & CPR cert.)• Team Insurance form• Players registration form• Player Physical form• Birth Certificate (for new players)

Materials You will need:• One 3 ring binder• Approx. 30 Clear plastic/vinyl sleeves

Your Next Pages…

In the same sleeve as the roster, insert your Head coaches USA Football Cert. If you have all of your coaches you can put them as well, Head coach on top. In the next sleeve insert the signed copy of the Head Coaches Contract. The contact is in a sleeve by itself, we will need to see the signature on the back side.

Your Next Pages Con’t…The sleeve on the left is the backside of the Signed Coaches Contract. In the sleeve on the right insert a copy of the coaches current CPR card as well as a copy of the Head Coaches ID. If they have their League ID, a copy of that will be fine.

Your Next Pages Con’t…

The sleeve on the left is the backside of the Head coaches ID/CPR. In that same sleeve place copies of Asst. Coaches ID’s, Copies of league ID’s are fine. In the sleeve on the right insert a copy of your chapters current insurance form.

Your Next Pages Con’t…The next pages are the main part of your book. It will be your Player registration form, Player physical form and Birth Certificate (for new players only).

The following pages will show how to place and arrange the form into the sleeves so that they can be checked without having to remove or damage important documents…

Inserting Birth Cert. and Physical Form

The birth certificate is inserted flat and not folded. Your first birth certificate is placed in the same sleeve as the insurance form on the backside. Notice the holes in the sleeve are on the right side in this picture.

Next you will need to fold the player physical form so that the players name, on the top and the Doctors signature on the bottom, are visible and will fit on the lower part of the birth certificate.

Inserting Birth Cert. & Physical Form Con’t

If folded correctly you will be able to see the info needed from the birth certificate, the players info and the doctors sign-off from the physical form with out having to remove the documents.

Arranging forms in the book…The first player registration will go into the book next and be placed on the right hand side, opposite of the birth certificate and physical form for that player. This is so that the information can be checked without having to flip pages or remove doc’s.

The next players birth cert and physical form will be inserted into the backside of the first/previous players registration form and this will be continued through out the book to the final player.

The highlighted area of plastic around the picture on the registration form will need to be removed so that the form can by signed and stamped for certification.

Arranging forms in the book con’t…

For returning player you will not need a birth certificate. Simple place the players physical form where the birth certificate would have gone

Also for returning players, you will need to write on the edge of the registration form, the name of the chapter, the division and the team color that they played for last season. If the player played the year before last, but not last year, they will need a birth certificate.

All Forms Need to be CompletedMake sure all of your forms are completed and signed or they will not be certified.

Physical form must be signed and stamped by Doctor’s office and player must be cleared to play

Acceptable forms of age verification:- Passport/Immigration documentation- U.S. Military Birth Registration or ID card- Federal Government Birth Certificate- County Birth Registration- Letter from school the player is attending, attesting to the date of birth of the player, by his full name, to include the names of player’s parent(s) that are being carried on the school records. This letter is to be on school letterhead stationery, and signed by the principal, vice-principal, counselor, or registrar of the school. Letter must be dated.- Hospital record with the hospital seal and doctor's signature- Document must be original or certified copy
