REQUIREMENTS• You must be a 501(c)3 organization.• A physician (M.D. or D.O.) may place an order using our US Application and Order

Form for US clinics.• For clinics with a pharmacy license, a representative of the pharmacy may place

the order.• A dentist (D.M.D. or D.D.S.) may place an order using our Dental Application and

Order Form which has a specific list of medicines and supplies for those with a dental license.

• Any other person may order vitamins and supplies using our US Vitamins and Supplies Application and Order Form.

TO PLACE AN ORDER1. Go to www.blessing.org/order. 2. Complete the appropriate APPLICATION and ORDER FORM (must be completed

every time for each outreach).3. Submit order for approval (via fax or email).

PAYMENT OPTIONS• If you are a first-time customer, you must pay in advance (via check or credit card).• Returning and approved customers will be sent their medicines and then, at a later

date, an invoice via email. The invoice may be paid via check through the mail or credit card through Blessings’ online Self-Service Portal.

Important Note: Occasionally some medicines are on back order and will be delivered at a later date. You will be invoiced for those medicines or supplies separately.
